TRANSPORTATION IMPACTIUTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands
� that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings,change of
use in existing buildings, or expansion of existing buildings, as specified in Pasco County Ordinance number 89-07 and
90-07, as amended. The undersigned also understands,that such fees,as may be due,will be identified at the time of
permitting. It is further understood that Transportation Impact Fees and Resource Recovery Fees must be paid prior to
receiving a"certificate of occupancy"or final power release. If the project does not involve a certificate of occupancy or
final power release, the fees must be paid prior to permit issuance. Furthermore, if Pasco County Water/Sewer Impact �
fees are due,they must be paid prior to permit issuance in accordance with applicable Pasco County ordinances.
CONSTRUCTION LIEN LAW(Chapter 713,Florida Statutes,as amended): If valuation of work is$2,500.00 or more, I
certify that I, the applicant, have been provided with a copy of the "Florida Construction Lien Law—Homeowner s
Protection Guide"prepared by the Florida Department of Agriculture and Consumer Affairs. If the applicant is someone
other than the"owner", I certify that I have obtained a copy of the above described document and promise in good faith to
deliver it to the"owner"prior to commencement.
CONTRACTOR'S/OWNER'S AFFIDAVIT. I certify that all the information in this application is accurate and that all work
will be done in compliance with all applicable laws regulating construction,zoning and land development. Application is
hereby made to obtain a permit to do work and installation as indicated. I certify that no work or installation has ,
commenced prior to issuance of a permit and that all work will be performed to meet standards of all laws regulating �
construction, County and City codes, zoning regulations, and land development regulations in the jurisdiction. I also
certify that I understand that the regulations of other government agencies may apply to the intended work,and that it is
' my responsibility to identify what actions I must take to be in compliance. Such agencies include but are not limited to:
- Department of Environmental Protection-Cypress Bayheads, Wetland Areas and Environmentally Sensitive
Lands,Water/Wastewater Treatment.
- Southwest Florida Water Management District-Wells, Cypress Bayheads, Wetland Areas, Altering
Watercourses. �
- Army Corps of Engineers-Seawalls,Docks,Navigable Waterways.
- Department of Health & Rehabilitative Services/Environmental Health Unit-Wells, Wastewater Treatment,
Septic Tanks.

- Use of fill is not allowed in Flood Zone"V"unless expressly permitted.

"compensating volume"will be submitted at time of permitting which is prepared by a professional engineer
licensed by the State of Florida.
- If the fill material is to be used in Flood Zone "A" in connection with a permitted building using stem wall
, construction,I certify that fill will be used only to fill the area within the stem wall.
- If fill material is to be used in any area, I certify that use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversel affect ad'acent ro erties,the owner ma be cited for violatin

' the conditions of the building permit issued under the attached permit application,for lots less than one (1)
acre which are elevated by fill,an engineered drainage plan is required.
' If I am the AGENT FOR THE OWNER, I promise in good faith to inform the owner of the permitting conditions set forth in
this affidavit prior to commencing construction. I understand that a separate permit may be required for electrical work,
I plumbing, signs, wells, pools, air conditioning, gas, or other installations not specifically included in the application. A
permit issued shall be construed to be a license to proceed with the work and not as authority to violate, cancel,alter,or
set aside any provisions of the technical codes,nor shall issuance of a permit prevent the Building Official from thereafter
requiring a correction of errors in plans,construction or violations of any codes. Every permit issued shall become invalid
unless the work authorized by such permit is commenced within six months of permit issuance, or if work authorized by
the permit is suspended or abandoned for a period of six(6)months after the time the work is commenced. An extension
may be requested, in writing, from the Building Official for a period not to exceed ninety(90)days and will demonstrate
justifiable cause for the extension. If work ceases for ninety(90)consecutive days,the job is considered abandoned.

The following roof covers are mechanically attached or fully adhered to Approved substrates using fasteners, stress plates and
adhesives,as outlined in the Limitations/Conditions of Use herein.
➢ Mule-Hide TPO-c membranes are nominal 45-mil(1.1mm)or 60-mil(1.5 mm)thick,polyester-scrim reinforced,thermoplastic
olefin(TPO)single-ply roof inembranes.
Y Mule-Hide TPO-c EXTRA membranes are nominal 72-mil (1.Smm) or 8d-mil (2.Omm� thick, polyester-scrim reinforced,
thermoplastic olefin(TPO),single-ply roof inembranes.
Y Mule-Hide TPO-c Fleece Back membrane is a nominal 45-mil (1.1mmj or 60-mil f 1.5 mm)thicl<, polyester-scrim reinforced,
thermoplastic olefin(TPO)single-ply roof inembrane with a polyesterfleece backing.
S. LIMITATIONS:
5.1 This is a building code evaluation. Neither Trinity�ERD nor Robert Nieminen,P.E.are,in any way,the Designer of Record
for any project on which this Evaluation Report, or previous versions thereof, is/was used for permitting or design
guidance unless retained specificallyforthat purpose.
5.2 This Evaluation Report is not for use in HVHZ.
5.3 Refer to a current Roofing Materials Directory for fire ratings of this product.
5.4 For steel deck installations,foam plastic insulation shall be separated from the building interior in accordance with FBC
2603.4 unless the exceptions stated in FBC 2603.4.1 or 2603.6 apply.
5.5 The roof system evaluation herein pertains to above-decic roof components. Roof decks and structural members shall be
in accordance with FBC requirements to the satisfaction of the AH1. Load resistance of the roof deck shall be documented
through proper codified and/or FBC Approval documentation.
5.6 For recover installations,the existing roof shall be examined in accordance with FBC 1510.
5.7 For mechanically attached insulation or membrane or strip-bonded insulation, the maximum design pressure for the

