AN ORDINANCE OF THE CITY COUNCIL OF THE CITY OF
ZEPHYRHILLS, A MUNICIPAL CORPORATION, INCORPORATED
UNDER THE LAWS OF THE STATE OF FLORIDA, PROHIBITING
THE BURIAL OF A DEAD BODY OR FETUS IN A MUNICIPAL
CEMETERY, OR INSTALLATION OF A MONUMENT OF ANY TYPE,
WITHOUT FIRST SECURING A PERMIT FROM THE OFFICE OF
THE CITY CLERK OF THE CITY OF ZEPHYRHILLS, AND
PROVIDING A PENALTY FOR VIOLATION THEREOF.
Be It Ordained by the City Council of the City of Zephyr-
hills, Florida, as follows :
I.
Any person, firm or organization desiring to have buried
.a human body or fetus in a municipally owned cemetery of the
City of Zephyrhills, shall first secure a burial permit from
the City Clerk' s Office of the City of Zephyrhills, Florida,
before such burial, said application for such permit and such
permit. to be in substantially the following form :
CITY OF ZEPHYRHILLS BURIAL PERMIT
I, (Name of Applicant) , the undersigned, hereby
make application for permit to bury (Name of• deceased.)1,
who died at (Place of Death) on (Date of Death)
19 , in Grave , Row , Lot ,
Block , Cemetery, Zephyrhills,
Florida.
The undersigned further states that the record owner
of said lot is . The above information
is given according to the best of my information and
belief. My relation to decedent is

That following the issuance of such burial permit, the
City Clerk of the City of Zephyrhills, or his designated re—
presentative, shall clearly mark the location of said grave
before excavation is commenced.
III.
Any person, firm or organization desiring to install a
monument on any gravesite in amunicipally owned cemetery of
the City of Zephyrhills, shall first secure a permit authorizing
such installation from the City Clerk' s Office of the City of
Zephyrhills, the application for such permit to be in sub—
stantially the following form; !
INSTALLATION :OF MONUMENT PERMIT
I, (Name of. Applicant) , the undersigned, hereby,
make application for permit authorizing the installa-
tion of a monument on Grave , Row
Lot , Block , Cemetery,
Zephyrhills, Florida. i
The undersigned further states that the record owner
of said lot is
By the execution of this ,application, Applicant
agrees to install said monument in accordance with
instructions received from the City Clerk' s Office.
