Hand auger borings are performed in accordance with the ASTM D1452-80 Standard Practice for Soil
Investigation and Sampling by Auger Borings,for the purpose of obtaining information about the
shallow soil conditions and to collect soil samples for laboratory analysis.

The borings were performed by rotating an orchard bucket auger into the ground in approximately 6- �
inch increments. As each soil sample is recovered a representative sample is collected. The soil
samples are taken to the laboratory where they are visually classified using the Unified Soil
Classification System (USCS), and the color is identified using the Munsell0 Soil Color Chart.The soil
description with the appropriate color and USCS symbol for each sampled interval is presented in the
hand auger soil boring logs.
Results
Hand auger boring(s)were excavated to e�lore the shallow subsurface near the foundation.
Samples were collected at one-foot intervals in the hand auger borings performed near the location of
the distress.The boring encountered fine sands (SP)from the surface to four feet below land surface
(bls)
Standard Penetration Test(SPT)
Method '
The SPT boringsare performed in general accordancewith ASTM D-1586 PenetrationTest and Split
Barrel Sampling of Soils,for the purpose of obtaining irYformation about the deepersoil conditions and
to collect soil samples for laboratory analysis. Engineering properties such as relative density and soil
consistency of the in-situ soils can be evaluated using this method of soils e�loration.
The borings are advanced into the ground using rotary wash drilling techniques. Drilling fluid (mud) is
used to flush the cuttings out ofthe borehole to prevent the boring from collapsing.
The soil samples are collected with a split spoon sampler. The spoon is driven a distance of 18 inches
into the bottom of the hole using a 140 pound weight dropped from a height of 30 inches. The number ;
of blows required to drive the spoon each 6-inch increment is recorded on the boring log,with the last i
12 inches reported as the "N" value for that sample interval. When soils are sampled continuously, the '

spoon is driven 24 inches,and the middle two sets of blow counts are summed and reported as the
"N" value. After the split spoon sampler is removed from the borehole, it is opened and the •
representative soil sample is collected, placed in a labeled jar, and the samples are taken to the
laboratoryfor testing and classification.
Results �
One SPT boring B-1 was pertormed on the subject property. The standard penetration tests boring �
B-1 encountered three layers. The first layer consisted of inedium dense fine sands (SP)from I
ground surface to bls to 4 feet bls was excavated with hand auger. No penetration values were
obtained in layer 1.The second layer was comprised of soft clayey sand (SC)to very stiff clay(CL) �
from 4 feet bls to 23.5 feet bls with the lowest penetration value of 4 and a highest value of 19. �
The third layer consisted of hard limestone at 25 feet bls with penetration values of 100 bpf �

FOUNDATION REPAIR RECOMMENDATIONS
I
It is our opinion that the remediation of the subsurFace soils conditions at tfie residence should follow i
a limited foundation helical pinning program that includes: _ i
�
• The installation of 6 helical piers with a 10"/12"/14"triple helix lead section configuration to an
estimated average depth of(20-25)feet bls to stabilize the foundation (See Attached Figure).
The exact location of the helical piers will be determined in the field.
• The helical should extend at a maximum torque depth range of 20 feet BGS with a maximum
torque of 3943 ft-Ibs to generate an ultimate compression capacityof 26.7 kips and an
ultimate tension capacity of 23.5 kips.
• In addition we recommend the injection of 500 pounds polyurethane at 4 foot centers to depth ,
of 3 feet along 40 linear feet of portions of the structure that have e�erienced movement �
(See Attached Figure) �
Pier Design Analysis �
The maximum loadings associated with the structure are e�ected to be as follows:
Wall Load: 1%z kips/linearft
Pier Load: 10 kips
Floor Load: 80 Ibs/sq ft
The e�asting structure appears to be supported with conventional shallow foundations. The structure ,
has experienced diiferential movement. Based upon our evaluation and analyses,the soils within the ;
zone of ir�fluence may be experiencing excessive movement due to a combination of volumetric !

changes within the zone of influence. Structures which averlie loose soil t�nd to e�cperience adverse
structural distortions, such as warping of flaor slabs and cracking of masonrywall units.
In orcier to pravide proper support far the existing structure, it is recommended that the proposed
structure be supported with the use of a deep foundation system. Based upon our evaluation and
analyses of the subsurface soils encountered, efficient foundation systems are expected to consist of
helical piers. Recommendations for this foundatian system are presented for your consideration.

