UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES: If the owner has hired a contractor or
contractors to undertake work,they may be required to be licensed in accordance with state and local regulations. If the
contractor is not licensed as required by law, both the owner and contractor may be cited for a misdemeanor violation
under state law. If the owner or intended contractor are uncertain as to what licensing requirements may apply for the
intended work,they are advised to contact the Pasco County Building Inspection Division—Licensing Section at 727-847-
8009. Furthermore, If the owner has hired a contractor or contractors, he is advised to have the contractors) sign
portions of the"contractor Block"of this application for which they will be responsible. If you,as the owner sign as the
contractor,that may be an Indication that he Is not properly licensed and is not entitled to permitting privileges in Pasco
County.
TRANSPORTATION IMPACTIUTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands
that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings,change of
use in existing buildings,or expansion of existing buildings,as specified in Pasco County Ordinance number 89-07 and
90-07.as amended. The undersigned also understands,that such fees,as may be due,will be identified at the time of
permitting. It is further understood that Transportation Impact Fees and Resource Recovery Fees must be paid prior to
receiving a'certificate of occupancy'or final power release. If the project does not Involve a certificate of occupancy or
final power release,the fees must be paid prior to permit Issuance. Furthermore,if Pasco County Water/Sewer Impact
fees are due,they must be paid prior to permit issuance In accordance with applicable Pasco County ordinances.
CONSTRUCTION LIEN LAW(Chapter 713,Florida Statutes,as amended): If valuation of work is$2,500.00 or more,I
certify that I, the applicant, have been provided with a copy of the "Florida Construction Lien Law—Homeowners
Protection Guide"prepared by the Florida Department of Agriculture and Consumer Affairs. If the applicant is someone
other than the'owner",I certify that I have obtained a copy of the above described document and promise in good faith to

"compensating volume°will be submitted at time of permitting which Is prepared by a professional engineer
licensed by the State of Florida.
- If the fill material is to be used In Flood Zone'A'in connection with a permitted building using stem wall
construction,I certify that fill will be used only to fill the area within the stem wall,
- If fill material is to be used in any area, I certify that use of such fill will not adversely affect adjacent
properties. If use of fill Is found to adversely affect adjacent properties,the owner may be cited for violating
the conditions of the building permit issued under the attached permit application,for lots less than one(1)
acre which are elevated by fill,an engineered drainage plan is required.
If I am the AGENT FOR THE OWNER,I promise in good faith to inform the owner of the permitting conditions set forth in
this affidavit prior to commencing construction. 1 understand that a separate permit may be required for electrical work,
plumbing,signs,wells, pools, air conditioning,gas, or other installations not specifically included in the application. A
permit Issued shall be construed to be a license to proceed with the work and not as authority to violate,cancel,after,or
set aside any provisions of the technical codes,nor shall Issuance of a permit prevent the Building Official from thereafter
requiring a correction of errors in plans,construction or violations of any codes. Every permit issued shall become Invalid
unless the work authorized by such permit Is commenced within six months of permit issuance,or if work authorized by
the permit is suspended or abandoned for a period of six(6)months after the time the work is commenced. An extension
may be requested,in writing,from the Building Official for a period not to exceed ninety(90)days and will demonstrate
justifiable cause for the extension. If work ceases for ninety(90)consecutive days,the job is considered abandoned.