2 and 3 shall employ an attachment density designed by a qualified design professional to resist the elevated pressure
criteria. Commonly used methods are RAS 117, RAS 137,FM LPD51-29 and ANSI/SPRI WD1. Assemblies marlced with an
asterisk*carry the limitations set forth in Section 2.2.1.5.1�a}of FM LPDS 1-29 for Zone 2/3 enhancements.
5.8 For assemblies with all components fully 6onded in place,the maximum design pressure for the selected assembly shall
meet or exceed critical design pressure determined in accordance with FBC Chapter 16. No rational analysis is permitted
for these systems.
5.9 For mechanically attached insulation or membrane over existing roof decks,fasteners shall be tested in the existing deck
for withdrawal resistance. A qualified design professional shall review the data for comparison to the minimum
requirements for the system. Testing and analysis shall be in accordance with TAS 105 or ANSI/SPRI FX-1.
5.10 For bonded insulation or membrane over existing substrates in a re-roof(tear offl or recover installation,the existing deck
or existing roof surface shall be examined for compatibility with the adhesive to be installed. If any surface conditions
exist that bring system performance into question, field uplift testing in accordance with ASTM E907, FM LPDS 1-52,
ANSI/SPRI IA-1 or TA5 124 shall be conducted on mocic-ups of the proposed new roof assembly.
5.11 For bonded insulation or membrane over existing substrates in a recover installation, the existing roof system shall be
capable of resisting project design pressures on its own merit to the satisfaction of the AHJ,as documented through field
uplift testing in accordance with ASTM E907,FM LPDS 1-52,ANSI/SPRI IA-1 or TAS 124.
5.12 Metal edge attachment(except gutters),shall be designed and installed for wind loads in accordance with FBC Chapter 16
and tested for resistance in accordance with AN51/SPRI ES-1 or RAS 111,except the basic wind speed shall be determined
from FBC Figure 1609.
5.13 All products in the roof assembly shall have quality assurance audit in accordance with the FBC and F.A.C.Rule 61G20-3.

6. INSTALLATION:
6.1 Mule-Hide TPO-c Single Ply Roof Systems shalf be installed in accordance with Mule-Hide Products Co,, Inc. published
installation instructions,subject to the Limitations/Conditions of Use noted herein.
6.Z System attachment requirements for wind load resistance are set forth in Appendix 1. "MDP" = Maximum Design
Pressure is the result of testing for wind load resistance based on allowable wind loads, and reflects the ultimate passing
pressure divided by 2 (the 2 to 1 margin of safety per FBC 1504.9 has already been applied). Refer to FBC 1609 for
determination of design wind pressures.
6.3 For mechanically fastened membrane systems (Type D) over profiled steel deck, membrane shall be installed running
perpendicular to steel deck flutes.
7. BUILDING PERMIT REQUIREMENTS:
As required by the Building Official or Authority Having Jurisdiction in order to properly evaluate the installation of this product.

The following notes apply to the systems outlined herein:
1. The roof system evaluation herein pertains to above-deck roof components. Roof decks and structurel members shall be in accordance with FBC requirements to the satisfaction of the AHJ. Load resistance of the
roof deck shall be documented through proper codified and/or FBC Approval documentation.

3. Unless otherwise noted,insulation may be any one layer or combination of polyisocyanurate,polystyrene,wood fiberboard,perlite,Dens Deck,Dens Deck Prime,SECUROCK Gypsum-Fiber Roof Board or SECUROCK
Glass-Mat Roof Board that meets the QA requirements of F.A.C.Rule 61G20-3 and is documented as meeting FBC 1505.1 and,for foam plastic,FBC 2603.4.1 or 2603.6,when installed with the roof cover.
4. Minimum 200 psi,minimum 2-inch thick lightweight insulating concrete may he substituted for rigid insulation board for System Type D(mechanically attached membrane),whereby the membrane fasteners are
' installed through the LWIC to engage the structural steel or concrete deck. The structural deck shall be of equal or greater configuration to the steel and concrete deck listings. The structural deck shall be of equal or
greater type,thickness and strength to the steel and concrete deck listings. Roof decks and structural members shall be in accordance with FBC requirements to the satisfaction of the AHI. Load resistance of the roof

• SECUROCK Glass-Mat Roof Board or min.3/S-inch Insulfoam R-Tech EPS or Fan-Fold may be used as a separator board,preliminarily attached prior to roof cover installation. The existing roof system shall be suitable
for a recover application.
15. For adhered membrane systems,side laps shall be minimum 3-inch wide sealed with min.1.5-inch heat weld. Membrane adhesive application retes are as follows:
➢ Mule-Hide TPO-c with Mule-Hide TPO-c Bonding Adhesive in a contact application at rate of 60 ftZ/gal.
➢ Mule-Hide TPO-c with WBBA 200p in a contact application at rate of 120 ft2/gal.
➢ Mule-Hide TPO-c Fleece Back with WBBA 2000 in a wet-lay application at a rate of 120 ft'/gal.
16. For membrane systems attached using TPO 10-inch Pressure-Sensitive RUSS,the underside of the membrane which comes in contact with the tape shall be primed with HP-250 Primer. The membrane shall be
secured by piacing the primed portion onto the tape portion of the TPO 10-inch Pressure-Sensitive RUSS and roli with a steel hand roller to ensure contact.
