I understand that the following restrictions apply to the use of fill:
- Use of fill is not allawed in Flaad Zone"V" uniess expressly permitked.
- !f the frEl material is to be used in. Elood Zone "A", it is understood tttat a drainage pian �ddressing a
"campensating volume" wil! be submittsd at time of permitting which is prepared by a professic�nal engirteer "
licensed by the State of Florida.
- If the fill ma#erial is to be used in Flood Zane "A" in connection wi#h a permitted building using stem wall
construction, I certify that fill will be used only to fill the area within the stem wall.
- If fill� materiai is to be used in any area, I certify #hat use af such fill will not adversely af�ect adjacent
properties. If use of fN is found to adversely affect�atljacent properties, the owner may be citecf for vialating
the conditians af the building permit issued under the attached permit application, for lots less than one (1}
acre which are elevated by fill, an engineered drainage plan is required.
If I am the AGENT FOR THE OWNER, I promise in good faith to inform.the owner of the permitting conditior�s set farth in a
this affidavit priar ta commencing construction. I understand that a separate permit may be required for elE:ctrical work,
piumbing, signs, wells, poois, air conditioning, gas, or other installations not specificaliy included in the applicatian. A � `
permit issued shall be construed to be a license to praceed with the work and nat as authority to vialate, car�cel, aiter, or
set aside any provisions of the technica! codes, nor shall issuance of a perrnit prevent the Building Official from thereafter
requiring a correction of errars in plans, construction or violations of any�codes. Every permit issued shaU become invalid :
unless the work authorized by such permit is commenced within six months af permit issuance, or if work autharized by �
#he permi#is suspended or abandaned for a period�of six(6) months after the time the work is�commenced. f�n extension
may be requested, in.writing, from the Bui(ding Offic'sal far a periad not to exceed�ninety(90} days and will �dernonstrate
justifiable eause far the extension. If work ceases for ninety(90)consecutive days,the job is considered abar�doned.
