which may be more restrictive than County regulations. The undersigned assumes responsibility for compliance with any
applicable sieed restrictions.
UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES: If the owner has hired a contractor or
contractors to undertake work, they may be required to be licensed in accordance with state and local regulations. If the
contractor is not licensed as required by law, both the owner and contractor may be cited for a misdemeanor violation under
state law. If the owner or intended contractor are uncertain as to what licensing requirements may apply for the intended
work, they are advised to contact the Pasco County Building Inspection Division—Licensing Section at 727-847-8009.
Furthermore, if the owner has hired a contractor or contractors, he is advised to have the contractor(s) sign portions of the ,
"contractor Block" of this application for which they will be responsible. If you, as the owner sign as the contractor, that may '
be an indication that he is not properly licensed and is not entitled to permitting privileges in Pasco County.
TRANSPORTATION IMPACT/UTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands
that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings, change of use
in existing buildings, or expansion of existing buildings, as specified in Pasco County Ordinance number 89-07 and 90-07, as
amended. The undersigned also understands, that such fees, as may be due, will be identified at the time of permitting. It is
further understood that Transportation Impact Fees and Resource Recovery Fees must be paid prior to receiving a "certificate
�I of occupanc�' or final power release. If the project does not involve a certificate of occupancy or final power release, the fees
must be paid prior to permit issuance. Furthermore, if Pasco County Water/Sewer Impact fees are due, they must be paid
i prior to permit issuance in accordance with applicable Pasco County ordinances.
CONSTRUCTION LIEN LAW (Chapter 713, Florida Statutes, as amended): If valuation of work is $2,500.00 or more, I
certify that I, the applicant, have been provided with a copy of the "Florida Construction Lien Law—Homeowner's Protection
Guide" prepared by the Florida Department of Agriculture and Consumer Affairs. If the applicant is someone other than the
"owner", I certify that I have obtained a copy of the above described document and promise in good faith to deliver it to the
"owner" prior to commencement.
CONTRACTOR'S/OWNER'S AFFIDAVIT: I certify that all the information in this application is accurate and that all work will
be done in compliance with all applicable laws regulating construction, zoning and land development. Application is hereby
made to obtain a permit to do work and installation as indicated. I certify that no work or installation has commenced prior to
issuance of a permit and that all work will be performed to meet standards of all laws regulating construction, County and City
codes, zoning regulations, and land development regulations in the jurisdiction. I also certify that I understand that the
regulations of other government agencies may apply to the intended work, and that it is my responsibility to identify what
actions I must take to be in compliance. Such agencies include but are not limited to:
– Department of Environmental Protection-Cypress Bayheads, Wetland Areas and Environmentally Sensitive
Lands, Water/Wastewater Treatment.
– Southwest Florida Water Management District-Wells, Cypress Bayheads, Wetland Areas, Altering Watercourses.
– Army Corps of Engineers-Seawalls, Docks, Navigable Waterways.
– Department of Health & Rehabilitative Services/Environmental Health Unit-Wells, Wastewater Treatment, Septic
Tanks.
– US Environmental Protection Agency-Asbestos abatement.
– Federal Aviation Authority-Runways.
I understand that the following restrictions apply to the use of fill:
– Use of fill is not allowed in Flood Zone"V" unless expressly permitted.
– If the fill material is to be used in Flood Zone "A", it is understood that a drainage plan addressing a
"compensating volume" will be submitted at time of permitting which is prepared by a professional engineer
licensed by the State of Florida.
– If the fill material is to be used in Flood Zone "A" in connection with a permitted building using stem wall
construction, I certify that fill will be used only to fill the area within the stem wall.
– If fill material is to be used in any area, I certify that use of such fill will not adversely affect adjacent properties. If
use of fill is found to adversely affect adjacent properties, the owner may be cited for violating the conditions of
the building permit issued under the attached permit application, for lots less than one (1) acre which are elevated
by fill, an engineered drainage plan is required.
If I am the AGENT FOR THE OWNER, I promise in good faith to inform the owner of the permitting conditions set forth in this
affidavit prior to commencing construction. I understand that a separate permit may be required for electrical work, plumbing,
signs, wells, pools, air conditioning, gas, or other installations not specifically included in the application. A permit issued shall
be construed to be a license to proceed with the work and not as authority to violate, cancel, alter, or set aside any provisions
I of the technical codes, nor shall issuance of a permit prevent the Building Official from thereafter requiring a correction of
errors in plans, construction or violations of any codes. Every permit issued shall become invalid unless the work authorized
'I by such permit is commenced within six months of permit issuance, or if work authorized by the permit is suspended or
abandoned for a period of six (6) months after the time the work is commenced. An extension may be requested, in writing,
from the Building Official for a period not to exceed ninety(90) days and will demonstrate justifiable cause for the extension.
If work ceases for ninety(90) consecutive days, the job is considered abandoned.

