' TRANSPORTATION IMPACT/UTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands
� that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings,change of
use in existing buildings, or expansion of existing buildings,�as specified in Pasco County Ordinance number 89-07 and
90-07, as amended. The undersigned also understands,that such fees,as may be due,will be identified at the time of
permitting. It is further understood that Transportation Impact Fees and Resource Recovery Fees must be paid prior to
receiving a"certificate of occupancy"or final power release� If the project does not involve a certificate of occupancy or
final power release,the fees must be paid prior to permit issuance. Furthermore, if Pasco County Water/Sewer Impact
fees are due,they must be paid prior to permit issuance in accordance with applicable Pasco County ordinances.
CONSTRUCTION LIEN LAW(Chapter 713,Florida Statutes,as amended): If valuation of work is$2,500.00 or more,I
certify that I, the applicant, have been provided with a copy of the "Florida Construction Lien Law—Homeowners
Protection Guide"prepared by the Florida Department of Agriculture and Consumer Affairs. If the applicant is someone
other than the"owner',I certify that I have obtained a copy of the above described document and promise in good faith to

will be done in compliance with all applicable laws regulating construction,zoning and land development. Application is
hereby made to obtain a permit to do work and installaUon as indicated. I certify that no work or installation has
commenced prior to issuance of a permit and that all work will be performed to meet standards of all laws regulating
construction, County and City codes, zoning regulations, and land development regulations in the jurisdiction. I also
certify that I understand that the regulations of other government agencies may apply to the intended work,and that it is
my responsibility to identify what actions I must take to be in compliance. Such agencies include but are not limited to:

- If the fill material is to be used in Flood Zonel"A° in connection with a permitted building using stem wall
construction,I certify that fill will be used only to fill the area within the stem wall.
- If fill material is to be used in any area, I certify that use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversely affect adjacent properties,the owner may be cited for violating

this affidavit prior to commencing construction. I understand that a separate permit may be required for electrical work,
plumbing, signs, wells, pools, air conditioning, gas, or other installations not specifically included in the application. A
permit issued shall be construed to be a license to proceed�with the work and not as authority to violate,cancel,alter,or
set aside any provisions of the technical codes,nor shall issuance of a permit prevent the Building Official from thereafter
requiring a correction of errors in plans,construction or violations of any codes. Every permit issued shall become invalid
unless the work authorized by such permit is commenced within six months of permit issuance, or if work authorized by
the permit is suspended or abandoned for a period of six(6)�months after the time the work is commenced. An extension
may be requested, in writing,from the Building Official for a period not to exceed ninety(90)days and will demonsVate
justifiable cause for the extension. If work ceases for ninety(90)consecutive days,the job is considered abandoned.

REMEDIATION METHUD
Underpinning
Underpinning is a technique utilized to repair structures that exhibit major distress. The process of
underpinning forces thick walled steel pipes, common(y referred to as .Pin Piles, verticalty into the
ground service using the weight of the structure as the reaction mass. The Pin Piles are hydraulically
advanced until downward force results irt reaching competent load bearing strata, bedrock or 3,000 PSI
which is what most engineering firms recommend. The Pin Piles are then pinned at a safety pressure
with a 5/8 x 6 inch steel pin that carries the weight of the structure and is moun#ed to the foundation.
Underpinning transfers the weight of the structure from unstable surface soils to stable sub-surface
materials.

PREMIER TECHNOL.C?GY SYSTEMS works in campliance with all county requirements and wifl obtain
any �elated permits from the governing agency prior ta the initiation of work. An Oriainal
Stamped/Sea1 Soil Investi�ation Repant is required for permiftina use. In the event this agreement
cannot be performed due to circumstances beyond the control of PREMIER TECHNOLOGY
SYSTEMS, THE OWNER AGREES TO reimburse PREM/ER TECHNOLOGY SYSTEMS for all
expenses and fees in arder to obtain such permits.

