location and design criteria and standards and hereby issues this "Notice of intent"to:

You may appeal in writing this intent to deny directly with the Department or submit a revised
permit Application addressing'the Department's comments within 30 days of receipt of this
-notice. Failure to respond within this time frame will require submittal of a new permit
• _ Application, including the applicable application fee, for a connection to the subject property.
This notice of Intent to issue a permit does not constitute Department permit issuance. The permit will be

order has beengiven to the sites served by the connection and the specific specialprovisions of the