Plan Review Comments
1. F.F.E. shall be a minimum of 8" above the road elevation and an engineered site plan.
2. Lots shall be graded to comply with R401.3 of the F.B.C.
3. Compaction test required if 24" or more of fill dirt is brought in at any one place.
4. Tie in survey required before pouring concrete.
5. Driveways require a R.O.W. use permit. A114 sides of driveway thru the sidewalk shall have
expansion material.
6. All setbacks shall be met.
7. All property markers shall be uncovered and marked at time of first inspection.
8. All A.D.A. requirements shall be met.
9. No electric, plumbing, mechanical, or framing shall be covered without an inspection and
approval first.
10. All Garages shall comply with section R309.2 of the F.B.C. (Fire Separation).
11. Appliances shall not be installed in a location where subject to mechanical damage unless

12. Water heaters shall comply with section P607.3 of the .F.B.0
13. Foundation supports for A/C units shall be raised at least 3" above finished grade. M1308.1

8.PREFERRED L.ENDERS: The BUYER(S)is not required to use one of Highlar�d Holdings',Inc.preferred lenders. The BUYER(S)
is free to obiain moRgage services from any source they choose. It should be noted that Highland Holdings,Inc.has a business
relatlonship with muitlple lenders. Because of this relationship,if you select one of our Preferred Lenders as your LENDER,that
selection may provide Highland Holdings,Inc.a financial or other benefit.However,by obtai�ing the BUYER'S MORTGAGE LOAN
through one of Highlend Holdings',Inc.preferred lenders. Highland Holdings,Inc.agrees to pay the following closing costs:

passing upon the credR of the Buyer(s},fall to provide an unconditional loan and/oriunding assistance approval in writing for financing,
subject only to receipt of an accepfabie appraisal, updaling the Buyers documents within 60 days of Closing and normal closing
conditions,then Seller may,at its option,terminate thls Purchase and Sales Agreement either verbally or in writing and then retum the
earnest money deposlt to the 6uyer(s). Upon retum of the deposft by Seller,ail obligations of the parties hereunder shail tertninate. If
conditional loan approval has been obtained,but the Buyer(s)alters their financiat condition in a way that disqualifies them irom final

responsible for alt such charges whether or not these charges were levied or assessed before or after Closing.
92.LANDSCAPING/DRAINAGE: Ail gradfng,fill,disposition of hees and control of water flow will be performed in a law(ul manner,
but otherwise will be completed at Sellers sole discretion. Certain areas of the Lot may be left in their natural state and may not be
landscaped in any way. At Closing,Seller's responsibility with respect to soit erosion,soil condftions,drainage,grass,shrubbery and
landscaping terminates and Buyer's responsibiliry begins. Selier is not liable for trees or lack thereof,or damages or destruction of
hees on the property and makes no warranty whatsoever as to the type,location or amount of trees which will be on ihe property
and/or the condition of those trees before,after or during constructlon.
13.LOTS: Each Lot is unique in ks size,shape and drainage characteristics. Buyer(s)understands and agrees that the size oi the lot
slope;the exect location of sidewalks and driveways('rf any);and the drainage patterns of their!ot will differ from the model home
plans,drawings or renderings they have examined. Seller reserves the right to detertnine the location and configuration of the house
upon the lot subject to subdivision requirements. In the event that the type of house desired by Buyer(s)wiil�ot fit on the lot within
subdivision requlrements,Seller shall so notity Buyer(s)and if Closing has not occurred,this Purchase and Safes Agreement shall be
terminated ar�d the deposits retumed to Buyer(s),and the partias shall be released from a!i further liability hereunder. In the event this
Is discovered after the Clasing of a Construction/Perm toan and prior to the modification of the toan,the exclusive remedy available to

adjacent to,or in the vicinity of the Home and Purchaser understands that indiv�duals,corporations,and/or utilfties or other third parties
may have specific rights granted by those easements,'rf any,including,but not limited to,access and use of the property described by
the easements,which easement rights may exist whether or not such easements are being utilized at the present time.
B. Conhol over Adjacent Propert� Selier has no control over land which ft does not own that is located adjaceM to or
in the vicinity of the Home. Seller therefore makes no representations as to what mey or may not be developed on adjacent property
or how such property may be used in the iuture.
C. Future Develooment in the Subdlvis9on. Seller reserves the right to alier, change,and/or dlscontinue its pricing
structure and building programs on any properly or lois owned or controlied by Seller including,but not limfted to,those lots in the
subdivision in which the Home is taqted(subject to any e�asdng reshictive covenants affecting such property or lots),which alterations,
changes or discontinuation include,but are not limited to,changes from residential land usage to commerciat land usage. Purchaser
acknowledges that curcent development plans for this community are subject to change without notice and Seller makes no
guarantees that any of the components or amenities presenUy proposed wiii be a part of the development, or that additional
components or amenfties will be added to the development. If some or ail of such cromponents or amenities are provlded,Seiler
makes no guarantees as to the date of avaflability for use or that current pla�s wili accurately reflect actual construction.
D. Enviranmental Risks. The Home and its occupants may now ar in the tuture be exposed to various environmental

presence of surFace and underground utility taciliNes,and the possihfiity of air,water and soil polluUon. Selier has no e�ertise
concerning such conditions or any affect they may have on the Home or its occupants. Selier makes no representations or warranties,
express or implied,about such condidons and expressly disdaims any liability for any and a0 damages which such conditions might
cause to the Home or its occupants. Purchaser agrees to hold Seller harmless in the event any of these condi8ons are discovered
and waives and releases Seller in advance from any claims against Seller in any way related to the presence of,or which may arise
out of,said conditions. These conditions may be identified with a home inspection and it is Purchaser's responsibility to obtain such

Looked at the report.
Since this is the 2nd time around, I need to be a little more cautious.
To get a 640 in 60 days,she has to be able to do the following the same week I accept her into Quick Fix:
1. Pay down Sam's Club to a baiance of$500(ThaYs a$1500 payment)
2. Pay down the Credit One Bank balance to a balance of$$200 or less(that's a$500 payment}
3. Pay off the Verizon collection of$$196.
After paying down the 2 cards—no more use of those accounts until in the new home.
Do not pay them down to-0-.
When you can get a firm commitment to do exactly those things I will put her in Quick Fix. Otherwise, I fear it will
be more of the same. This way there can me no misunderstanding. If you get a commitment today,write her today,
with the undersfanding that I will have to have proof of payments made in the exact time period,or we cancel as
quick as we wrote it.