Our professional services have been performed, our findings obtained and our recommendations
prepared in accardance with generally accepted geotechnical engineering pr�nciples and practices.

based upon this data.
The scope of the e�qaloration was intended to evaluate soil canditions within the primary influence of
the considered structure and does not include an evaluation of patential deep soil conditions,such as
sinkholes. The analysis and recommendatians submitted in this repor�are based upon the data
obtained from the soil borings pertormed at the locatians indicated. Regardless of the thoroughness �
of a geatechnical e�larat�on,there is aiways a possibilitythat conditions between boringswill be
different from those at specific boring locatians and that conditions will not be as anticipated by the
designers or cantractors. In addition,the canstruction pracess itself may alter soil candi#ians.
�f any subsoil variatians became evider►#during the course of this praject,a re-evaluation af the
recammendations contained in this report will be necessary after we have had an opportunity to
observe the characteristics afthe canditions encauntered. The applicab9lityof this repork should alsa
be reviewed in the event that significant changes occur in the design, nature or location of the
proposed eonstruction.
The recommendatians pravided herein are based in part upon praject irrformation pravided to us and
fhey apply only to the specific praject and site discussed in this report. Once complete project '
infarmatian is available,the proposed faciliiy structure characteristics should be conveyed to us for
review. Our recommenda�ions may then be modified,if necessary. Experienced geotechnical
personne! should abserve and document#he construction procedures used and the conditions

' HELICAL PIERS
I�� A Helical Pier is comprised of a steel shaft with one or more helices attached to the steel shaft. The I I�
shafts typically range from 1'/ to 2 '/ inch rounded comer steel squares to 2 7/8 to 8 5/8 inch steel
, pipes. The helices generally range in thicknesses from 3/8 to 1/2 inches, and range from 6 to 16
inches in diameter. The helical plates are formed as a true helix with uniform pitch. Helical piers are
; designed so that the smallest diameter helix is on the leading end of the shaft, followed with larger or
I equal diameter helices. The helices are utilized to transfer load into the soil by the development of j
bearing pressure following installation. I

' Helical piers are installed using high-torque hydraulic equipment that provides rotational energy and �
I
downward pressure necessary for successful pier installation. The piers are rotated into the ground to
the specified bearing depth. Helical piers are may be designed to resist both uplift and compression
loads. The center shaft of the piers is used to transfer a�aal loads to the helix plates. The helices are I
utilized to transfer load into the soil by the development of bearing pressure following installation. �

The helical pier capacities have been determined based on the subsurface conditions encountered, �
and our extensive e�erience with similar conditions. Helical piers anchors with an 8-10-12-inch plate
configuration are e�ected to attain an allowable compressive capacity on the order of 16 kips at an
embedment depth of at least 15 feet below�the existing ground surface elevations. ;
It is anticipated that the helical piers will be embedded into the underlying SAND Based on this
information; we e�ect the total settlements will be appro�amately less than %2 inches and differential �,
settlements on the order of 50 percent of the total settlements.
, G3 Group recommends that the portion of the e�asting structure illustrated in the attached figure be
stabilized with the use of an underpin foundation system. G3 Group recommends the stabilization
program follow the guidelines included in this report.
In order to allow time for re-distribution and equilibration of stress induced bythe underpin program, it
�� is recommended that final cosmetic repairs of cracking and other distress to the house not be
performed for a period of two months following the installation of the underpins.

STRUCTURE MONITORING PROGRAM
It is recommended that the ewsting structure be closely monitored for future potentially adverse
settlement. Should future subsurface settlement result in adverse structure distortion (such as
cracking), additional structure stabilization is expected to be warranted. In this regard, it is
recommended that the walls be monitored by installing a series of crack monitors and/or vertical ! '�
survey points along the walls. Areas in which the movement of the walls is considered excessive may ; ',
be further supported with the use of a deep foundation system, as previously described. '
FLOOR SLAB ,
The floor slab is not considered to be a structural member. In this regard, repair of the floor slab is ' I
typically not considered as critical as overhead members. 'However, the displacement of the floor '
slab due to settlement may be aesthetically disruptive and create tripping hazards. Movement of floor
'� slabs within the building interiors tends to be marginal due to low moisture variations.
�
Polyurethane foam may be applied to fill voids which have developed below the e�sting structure
� floor slab and re-level the floor slab. Polyurethane consists of specialized foam having well-defined
expansion characteristics that may also be injected below the concrete slab using small-diameter '
, holes. The expansion of the polyurethane allows the unrestrained concrete to be lifted to the
preferred floor level. Following injection, the polyurethane solidifies as a rigid form and will not
l� change shape.
The floor stabilization is not considered a one-time solution to voids underlying the concrete slab, but
should be anticipated to require multiple applications. This process tends to require re-application as
the subsurface soils intrinsically settle. ,

