NOTICE OF DEED RESTRICTIONS: The undersigned understands that this permit may be subject to"deed"restrictions"
which may be more restrictive than County regulations. The undersigned assumes responsibility for compliance with any
applicable deed restrictions.
UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES: If the owner has hired a contractor or
contractors to undertake work,they may be required to be licensed in accordance with state and local regulations. If the
contractor is not licensed as required by law, both the owner and contractor may be cited for a misdemeanor violation
under state law If the owner or intended contractor are uncertain as to what licensing requirements may apply for the
intended work,they are advised to contact the Pasco County Building Inspection Division—Licensing Section at 727-847-
8009 Furthermore, if the owner has hired a contractor or contractors, he is advised to have the contractor(s) sign
portions of the"contractor Block"of this application for which they will be responsible. If you, as the owner sign as the
contractor, that may be an indication that he is not properly licensed and is not entitled to permitting privileges in Pasco
County.
TRANSPORTATION IMPACT/UTILITIES IMPACT AND RESOURCE RECOVERY FEES The undersigned understands
that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buiidings,change of
use i�existing buildings, or expansion of existing buildings, as specified in Pasco County Ordinance number 89-07 and
90-07, as amended. The undersigned also understands,that such fees, as may be due,will be identified at the time of
permitting. It is further understood that Transportation Impact Fees and Resource Recovery Fees must be paid prior to
receiving a"certificate of occupancy"or final power release. If the project does not involve a certificate of occupancy or
final power release, the fees must be paid prior to permit issuance. Furthermore, if Pasco County Water/Sewer Impact
fees are due,they must be paid prior to permit issuance in accordance with applicable Pasco County ordinances.
CONSTRUCTION LIEN LAW(Chapter 713,Florida Statutes,as amended). If valuation of work is$2,500.00 or more,I
certify that I, the applicant, have been provided with a copy of the "Florida Construction Lien Law—Homeowner's
Protection Guide"prepared by the Florida Department of Agriculture and Consumer Affairs. If the applicant is someone
other than the"owner",I certify that I have obtained a copy of the above described document and promise in good faith to
deliver it to the"owner"prior to commencement.
CONTRACTOR'S/OWNER'S AFFIDAVIT I certify that all the information in this application is accurate and that all work
will be done in compliance with all applicable laws regulating construction,zoning and land development. Application is
hereby made to obtain a permit to do work and installation as indicated. I certify that no work or installation has
commenced prior to issuance of a permit and that all work will be performed to meet standards of all laws regulating
construction, County and City codes, zoning regulations, and land development regulations in the jurisdiction. I also
certify that I understand that the regulations of other government agencies may apply to the intended work,and that it is
my responsibility to identify what actions I must take to be in compliance. Such agencies include but are not limited to:
- Department of Environmental Protection-Cypress Bayheads, Wetland Areas and Environmentally Sensitive
Lands,WaterlWastewater Treatment.
- Southwest Florida Water Management District-Wells, Cypress Bayheads, Wetland Areas, Altering '
Watercourses.
- Army Corps of Engineers-Seawalls,Docks,Navigable Watervvays.
- Department of Health & Rehabilitative Services/Environmental Health Unit-Wells, Wastewater Treatment,
Septic Tanks.
- US Environmental Protection Agency-Asbestos abatement.
- Federal Aviation Authority-Runways.
I understand that the following restrictions apply to the use of fill:
- Use of fill is not allowed in Flood Zone"V"unless expressly permitted.
- If the fill material is to be used in Flood Zone "A", it is understood that a drainage plan addressing a
"compensating volume"will be submitted at time of permitting which is prepared by a professional engineer
licensed by the State of Florida.
- If the fill material is to be used in Flood Zone "A" in connection with a permitted building using stem wall
construction,I certify that fill will be used only to fill the area within the stem wall.
- If fill material is to be used in any area, I certify that use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversely affect adjacent properties,the owner may be cited for violating
the conditions of the building permit issued under the attached permit application,for lots less than one(1)
acre which are elevated by fill,an engineered drainage plan is required.
