DISCLOSURE STATEMENT FOR OWNER
CITY OF ZEPHYRHILLS BIIILDING DEPARTMENT

agree to the provisions of this i:nstrument.
The undersigned states and affirms that he or she is desirous of coastructing,
renovating, adding to or reroofiag his or her owa domicile, that he or she
actually occupies, or will occupy by said domicile, and same is not for
reat, lease or sale.' That he or she shall comply with the followiag conditions:
1. That the owner and he or she aloae shall act as the builder for all phases of
coastruction.
2. That the owner will comply with all provisions of the City of Zephyrhills
ordinances and codes pertiaent to the buildiag.
3. That in the event various phases of coastruction are subcontracted, he will
eagage only properly liceased subcontractors and will personally supervise
such work.
4. That ia the event the Building Iaspector shall require correctioas to be made,
the owner will assume full responsibility to insure they are made, and upoa
completioa will call for a reinspection before proceeding with the building.
5. , That the owner shall assume full responsibility for the constructioa and will
not expect supervision of his work from the City of Zephyrhills Suilding
Department.
6. That prior to fiaal inspectioa any additional fees, includiag reinspection
fees, must be paid in full. A written request from this office shall
coastitute an official notice to pay additional fees.
7. That the owner shall comply with all City, State aad Federal laws in regard to
social security, workman's compensation, lien laws, etc. , where applicable.
8. That the owner shall comply with all the safety codes issued by the Florida
Industrial Commissioa.
9. State law requires construction to be done by liceused coatractors. You have
applied for a permit under an exemption to that law. The exemptioa allows
you, as the owaer of your property, to act as your own contractor with certain
restrictioas even though you do aot have a licease. You must provide direct
onsite supervision of the constructioa yourself. You may build or improve a
one-family or two-family residence or a farm outbuildiag. You may also build
or improve a commercial building, provided your costs do aot exceed $75,000.
The building or residence must be for your own use or occupancy. It may not
be built or substantially improved for sale or lease. If you sell or lease a
buildiag you have built or substantially improved yourself withia 1 year after
the coustruetioa is complete, the law will presume that you built or
substantially improved if for sale or lease, whieh is a violation of this '
exemptioa. You may aot hire an ualiceased person to act as your contractor or
to supervise people working on your building. It is your responsibility to
make sure that people employed by you have liceases required by state law aad
by county or municipal licensiag ordiaances. You may not delegate the
responsibility for supervisiag work to a liceased contractor who is aot
licensed to perform the work beiag doae. Any persoa working oa your building
who is aot licensed must work under your direct supervisioa and must be
employed by you, which means that you must deduct F.I.C.A. aad withholdiag tax
and provide workers' compensation for tha � employee, all as prescribed by law.