MONITORIPIG REQUESTED CHANGE ORDERS AND MATERIALS
Owner shall pay all valid charges for materials used by PREMIER TECHNOLOGY SYSTEMS for
proper completion of the Project. PREMIER TECHN4LOGY SYSTEMS reserves the right to request
Change Orders for any changes of quantities or additional materials required by Monitoring Engineer.
If PREM/ER TECHNOLOGY SYSTEMS1Monitoring Engineer discovers any unforeseen problems,
PREMIER TECHNOLOGY SYSTEMS will stop work until PRFMIER TECHNOLOGY SYSTEMS and
the Owner enter into a revised Agreement which will take into account the cost and expense of
correcting the unforeseen problem into consideration.
INDEMNITY
PREMIER TECHNOLOGY SYSTEMS shall indemnify and shall hold harmless the Owner, architect and
their agents against claims, damages or lawsuifs arising out oflor from work as a result of negligence on
the part of PREM/ER TECHNOLOGY SYSTEMS. Th'ie limit af PREMIER TECHNOLOGY SYSTEMS
liability shall be the contract price, not to exceed $1,OQ0,000.00.
PAYMENT SCHEDULING AND C�NDITfONS
PREMIER TECHNOLOGY SYSTEMS will require, at time of contract execution, 1/3 down of the Total
Praject Costs. The remaining Balance, based on unit price for actual quantities used, is due upon
completion of the remediation and repair work. If the owner fails to �emit the final payment, PREMIER
TECHNOLOGY SYSTEMS will be entitled to fu11 reimbursement of all costs and expenses, including
attorney fees at owners' expense. Should fhe Owner b�reach payment scheduling by exceeding Fifteen
(15) days, bafance due will accumulate an interest ch�arge of 1.5%. All payments are scheduled and

CONTRACT CONDiTIONS
For the purposes of the follow+ng contract conditions, PREMIER TECHNOLOGY SYSTEMS will hereinafter
be called "PTS", the second party to this contract; the Owner wiil hereinafter be called "Owne�' or
"Customer".
1. PREVI�US NEGOTIATIONS: All prior negotiations, proposals, correspondence and memoranda
between "Customer" and "PTS" are superseded by this proposal. This proposal, in its entirety, shall be made
an integral part and incorporated into any purchase orde�, proposal, unde�standing, or contract agreement
resufting from it. "PTS" offer is subject to possible revision in scope, price and terms if not accepted hy
Customer within thirty(30) days of the date of fhis proposal.
2. PRE-BID INFORMATION:"Customer" has furnished information used in planning the work covered in this
proposal to "PTS", and "PTS" assumes no responsibility for ifs accu�acy. If the conditions were not in
accordance with the information supplied to "P7S° by "Custome�' or others, the recommended procedures
and scope of work of this proposai may not necessarily apply, and Customer� will reimburse any additional
expenses incurred by"PTS"as a result of this difference to "PTS".
3. OWNERSHIP: "Customer"warrants and represents that no one other than "Customer" owns any interest
in the real property, which is the subject of the proposal, jn fhe event any other person owns any interest in
said real property, °Customer" hereby warrants and rep�resents that "Custome�' is the agent of all other
awner(s) and that "Custome�' is expressly authorized by such owners to enter into and execute this
agreement on their behalf.
4. HIDDEN CONDITIONS: It is understood that while periorming the proposed work, other conditions that
may have been hidden may become known. Such cansequences will be beyond "PTS'" control, and "PTS"
assumes no responsibility for such consequences, although "PTS" will use reasonable efforts and expertise
to avoid or minimize these consequences.

