� Florida Geotechnical Engineering, Inc. (FGE) reviewed reports and site data generated by Andreyev
1 Engineering, Inc. (AEI), Ramos Engineering and Associates, LLC. (REA), and Applied Engineering &
Geosciences (AEG) regarding the Gamez-Guliarte residence located at 5148 Summer Hill Drive,
Zephyrhills,Florida. Based on their Contractual Geological Evaluation and the site data,AEG concluded
, sinkhole activity is present at the residence within a reasonable professional probability. FGE has
prepared this Subsurface Stabilization Plan in response to AEG's conclusion.

Authorization to proceed with the preparation of a subsurface stabilization plan was received by FGE on
IMarch 3, 2015 from the Citizens Property Insurance Corporation. As detailed herein, the site data has
resulted in the conclusion by AEG that sinkhole activity is present at the residence. Accordingly,
' subsurface stabilization should be implemented to abate the sinkhole activity, stabilize the land and
structure, and repair the foundation. Enclosed herein are recommendations and specifications to
' implement the subsurface and structural stabilization program as required by applicable Florida statutes.
FGE has processed the necessary paperwork to become the Engineer-of-Record for this project based on
� ! the recommendations presented herein. Therefore, this plan may be used by the selected stabilization
contractor to secure all necessary permits to complete the recommended stabilization measures. It is the

This Subsurface Stabihzahon Plan (SSP) as b p p y g g,
�� � (FGE) for the insured's residence located at 5148 Summer Hill Drive, Zephyrhills, Florida. Pursuant to
Florida Statute 627.707, FGE has prepared subsurface stabilization measures designed to abate the
' identified sinkhole activity below the residence and to stabilize the subsurface and structure to reduce
future settlement. The Citizens Property Insurance Corporation authorized the preparation of the
stabilization plan presented herein on March 3,2015. The site locarion is illustrated on Figure 1.

' The Gamez-Guliarte residence was the subject of a comprehensive geotechnical investigation performed
by AEI which culminated in a report dated August 2010. AEI concluded that there was no sinkhole

' Rodgers, P.G., P.A. and Biller Reinhart Structural Group, Inc. which concluded that there was sinkhole
activity related damage at the property. FGE was not provided copies of the peer reviews. Accordingly, a
Neutral Evaluation of the residence was completed by REA which culminated in a report dated October
� 2014. REA concurred with AEI that there was no sinkhole activity related damage at the property. AEG
completed a Contractual Geologic Evaluation and issued their report dated February 2015. AEG
� concluded that sinkhole activity was present at the subject property.
, Based on the result of AEG's evaluation and the available subsurface data, FGE recommends that
subsurface stabilization be accomplished through the completion of deep compaction grouting and
' shallow chemical grouting. The deep compaction grouting program is intended to stabilize the land and
building by abating the sinkhole activity via sealing the limestone surface and densifying very loose to
� loose(very soft to soft) soil above the limestone. The presence of loose shallow soil is also a contributing
cause of any settlement related damage at the residence. Accordingly, FGE recommends shallow
� chemical grouring to abate this component of settlement and repair the foundation. Based on the reported
damages, the structure construction, the subsurface conditions, and the results of the floor elevation
' survey(s),underpinning is not necessary and should not be performed.

2.0 PROPOSED REMEDIAL MEASURES
� Based on the subsurface data and the reported/observed damages, the appropriate stabilization measures
� should include compaction grouting and shallow chemical grouting. The plan presented herein
implements these methods. The compaction grouting point locations are illustrated on Figure 2, and
generally represent an 8 to 10 feet spacing around the perimeter of the structure. Based on the subsurface
' data, the 23 grout points will range in depth from 55 to 75 feet, and the estimated volume of grout is 225
to 275 cubic yards. The compaction grouting should be perfortned from the total depth of each grout
' point to 10 feet below land surface unless specified otherwise by the Engineer. The compaction grouting
specifications are provided in Attachment A. The total cost for the compaction grouting program is
� estimated to range from $65,250.00 to $73,750.00. This estimate is based on unit costs of$18/foot for
drilling and$170/cubic yard of grout.
� Due to the presence of loose sandy soils within the upper 10 feet, we also recommend a program of
� shallow chemical grouting be implemented using an estimated 1,800 pounds of expanding urethane grout
to stabilize the shallow subsurface. The shallow injection points should be spaced on approximate 3 to 5
foot centers around the perimeter of the house. The shallow grouting point locations are illustrated on
, Figure 3. The total cost for the shallow grouting program is estimated to be $25,200.00 based on unit
costs of$7,000/500 pounds of expanding urethane grout.

