Complete Plans, Specifications and Fee Must Accompany Application. Commencement of work without written approval of
the Fire Department's Fire Marshal or required permits or opening up for commercial activity without an approved final
inspection shall be charged double permit fee per day of operation or a minimum of$100.00, whichever is greater. All
work shall be performed in accordance with City Codes and Ordinances.

UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES: If the owner has hired a contractor or
contractors to undertake work, they may be required to be licensed in accordance with state and local regulations. If the
contractor is not licensed as required by law, both the owner and contractor may be cited for a misdemeanor violation
under state law. If the owner or intended contractor are uncertain as to what licensing requirements may apply for the
intended work, they are advised to contact the Pasco County Building Inspection Division—Licensing Section at 727-847-
8009. Furthermore, if the owner has hired a contractor or ,contractors, he is advised to have the contractor(s) sign
portions of the "contractor Block" of this application for which;they will be responsible. If you, as the owner sign as the
contra�tor, that may be an indication that he is not properly licensed and is not entitled to permitting privileges in Pasco
County.
CONSTRUCTION LIEN LAW (Chapter 713, Florida Statutes; as amended): If valuation of work is $2,500.00 or more, I
certify that I, the applicant, have been provided with a copy of the "Florida Construction Lien Law—Homeowner's
Protection Guide" prepared by the Florida Department of Agriculture and Consumer Affairs. If the applicant is someone
other than the "owner", I certify that I have obtained a copy of the above described document and promise in good faith to
deliver it to the"owner" prior to commencement.
- CONTRACTOR'S/OWNER'S AFFIDAVIT: I certify that all the information in this application is accurate and
that all work will be done in compliance with all; applicable laws regulating construction, zoning and land
development. Application is hereby made to obtai'n a permit to do work and installation as indicated. I certify
that no work or installation has commenced prior to issuance of a permit and that all work will be performed to
meet standards of all laws regulating construction, County and City codes, zoning regulations, and land
development regulations in the jurisdiction. I also certify that I understand that the regulations of other
government agencies may apply to the intended work, and that it is my responsibility to identify what actions I
must take to be in compliance.
If I am the AGENT FOR THE OWNER, I promise in good faith'to inform the owner of the permitting conditions set forth in
this affidavit prior to commencing construction. I understand ,that a separate permit may be required for electrical work,
plumbing, signs, wells, pools, air conditioning, gas, or other installations not specifically included in the application. A
permit issued shall be construed to be a license to proceed with the work and not as authority to violate, cancel, alter, or
set aside any provisions of the technical codes, nor shall issuance of a permit prevent the Building Official from thereafter
requiring a correction of errors in plans, construction or violations of any codes. Every permit issued shall become invalid
unless the work authorized by such permit is commenced within six months of permit issuance, or if work authorized by
the permit is suspended or abandoned for a period of six (6) months after the time the work is commenced. An extension
may be requested, in writing, from the Building Official for a period not to exceed ninety (90) days and will demonstrate
justifiable cause for the extension. If work ceases for ninety(90)consecutive days, the job is considered abandoned.

CHANGES: The Subcontractor may submit Requ Ist for Change Order(s)consisting of
additions, deletions or modifications. The cost or credit and its effect on timing will be determined
by mutual consent in advance and documented by a written Change Order signed by the
Contractor. All Changes to Contract Sum or Contract Time shall be changed ONLY by written
Change Order.

PAYMENTS: The subcontractor will submit final invoice for project after contract is 100%
campleted in the amount of 10 500.00 and payment will be made NET 30 from the day Cross
Construction Company's final job inspection and approvaf of work completed, receipt of
subcontractor's invoice and the Clients' Final Paym'ent is received. The parties agree that the
Contractor's obligation to pay does not arise and is�specifically conditioned upon payment for the
services rendered being received by the Contractor. RETAINAGE:A Retainage Fee of Five
Percent(5%)will be hefd by the Contractor on the contracted amount and will be applied to all
subsequent change orders. Payment of Retainageishall be made when the work is complete,
and the contract full performed and the Clients'Final Payment is received. LIENS:
Subcontractor agrees to pay all bills when due for materials and labor used on Contractor's jobs.
Subcontractor agrees to keep such jobs free and clear of inechanics'or material men's claims or
liens. The Subcontractor agrees to provide a Release of Lien with each Draw Request.
Contractor may,at its option,make any checks payable to Subcontractor and anyone or more of
its suppliers or workers who have filed a Notice to Owner. BACK CHARGES: The Contractor
reserves the right to Back Charge the Subcontracto'r for the repair and/or completion of any
damaged or defective work caused by Subcontractor or Subcontractor's employees or
representatives during the completion of the contract. Any work or damage pe�formed by the
Subcontractor that affects the integrity of existing work performed by another Subcontractor,or
the existing structure,including common areas, elevators,or firewall penetrations made by you,