6. EXCL.USIONS: Any i#ems of work not specifically included in this proposal shall not be the responsibility o#
"PTS". Any altera#ion or deviation f�om the attached or referenced sp�cifications, involving extra costs will be
executed only upon written change orders, and will becom�e an extra charge over and above this proposal.
7. LIABILITY: No liability shall be accepted by"PTS"for conditi�ns that exist beyond its immediate control or
knowledge. This shall include hidden or unknown conditions, Acts of Gad, weather events or conflicts
between "PTS"worfc and the work of athers. It is understood that"ATS" must follow and adhere to the repair
recvmmendations prepared by the hired geotechnioal engineer. It is understood that no repair
recommenclation guarantees 100% success. "PTS" is licensed properly and qualified to perform all
recommendations outlined for necessary remediation. "P7S" will hold no responsibifity for damages caused
by or there of the mandated recommendations establistied by the hired geotechnical engineer. However, �
"PTS" 9s hired by fhe owner and will set forth all efforts to protect and comply with owner's right and personal
interest throughout project completion. "PTS" agrees that if these rights or persona! interest are hindered at
any time during the project, at the time "PTS"and owner witl reach mutual grounds to move forward. )f mutual
grounds are not agreed upon in writing, no obligations will�be revised from original contract agreement.
8. INCORPORATION: The terms and conditions contained herein are hereby incorporated into and made a
part of the Proposal f�om uPTS" to "Customer". (Any such Proposal and the provisions contained herein are
hereinafter referred to as the"Agreement."ar"Contract").
9. BINDING AGREEMENT: The rights and obligatians of�the pa�ties under the"Agreement"shall inure to the
beneflt of, and shall be binding upon,the successors and assigns of the parties.
10. .iURISDICTION AND VENUE: The "AgreemenY' shall be governed by the laws of the State of Florida.
Venue for any action, either by "PTS "or "Customer", arising out of or in any way related to the "Agreement",
shall lie exclusively in Brevard and Pasco Counties, Florida, to be determined by"PREMIER TECHNOLOGY
SYSTEMS'" sole discretion. "Customer' hereby consents to the personal jurisdiction of the state courts of
Brevard and Pasco Counties, Florida. The parties further agree that mailing by registered mail of any
process to the last known address of either party shaN constitute lawful and valid service of such process.
11, ATTORNEY'S FEES: In any litiga#ion involving the breach, enforcement or interpretation of this
"Agreement", the prevaiting party in such litigation shall recover reasonable attorneys' fees, costs and
expenses.
12. ENTiRE AGREEMENT: The "AgreemenY' constitutes the entire agreement between the parties with
respect to the subject matter hereof and may not be changed or amended except by a subsequent -
agreement in writing signed by"PTS°and "Custome�".
13. SEVERABILITY: If any provision of the "Agreement" is held to be unenforceable for any reason by any
court o�tribunal, that enforceability shal( not affect the remainder of the"Agreement"which shall �emain in full

16. WARRANTY: "PTS" offers a lifetime transferable limited warranty for structural damage related to the
Underpinning Process. In the event additionaf structural settlement occurs, °PTS" will evaluate the problem
free of charge. "PTS° will not be hefd responsible for movement caused by; earthquake, hurricanes, flood,
soil erosion or any other natural disaster. Your exclusi�e remedy shall be for correction of any de#ect in
workmanship. In no event shall you be entitled to consequ�ential af incidental damages, regardless of whether
the claim is based on warranty, contract, tort or otherwis�. The toal limit of"PTS"'s liability shali not exceed
the cost of the defective work in place by"PTS".

STRUCTURE CAUSED BY PLUMBING LEAKS BEFORE, DURING OR AF7ER AS A RESULT OF THE
WQRK PERFORMED. "PTS" WILL INFURM THE OWNER THAT NEARBY WELLS MAY BE AFFECTED
BY THE GROUTING PROCESS (i.e cloudy/sandy wate� may appear}. WITH THIS KNOWLEDGE, "PTS�
RECOMMENDS THA7 A HYDROSTATIC PLUMBING TEST QF ALL SUPPLY AND WASTE SYSTEMS BE
PERFORMED UPON COMPLETION OF THE PROJECT AND THE OWNER RELEASES "PTS" OF ANY

by SGT for the project site was to evaluate the foundation and determine if the residence is
stabilized and habitable at this time.To achieve this,a two-stage engineering investigation
was performed which consisted of the elements listed below

• Performing a field inspection of the residential structure, selecting field test
locations,and reviewing existing information consisting of general subsoil
conditions in the project area and any other available project area related
information.

on our site observations, and hand auger data collected by Sinkhole Geotech (SGT). In
evaluating this test boring data,SGT has used previous characteristics,which have been
observed for structures and soils similar to those encountered at this site as well as our
knowledge of the general area surrounding the project site in Zephyrhills, Pasco County,
Florida.

