Attached is the itemized list of materials for the sewer meter for the bottle water company. This is the total
meter cost that Nestle would be responsible to pay. Mr. Rivera from the Nestle Company would like all the
documentation and fees that he would need to complete and pay to get this going. I am unsure of the water&&
sewer impact fee. Could you please let Mr. Rivera know that Nestle will need to contact Fathom after the
installation is complete to set up an account for billing. Please let me know if you have any questions.

At the request of the client the undersigned visited the project site on the above date and conducted a number of special
inspections and observations of the contractor's work in progress. The observations were performed in general accordance with
the requirements of Section 553.79,Florida Statutes,Permits;applications;issuance;inspections. The observations employ visual
observation techniques of the work to verify general compliance with the requirements of the project plans and specifications.
This report is a follow'up to the report dated 7,19.2018.The discrepancy noted below has been corrected.This was
observed on site visit on 10.18.2019. ,