POLYURETHANE SUBSURFACE INJECTION SPECIFICATIONS

1.0 Description
i
This work shall consist of fumishing and injecting a two component free rise, expansive,thermoset polyurethane material. The
injection shall be performed to fill potential voids and to enhance the soil mass matrix under the structure and slabs as directed by
the Engineer. i
2.0 Scope of Work i
The grouting Contractor shall provide project control, supervision, labor, materials, and equipment to accomplish the followiing
items:
i
2.1 Ensure that the Sunshine One-Call Utility Locating System has been contacted and the �,
site utilities have been identified. '
2.2 Probe all injection point areas using a pointed tip probe rod,with a maximum I�
diameter of%z"(Preferably a hand cone penetrometer to measure relative , �
densities),to determine the vertical extent of loose soils and/or voids relative �
to the site. �,
2.3 Install and if feasible,remove injection pipes,as necessary. I
2.4 Inject polyurethane material at the required depths and volume under the
supervision of the project engineer,or his representative.
2.5 Monitor for movement of the structure or ground surface during the injection
process.
2.6 Site clean-up during and after injection.
3.0 Equipment Requirements ,
3.1 A mobile pumping unit capable of injecting the high-density polyurethane
material into the soils and/or beneath the structure to the depths required shall '
be used.The pumping unit shall be capable of controlling the rate of flow of
material as required to place the polyurethane,fill voids,and to raise
foundation elements in a controlled manner.The unit shall be equipped with a
manufacturer's certified flow meter to measure the amount of high-density
polyurethane injected at each location.The certified flow meter shall have a
digital output in both pounds and gallons.
3.2 Pressure and temperature control devices capable of maintaining proper I
temperature and proportionate mixing of the polyurethane component
materials shall be used. i
3.3 Pneumatic or electric drills capable of efficiently drilling 5/8"to 3/4" ,

3.4 Laser levels or dial indicator devices capable of monitoring movement I
at the surface of the concrete shall be used.

3.5 A portable probe rod or hand cone penetrometer approved by the Engineer I
with relative density dial for on-site soils investigation to assist in location �
of weak sub-base soils and determination of the injection pattern.

3.6 All necessary equipment and materials,including but not limited to;electric
generators,compressors,heaters,hoses,containers,valves and gauges to �
efficiently conduct and control the work and minimize the impact to the '�
existing structure shall be used. I,
4.0 Injection Material Requirements
4.1 The material used for placement under the concrete or structure shall be a i
closed cell,hydroinsensitive,high-density polyurethane. �,
4.2 The material shall have a minimum free rise density of 3.O lbs./cubic ft and a
minimum compressive strength of 38.0 psi.
43 The material shall have a maximum free rise density of 5.0 lbs./cubic ft.
4.4 The material used shall be a high-density polyurethane material as approved
by the engineer.
The material shall be a polyurethane-forming mixture that reacts in both dry
and wet environments without dilution.
5.0 Injection Point Installation/Extraction
5.1 Contractor shall lay out the injection point locations for review by the
Engineer,or his site representative.
5.2 Contractor shall provide a concrete profile from laser level measurements
of each area where the concrete structures require attention. Each profile
shall be accepted by the Engineer or his representative prior to perfornung
the work at the project location.
53 Hand cone penetrometer testing should be conducted in the presence of the
Engineer or his representative prior to injection,in order to confirm existing
base soil conditions at representative locations within the treatment area.
5.4 Contractor shall install injection points through a series of 5/8"—3/4"holes
(as required for tube placement)drilled at approacimately 4-6 foot spaced
intervals through or adjacent to the concrete where indicated by the Engineer �
based on soil conditions. '

5.5 Tubing shall be used for injection of the polyurethane material into the soil.
The tubing should be pressed into place or installed with a pneumatic hammer
to ensure immediate contact with the surrounding soils to minimize material
travel along the annulus.
5.6 Tubing should be extracted via the method determined most suitable
for the site by the Engineer and Contractor,based on site conditions.
5.6.1 At uniform short intervals,progressing towards the ground
surface,or �

5.6.2 At one or more pre-determined depth intervals
5.6.3 As determined otherwise by the Engineer ,
5.7 If tubing becomes locked into the polyurethane material,it shall be cut off a �
minimum of six inches below the ground surface.
6.0 Polyurethane Injection
6.1 Void filling/Slab leveling—As necessary,polyurethane material shall first be
injected through a series of 5/8"—3/4"drilled holes until all known or
encountered voids directly under the structural element(slab,pool deck,etc.)
are filled and the element has been leveled to the extent practical. The rate and
amount of material injection shall be determined by the Contractor and the
engineer based on site conditions.

6.2 Subsurface Injection—The polyurethane material shall then be injected.
7.0 Monitoring
7.1 Continuous laser level or dial indicator micrometer readings shall be in place
and monitored by the Contractor during injection to determine sufficient
material usage and soil treatment as indicated by any recordable
movement in the ground surface or overlying structural element.
7.2 Contractor shall record the location and the uanti of material in'ected at

each grout location.
', 8.0 Basis of Payment
The accepted quantities of polyurethane material as displayed by the certified
flow meter,will be paid by the client at the contract unit price upon �
, acceptance of the quantity by the Engineer. '