A previous investigation by SDII (SDII Project No. 2800031710) concluded that sinkhole
activities was occurring underneath the residence and recommended deep injection-
grouting program. Sinkhole Geotech (SGT) was requested to check this residence to
recommend the appropriate stabilization for the damage. SGT inspected the residence to
study the nature and potential causes of the damage. The damage consists of stair-step
cracks in the northeastern corner of the residence and few cracks in the west elevation and
front of the residence. Our hand auger borings indicate the presence of very loose soils
within the zone of influence of the foundation. SGT was requested by the homeowner
and/or his contractor to provide; a stabilization plan. The plan consists of exterior
underpinning points around the perimeter of the structure was provided.

'I�vo hand auger borings were performed around the perimeter of the structure to explore
the shallow subsurface soils on July 3, 2015. Tlieses borings were advanced approximately
5 feet below land surface. The house founded at depth ranged between 12 and 15 inches.
Boring locations are presented in the Attachment.

homeowner to provide underpinning piers for foundation stabilization around the

WHAT IS UNDERPINNING? (
' Underpinning is a process of augmenting the foundation underneath the building. There
are a variety of inethods to accomplish this, each with its own benefits and drawbacks.
t Generally, underpinning is a method of transferring the wall and roof loads on the
foundations to deeper soils or even to the lime'stone itself.

home, and driving those piers into deeper competent soils or to the limestone formation. If

' preventative from settling of the underpinned area.
' WHEN SHOULD UNDERPINNING BE USED? !
There is a variety of reasons when underpinning should be used. When buildings are on
unpredictable land, there is often a possibility�the foundation will become unstable. In this
' situation, underpinning may be necessary. Here are a few examples of situations that would
require underpinning:
• Structure is not even: re-leveling the structure
' ' � Cosmetic/structural cracks: closing of wide cracks caused by excessive settlement
i • Soils have changed or cavities were created: bypass weak soil layers
• Soils have not been completely idi ntified before construction began: pre-
' construction underpinning ,
• The foundation may no longer be sti ong or stable enough for support of the
' structure: foundation augmentation
• The structure's purpose may have changed: meeting the weight increase on the
foundation
' • Structure needs additional foundation capacity to footings or mats with too little
bearing capacity (building movement or ground consolidation is excessive)
• Distressed or undermined Structure needs support: a catastrophic event (e.g.
I ' erosion,landslides,burst pipes, mine subsidence, etc.)
• Adding load capacity to an existing foundation system to accommodate a seismic
retrofit or addition of new structure,building stories, etc.
' • Cost considerations: homeowner prefer�ence to secure the foundation
. .
' The plan for the Southeast Property Acquisitions LLC Properry utilized 27 underpinning
points. The underpin piers were hydraulically driven down into the underlying soils to
' refusal or a pressure that was appropriate to avoid damaging the structure based the
condition of the foundation at each location. The pier depth ranged from 7 (piers no. 11, 15,
16, & 19) feet to 28 feet (piers no. 20 &22). '
' The jacking of each pier was monitored and terminated at a pressure that was appropriate
to avoid damaging the structure based the condition of the foundation at each location. All
, piers were jacked to a pressure of up to 2,000 to 3,000 psi, which provides more than
24,000 pounds of foundation support per pier. When the piers reached the ultimate
support depth, each underpin bracket was secured to the pressure pier with a 5/8' pin. The
' jacking depth and pressures of each individual pier are presented in tabular form in the
appendix of this report. �

completed this underpinning installation in ac�cordance with applicable standard practice
' and the foundation has been stabilized and habitable at this time. Therefore, it is also our
opinion that there is no visual evidence of new or continued settlement activity at this
location at this time. This documentation is limited to the underpinning installation only
' and not intended to deal with problem behind the limits of this underpinning repair. Our
opinion is based on a reasonable professional probability from similar conditions and
similar locations. In addition, it is not intended to remediate soil conditions that may or
may not exist at the site such as buried debris, or anic matter, or im ro er foundation