If I am the AGENT FOR THE OWNER, I promise in good faith to inform the owner of the permitting conditions set forth in
this affidavit prior to commencing construction. I understand that a separate permit.may be required for electrical work,
plumbing, signs, wells, pools, air conditioning, gas, or other installations not specifically included in the application. A
permit issued shall be construed to be a license to proceed with the work and not as authority to violate,cancel,alter,or
set aside any provisions of the technical codes,nor shall issuance of a permit prevent the Building Official from thereafter
requiring a correction of errors in plans,construction or violations of any codes. Every permit issued shall become invalid
unless the work authorized by such permit is commenced within six months of permit issuance, or if work authorized by
the permit is suspended or abandoned for a period of six(6)months after the time the work is commenced. An extension
may be requested, in writing,from the Building O�cial for a period not to exceed ninety(90)days and will demonstrate
justifiable cause for the extension. If work ceases for ninety(90)consecutive days,the job is considered abandoned.

ADDENDUM C

This scope of work is to RECOVER existing roof with a 20 year.060 TPO roof system
• Obtain Permits, NOC and dispose of debris at approved EPA landfill
� Loose lay 4 ounce poly mat over field sheets and Mechanically fasten 60 mill Mule Hide TPO
roofi system
• Tear off SBS membrane from all Parapet walls, A/C curbs and penetrations
• Fully adhere new 60 mill l'PO to flash all parapets, walls, curbs and penetrations
• Tear off existing edge metals, and all wood repair of leading edge nailers are included
• Fabricate and install new TPO clad metal drip edge and weld TPO to field attachments
o Fabricate and install new 22" coping cap with cover plates from Prefinished 24 gauge steel I
color to be determined by owner. Parapet wall dimensional lumber nailers repaired at rates
(see ADDENDUM D) minus 10%.
• Fabricate and install new 24 gauge mill finished expansion Joint Cap
• Provide new MIRO pad Cable tray supports to protect new roof system from A/C Drain Lines
• Immediately after completion of roof installation new A/C drain lines and P-traps must be
installed "BY OTHERS". Lines are not included in this proposal, A state licensed HVAC or
Plumbing contractor is required, via a separate proposal
• Provide and Install Mule Hide Factory 36" wide walk pads installed around all HVAC curbs,
Mechanical curbs, and the Roof Hatch to protect roof inembrane from damage during
equipment service.
• Payment draw schedule 1/3 at material delivery ($107,752.17), 1/3 at 50 % completion

4. PRODUCT DESCRIPTION:
The following roof covers are mechanically attached or fully adhered to Approved substrates using fasteners,stress
plates and adhesives,as outlined in the Limitations/Conditions of Use herein.
4.1 Mule-Hide TPO-c membranes are nominal 45-mil (1.1mm)�or 60-mil (1.5 mm) thick, polyester-scrim
reinforced,thermoplastic olefin(TPO)single-ply roof inembranes.
4.2 Mule-Hide TPO-c EXTRA membranes are nominal 72-mil (1.8mm) or 80-mil (2.Omm) thick, polyester-scrim
reinforced,thermoplastic olefin(TPO),single-ply roof inembranes.
4.3 Mule-Hide TPO-c Fleece Back membrane is a nominal 45-mil (1.1mm) or 60-mil (1.5 mm) thick, polyester-
scrim reinforced,thermoplastic olefin(TPO)single-ply roof inembrane with a polyester fleece backing.
4.4 Mule-Hide SA-TPO membrane is a nominal 60-mil (1.5-mm)thick, polyester-scrim reinforced, thermoplastic
olefin(TPO)single-ply roof inembrane laminated to an elastomeric pressure-sensitive adhesive.
5. LIMITATIONS:
5.1 This is a building code evaluation. Neither Trinity�ERD nor Robert Nieminen,P.E.are,in any way,the Designer
of Record for any project on which this Evaluation Report, or previous versions thereof, is/was used for
permitting or design guidance unless retained specifically for that purpose.