The cost for engineering monitoring and certification of the stabilization measures is estimated to be
, $8,500.00. It should be noted that no amount of subsurface stabilization will eliminate future cracking
due to thermal expansion and contraction and natural weathering of various building materials.There will
, always be a need for normal maintenance and upkeep. It is also recommended that management of
stormwater and roof runoff be properly managed through maintenance of a gutter system to discharge
' water away from the foundation at least five(5)feet.

, The stabilization measures presented herein are based on the exploratory borings at the reference site and
' our professional judgment. The stabilization measures described within this report are appropriate based
on the locations and depths that the soil borings were completed. Because soil types vary with location
and depth, subsurface conditions different from those reported may exist. Appropriate adjushnents to the
rproposed plan should be made at the direction of the Engineer based on actual site conditions at the time
the work is performed. This subsurface stabilization plan was completed in accordance with generally

GROUTING SPECIFICATIONS
'
� BID SOLICITATION
The purpose of the following grouting specifications is to provide basic information that should be used
' by qualified contractors to prepare a bid to implement the grouting program. The submittal of a bid will
be considered certification by the Contractor that the bidder has experience with similar grouting projects
and is licensed,insured,and qualified to perform the grouting program.
� The purpose of the grouting program is to increase the density of soil in the treatment zone. Density
increases will be realized by filling voids, sealing the surface of the limestone unit, and compressing very
' soft or loose soils above the limestone unit to minimize further karst-related settlement of the structure.
1 AVAILABLE GEOTECHNICAL INFORMATION
' A Subsidence Investigation Report has been prepared for the site.The data presented in the report may be
used to assist bid preparation and grouting implementation. It should be noted that the boring data
' presented in the report only represents subsurface conditions at the boring locations and soil intervals.
Differences in soil layers and density due to natural heterogeneity likely exist between boring locations.
The Contractor must consider and make allowances for moderate differences in soil layers due to
heterogeneity. Opinions and recommendations presented in the Subsidence Investigation Report are based
1 on geological and geotechnical interpretations of the test data.
' SCOPE OF WORK
The Contractor shall provide all necessary items to accomplish the grouting program, including all
' incidental items needed that are not specifically mentioned in this specification. The following items
must be included in the bid:
' a. A detailed grouting plan that includes a description of the method(s) that will be used to install
and remove the grout casings;
b. Any proposed injection locations that are different from the locations presented in the Subsidence
� Investigation Report;
c. A description of the method for monitoring injection pressures at the pump and grout pipe;
� d. A description of the method for monitoring ground surface and structural floor movements during
grouting operations; and

e. Site clean-up and restoration upon completing the grouting program.
, The Contractor will provide descriptions of any other work procedures, monitoring techniques, or
instrumentation that will be needed to accomplish the grouting program.
�
SUBMITTALS
� The Engineer assumes that all equipment to be used for the grouting program is appropriate for the
grouting program, and is in good and safe working order. Upon request,the Contractor will provide a list
, of major components to be used including pumps, hoses, pipe, fittings and drilling equipment, including
manufacturers' data regarding size, type, pressure rating, capacity and other critical characteristics for
each item for the Engineer's approval prior to the commencement of work. Before any site work is '
� performed, the Contractor will provide a detailed work schedule outline mobilization, drilling, grouting
and demobilization.
' MATERIALS
' The grout will consist of a combination of Portland cement, fine aggregate and water. Other additives
may be used,provided the grout mixture meets strength and slump requirements. The grout mix will have
a slump of 5 to 7 inches at the point of injection. Grout with a slump of more than 7 inches will not be
' accepted, and grout with a slump of less than 5 inches will only be accepted with prior approval from the
Engineer.
Fine aggregates will consist of hard, clean, strong, durable and uncoated particles, in accordance with
� ASTM C144-76. The fine aggregate will have a fines content of not less than 10 percent and not more
than 30 percent passing the No. 200 sieve. The gradation of the mix will be such that sand blocking is
eliminated at the grout working pressures specified.
� Shallow grouting programs shall be completed using either micro-fine cementitious grouts or chemical
grouts. `Slurried' mixtures of Portland cement will not be acceptable unless approved by the Engineer for
� void filling purposes.
Water used in the grout will be free of deleterious and organic material.
' No admixture will be used without testing and approval of the Engineer.
The Contractor is responsible to determine that an appropriate water source (purity and volume), is
' available and will be used in the course of the work.