the Subcontractor,will be corrected and the costs will be Back Charged to you,the
Subcontractor.
SUBCONTRACTOR RESPONSf�ILITI�S: Subcontractor should carefully examine the site as
well as any plans, specifications and sefection sheets in order to be fuliy informed as to the
existing conditions and to�the work of others in conjunction with Subcontractor's work, including
any restrictions of working hours, use of freight elevators, delivery entrances andlor security
_ procedures. Atl work will be completed in a good, neat and professional workman-like manner in
� accordance with plans and specifications and in compliance with federal,state,or local
requirements. All materials used shall be new, unless approved in writing, and of good quality
and in the proper quan6ty required. The subcontractor must be able to provide a packing slip
- generated by their supplier to support the materials and quantities used. All labor and material
will be guaranteed against defects. Subcontractor is to fumish all necessary equipment,labor
and materials to properly complete their scope of work within the specified schedule and such
items are deemed included in the bid price.
W.4RR�.(VTY: The Subcontractor warrants to the Owner, Client, and Contractor that materials
and equipment fumished under this Subcontract will be of good quality and new unless required
or permitted by the Subcontract documents,that the work of the Subcontractor will be free from
defects not inherent in the qualiry required or permitted and that the work will conform to the
requirements of the Su6contract documents and meet or exceed building standards. Work not
conforming to these requirements,including subsfifutions not properly authorized,will be
considered defective. Subcontractor agrees to provide a one('I)year warranty from the date
Contractor receives final inspection or achieves Substantial Completion. Subcontractor agrees to
respond to a request for warranty work within twenty-four(24)hours after receivirig a request and
promptly correct the problem within five(5)working days,at Subcontractors expense. If
� Subcontractar refuses or does not so perform within said time period, Contractor may contract for
other Subcontractor services to perform and correct the probfem and bill original Subcontractar
— for said expenses including but not limited to the labor and materials of another Subcontractor.
SubconYractor shall be obfigated to pay any such biil immediately upon its receipt.
GENERAL PROilISIOPIS: The Contractor may, at�any time,provide the Subcontractor with a
written Notice to Subcontractor,indicating the nature of any problem(s}and providing the
Subcontractor with a stated response time. Failure'to respond within the stated period will be
considered a Breach of this Agreement and the Contractor may take whatever steps deemed
necessary to keep the project on schedule,including but not limited to, suspending payments,
replacing the Subcontractor and 8ack Charging all expenses including cost overruns.

SpectrAlert Advance horns,strobes,and harn strabes shall mount to a standard 4 x 4 x 1'/z-inch back box,4-inch ocEagon back box,or
double-gang back box.Two-wirs products shail also mount to a single-gang 2 x 4 x 17B-inch back box.A universal mounting plate shall
be used for mounting cEiling and wall praducts.The notification appliance circuit wiring shall terminate at the universal mounting plate.
Also,SpeetrAlert Advance products,when used with the Sync•Circuit'"Module accessory,shalE be powered from a nan-coded nofification
appiiance circuit output and shall operate on a naminal 12 or 24 volts.When used with the Sync•Circuit Module,12-volt-rated notification

visible signaling appliances,flashing at 1 Hz over the strobe's entire operating voltage range,The strobe light shall cansist of a xenan flash

Horn Strobe Combination
The horn strobe shall be a System Sensor SpectrAlert Advance Model ' listed to UL 1971 and UL 464 and shal(be approved fot
fire pratective service.Tha harn stra6e shal(be wired as a primary-signaiing notification appliance and comply with the Americans with
Disabilities Act requirements for visible signaling appliances,flashing at'1 Hz over the strobe's entire operating voltage range.The strobe light
shalf consist of a xenon flash tube and associated lens(reflector system.The horn shal!have three audibility optians and an option to switch
between a temparal three pattem and a non-temporal(cantinuous}pattem.These options are set by a mu(tipie position switch.On four-wire
products,the strobe shall be powered independently of the sounder.The horn on horn strobe models shall operate an a coded or nan-coded
power supply.
Synchronization Module '
The modufe shali be a System Sensor Sync•Circuit madei iviDL3 listed to U�464 and sha(i be appraved for fire protective service.The module
shall synchranixe SpectrAiert strobes at 1 Hz and homs at temporai three.Also,while operating the strobes,the module shall silence the homs