5.2 This Evaluation Report is not for use in HVHZjurisdictions.
5.3 Refer to a current UL Roofing Materials Directory for fire ratings of this product.
5.4 For steel deck installations,foam plastic insulation shall be separated from the building interior in accordance �
with FBC 2603.4 unless the exceptions stated in FBC 2603.4.1 and 2603.6 apply. I
5.5 The roof system evaluation herein pertains to above-deck roof components. Roof decks and structural �
members shall be in accordance with FBC requirements to the satisfaction of the Authority Having �
Jurisdiction. Load resistance of the roof deck shall be documented through proper codified and/or FBC
Approval documentation.

5.7 For mechanically attached insulation or membrane or strip-bonded insulation,the maximum design pressure
for the selected assembly shall meet or exceed the Zone 1 design pressure determined in accordance with FBC
Chapter 16. Zones 2 and 3 shall employ an attachment density designed by a qualified design professional to
resist the elevated pressure criteria. Commonly used methods are ANSI/SPRI WDl,FM Loss Prevention Data
Sheet 1-29, RAS 117 and RAS 137. Assemblies marked with an asterisk* carry the limitations set forth in
Section 2.2.10.1 of FM Loss Prevention Data Sheet 1-29(January 2016)for Zone 2/3 enhancements.
5.8 For assemblies with all components fully bonded in place, the maximum design pressure for the selected
assembly shall meet or exceed critical design pressure determined in accordance with FBC Chapter 16. No
rational analysis is permitted for these systems.
5.9 For mechanically attached insulation or membrane over existing roof decks,fasteners shail be tested in the
existing deck for withdrawal resistance. A qualified design professional shall review the data for comparison
to the minimum requirements for the system. Testing and analysis shall be in accordance with ANSI%SPRI FX-
1 or TAS 105.
5.10 For bonded insulation or membrane over existing substrates in a re-roof(tear of�or recover installation,the
existing deck or existing roof surface shall be examined for compatibility with the adhesive to be installed. If
any surface conditions exist that bring system performance into question, field uplift testing in accordance
with ANSI/SPRI IA-1, ASTM E907, FM Loss Prevention Data Sheet 1-52 or TAS 124 shall be conducted on
mock-ups of the proposed new roof assembly.
5.11 For bonded insulation or membrane over existing substrates in a recover installation,the existing roof system
shali be capable of resisting project design pressures on its own merit to the satisfaction of the Authority
Having Jurisdiction, as documented through field uplift testing in accordance with ANSI/SPRI IA-1, ASTM
E907,FM Loss Prevention Data Sheet 1-52 or TAS 124
5.12 Metal edge attachment(except gutters),shall be designed and installed for wind loads in accordance with fBC
Chapter 16 and tested for resistance in accordance with ANSI/SPRI ES-1 or RAS 111, except the basic wind
speed shall be determined from FBC Figure 1609.
5.13 All products in the roof assembly shall have quality assurance audit in accordance with the FBC and F.A.C.
Rule 61G20-3.
6. I NSTALLATION:
6.1 Mule-Hide TPO-c Single Ply Roof Systems shall be installed in accordance with Mule-Hide Products Co.,Inc.
published installation instructions,subject to the Limitations/Conditions of Use noted herein.
6.2 System attachment requirements for wind load resistance are set forth in Appendix 1. "MDP" = Maximum
Design Pressure is the result of testing for wind load resistance based o.n allowable wind loads, and reflects
the ultimate passing pressure divided by 2 (the 2 to 1 margin of safety per FBC 1504.9 has already been
applied). Refer to FBC 1609 for determination of design wind pressures.
63 For mechanically fastened membrane systems(Type D)over profiled steel deck, membrane shall be installed
running perpendicular to steel deck flutes.
7. BUILDING PERMIT REQUIREMENTS:
As required by the Building Official or Authority Having Jurisdiction in order to properly evaluate the installation of
this product.