GROUT PIPE INSTALLATION
� The Contractor will use drilling equipment to install minimum 2 to 3 inch, inside diameter, flush joint
steel casing to minimize flow restrictions and prevent plugging when injecting the low-slump grout. The
' casing will extend to the anticipated depth of bedrock as indicated in the Subsidence Investigation Report.
The intent of the grout pipe installations is to intercept the limestone/soil interface. Appropriate record
keeping of grout pipe installation will include at a minimum: the date, location, designation and depth of
the pipe. Any losses of circulation of the wash fluid and the depth at which limestone was encountered
' must also be recorded. Unless approved by the Engineer,grout pipes will not be advanced deeper than 10
feet below the top of the limestone surface.
� The steel casing will have adequate strength to maintain the bore hole and withstand the required jacking
and pumping pressures. The casing will be installed such that there is sufficient contact with the drilled
hole in order to prevent grout leakage and/or premature upward movement of the casing during high-
� pressure grout injection. Rotary wash drilling is acceptable. However, any subsidence damage caused by
rotary wash drilling shall be at the Contractor's own risk and repaired at the Contractor's expense.
� GROUT INJECTION PROCEDURES
� Grout with slumps measuring no less than 5 and no more than 7 inches will be injected into the casings.
Grout with slumps measuring more than 7 inches will not be accepted, and grout with slumps measuring
less than 5 inches will only be injected with prior approval of the Engineer.
� Grouting pressure will be continuously monitored at each gout pipe and at the pump with suitably
protected gauges.
� Grout will be injected on a continuous basis throughout the depth of the hole with the grout casing being
withdrawn in increments of 48 inches(4 feet)to 60 inches (5 feet) or less unless back pressure is pushing
grout up to the ground surface.
� Controlled grout pumping rates of 1 to 5 cubic feet per minute will be required. Grout quantities will be
monitored and recorded in increments of 48 inches (4 feet) to 60 inches (5 feet) or less. Grout quantities
� will be recorded by the pump capacity and number of strokes injected at each interval. Grout quantities
estimated by time will not be accepted.
The criteria for raising the grout pipe to the next increment will be when one of the following occur:
' 1. The grout pressure at the gauge located at the gout pipe exceeds 200 psi over the necessary
pressure to initiate grout take.
� 2 If the grout take exceeds 10 cubic yards at any one interval,the grout pipe shall be raised no more
than 24 inches (2 feet) and flushed with water, and the initial (injected) amount of grout shall be
allowed to set a minimum of 8 hours. After the 8 hour set time, injection can resume at the new
interval. If the out take at the new interval exceeds 10 cubic ards er 2-foot interval the

The Contractor will replace any grout points lost due to faulty or damaged grouting equipment at no
charge to the client upon request by the Engineer's on-site representative.

TESTING AND QUALITY CONTROL
, All daily drilling, grouting and testing reports will be submitted to the Engineer's on-site representative
within 24 hours. Drilling reports will be required and should contain at a minimum the following
� information: name of driller, type of equipment and drilling method used, date started and completed for
each grout pipe, location and identification of each injection point, and total depth drilled and total
amount of pipe installed. Additional information should include drilling conditions including zones of
� hard and soft drilling.
Grouting reports will be required and should contain at least the following mformation:name of the onsite
supervisor, constituents and proportions of grout, log of quantity injected per given interval in each
' injection point,date,and rate of pumping and grouting pressures at each point.
A level control system will be installed and operated by the Contractor for use during grouting. The
' monitoring will be carried out so as to detect any structural or ground surface movement within 50 feet of
the grouting operations whenever grouting is occurring.
� PROTECTION AND CLEANUP
During work operations, the Contractor will take such precautions as may be necessary to prevent drill
� cuttings, equipment exhaust, oil, wash water and grout from defacing and/or damaging structures or the
landscape. The Contractor will furnish such pumps as may be necessary to care for wastewater and grout
from the grouting operations and clean up all resulting wastes. The Contractor will be responsible to,
' within reason at the Engineer's discretion,restore the site to pre-grouting conditions.
� POST SUBSURFACE STABILIZATION REPAIRS
Following completion of the subsurface stabilization, it is recommended that an appropriate period elapse
� before any repairs are made to overlying structures or concrete flatwork. This period allows the
subsurface materials time to cure, and allows equilibration of the structure when force redistribution
occurs. FGE recommends no repairs be made for at least 60 days following completion of the subsurface
' stabilization protocol.
