contractor is not licensed as required by law, both the owner and contractor may be cited for a misdemeanor violation
under state law. If the owner or intended contractor are uncertain as to what licensing requirements may apply for the
intended work,they are advised to contact the Pasco County Building Inspection Division—Licensing Section at 727-847-
8009. Furthermore, if the owner has hired a contractor or contractors, he is advised to have the contractor(s) sign
portions of the"contractor Block"of this application for which they will be responsible. If you, as the owner sign as the
contractor,that may be an indication that he is not properly licensed and is not entitled to permitting privileges in Pasco

TRANSPORTATION IMPACT/UTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands
that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings,change of
use in existing buildings, or expansion of existing buildings, as specified in Pasco County Ordinance number 89-07 and
90-07, as amended. The undersigned also understands,that such fees, as may be due,will be identified at the time of
permitting. It is further understood that Transportation Impact Fees and Resource Recovery Fees must be paid prior to

CONTRACTOR'SIOWNER'S AFFIDAVIT. I certify that all the information in this appiication is accurate and that all work
will be done in compliance with all applicable laws regulating construction,zoning and land development. Application is
hereby made to obtain a permit to do work and installation as indicated. I certify that no work or installation has
commenced prior to issuance of a permit and that all work will be performed to meet standards of all laws regulating
construction, County and City codes, zoning regulations, and land development regulations in the jurisdiction. I also

may be requested, in writing,from the Building Official for a period not to exceed ninety(90)days and will demonstrate
justifiable cause for the extension. If work ceases for ninety(90)consecutive days,the job is considered abandoned.

penetration from these fasteners.Ryman Roofing and Construction,Inc.with all due caution can not be
held responsible for said damages,especially when we have not been notified of the harmful conditions.
• Ryman Roofing and Construction,Inc.can not be held responsible for broken water lines due to re-

• Any skylight or vent hold penetrations may incur loose debris falling through. _

Inc.can not be held liable for any damage(s)to such items during the duration of this project.
�. • This agreement is based upon the price(s),specification(s),and/or conditions that are fore mentioned on
this agreement.Any and all materials that are left over from this job will remain the properiy of Ryman
Roofing and Construction Inc.and will not constitute a reimbursement/credit to the owner.Any and all
additional materials will be removed from the job site upon completion of the job.Ryman Roofing and
Construction,Inc.may at their discretion leave any additional materials(if applicable)for the
owner(s)/purchaser(s)if both parties agree to such.
• Change orders that arise during the duration of this project must be signed for and agreed upon by both
parties prior to the commencing of said work.Special order items/materials must be paid for in full
before such items will be ordered and/or installed.A clerical fee of$250.00 maybe chazged for major
change orders initiated by the purchaser(s)that result in the delay of the referred work.
Buyer(s)/purchaser(s)assumes all risks,liabilities,restocking fee's,delivery fee's,etc.that maybe
� incurred during a change order plus the above listed clerical fee(if applicable).
• We take every measure to inspect the total number of layers which your roof may currently have.As the
� authorized buyer(s)Uwe understand additional layers maybe found upon tear off of the existing
system(s)and that these additional layers cannot always be detected during an initial inspection.All
proposals are based upon single layer tear-offs unless contract states otherwise.If additional layers are
found upon teaz off,the purchaser(s)authorize Ryman Roofing and Construction to remove the
additional layers at and additional cost of$ - per square,per layer which will apply to the final
amount.Ryman Roofing and Construction Inc.hereby agrees to show proof of additional layers by
pictures of the additional layers and/or by personally showing the purchaser the additional layers if
owner is available.A change order will be issued prior to commencing of said work.
• All proposals,unless otherwise noted,are based upon the assumption that the existing roof(s)does not
contain any asbestos containing or hazardous materials.Ryman Roofing and Construction Inc.is not
engaged in the identification,abatement,encapsulation or removal of asbestos containing materials and
will not be responsible for the removal of such hazardous materials. At the buyers expense a firm
specializing in the business of tesring for hazardous materials shall be hired to survey the areas of
concern and identify such materials.Prior to proceeding with any abatement process,the buyer shall
authorize in writing for the seller to proceed.Ryman Roofmg and Construction,Inc.is not responsible
for any claims,demands,or damages arising out of the removal of asbestos and/or hazardous containing
materials from the work site and the buyer,by accepting this proposal,agrees to release the seller from
any such claims,demands,or damages.
• Installation of gutter,soffit materials and fascia"metal are NOT included in a standard roof package
unless otherwise stated in your contract.These items will be charged at an addition cost unless stated in
writing in your contract
• During your project,heary machinery maybe required and we will seek to impact the properly as lightly
as possible during this renovation.The purchaser(s)hereby hold Ryman Roofing and Construction
harmless for damages that may occur to.the-property in connection with the use of such machinery.