GENERAL TERMS AND CONDITIONS OF EVERY CONTRACT
Definitions: In this agreement,the terms"customer", "owner", and"you"refer to the customer identified above, and the terms"Momentum Solar", "seller"
"contractor","us",and"we"refer to Pro Custom Solar LLC d/b/a Momentum Solar and its employees, agents;and subcontractors,as applicable.The term
"agreement"or"contract"refers to this agreement between you and us. "Roof Installation"means the work described below.under"Scope of Work",and any
additional roofing work we agree to undertake iri a separate writing signed by us.
Attachment A sets forth.an overhead picture of your roof. The roof replacement will be on the portion of the roof that is shaded with XXXXXs. No replacement
will be made to the unshaded sections.
We will not provide,or arrange for,architecturaVeirgineering services or structural changes to dwellings or buildings unless we expressly agree to do so in a
separate writing signed by us.We have the right to determine the method,details,and means of performing the work.We will obtain the necessary building
permits and ensure code compliance. You agree to cooperate with us to obtain these permits and meet code compliance.You are solely responsible for
obtaining, at your own cost and expense, any and all zoning variances required by any municipality or local governing body.We will not obtain a zoning
variance for you unless we expressly agree to do so in a separate writing signed by us.
Customer Obligations and Notices: You must remove all lawn ornaments and landscape accessories prior to work beginning to prevent any
accidentalfincidental damage. The Roof Installation will cause dust and minor debris in attic spaces;you are responsible for protecting your possessions in
attic areas accordingly.
Time Period:We anticipate beginning work under this agreement within fourteen(14)days after we receive all required paperwork from the applicable state
and municipality:We anticipate that we will complete the work within thirty(30)days after we begin.This is our best estimate and is subject to change according
to various factors outside of our control.We are not responsible for delays resulting from any Force Majeure Event(as defined below)or customer requested
or caused delays. You and we agree that a definite completion date is not of the essence.
Force Maieure:We will not be liable to the you for any failure.or delay in performance under this agreement caused by events beyond our control including,
Without limitation:negligence or delay by any governmental entity or utility company;your noncompliance with this agreement;your failure to fumish necessary
information or furnishing of incorrect information; legal encumbrances on your property; your property's non-conformance with building codes or zoning
requirements;your credit rating or financing arrangements;acts of God,nature,government or any third parties;war or acts of terrorism; labor strife or disputes;
shortages of labor,fuel,raw materials,or equipment;technical failures;accidents:sabotage;failure or delays in transportation or communication;failures or
substitutions of equipment; or hidden or unforeseen physical or hazardous conditions, in but not limited to, environmental hazards such as mold,
asbestos,and lead paint(each,a"Force Majeure Event").
Your Responsibilities:You agree to pay us for the Roof Installation as specified in this Agreement. If your service address is subject to any easements,
covenants,or other legal encumbrances that could affect the Roof Installation,you agree to notify us in writing,including by electronic mail or by certified mail,
Within five(5)business days of your becoming aware of the encumbrance or prior to our beginning work;whichever is earlier.You agree to ensure that work
areas are free of preexisting hazards;including,but not limited to unsafe physical conditions or environmental hazards and building/zoning code violations.
You agree to allow us access to work areas during work hours. If you have a security system,you agree to ensure that it does not interfere with installation.
You agree not to allow unattended minors at the service address while we are performing the Roof Installation.You agree to control pets and keep them away
from work areas.You agree that,if anyone under your control interferes with or delays the Roof Installation,you may be subject to transportation and/or storage
charges or other resulting charges.
Payments;All payments to Momentum Solar must be made in cash,by credit card,or by check(s)made payable to"Momentum Solar"as specified in this
agreement. Any payments you owe under your agreement with the third-party finance company must be made in accordance with the terms of your agreement
with them.
Failure to Pay:You will be in breach of this agreement if(i)we do not receive any scheduled payment you owe us on time,in full,and otherwise in accordance
with this agreement;or(ii)you wrongfully refuse to execute a completion certificate or otherwise fail to acknowledge to your third-party finance company that
we have performed the Roof Installation and this causes us not to be paid or for payment to be recovered from us. If we have not received the full amount of
any scheduled payment within ten days after the scheduled due date,you will incur a late fee equal to one percent(1%)of the unpaid amount due. If the Roof
Installation is not completed when the breach occurs,we reserve the right to suspend all work until all past-due amounts have been paid in full. Subject to
applicable law,we may also pursue amounts you owe us through legal action,which may include perfecting or enforcing a lien against your property. If we
take legal action to enforce this agreement,you agree,to the maximum extent permitted by applicable law,to pay reasonable attorney's fees and costs we

incur before and after judgment and any other costs permitted under applicable law.',If you fail to pay your third-party finance company pursuant to the terms
Of your agreement with them,you may be subject to any of the terms and remedies set forth in that agreement. Please consult that agreement to determine
.your liability and rights regarding the third-party finance company.
Mechanic's Lien Warning: Any contractor, subcontractor, or materialman who provides home improvement goods or services pursuant to your home
improvement contract and who is not paid may have a valid legal claim against your property known.as a mechanic's lien.Any mechanic's lien filed against
your property may be discharged.Payment of the agreed-upon price under the home improvement contract prior to filing of a mechanic's lien may invalidate
such lien.You may contact an attorney to determine your rights to discharge a mechanic's lien.
Inspection of Roof Installation:You agree to allow any state or local governmental authority to inspect the Roof Installation at the service address.
Cancellation by Customer: If you cancel this agreement at any time after the cancellation period described in this agreement or otherwise prevent us from
completing the Roof Installation,you agree to reimburse us in full for the cost of materials,labor,and services provided u p to the date and time of cancellation.
Termination by Momentum: Prior to beginning design and permitting work,we will perform a physical assessment of the roof condition to verify the suitability
of the site for Roof Installation. If we determine that existing site conditions will not allow for a successful implementation of the Roof Installation,we may
terminate this agreement if you and we are unable to agree to modifications needed to provide for additional work needed to m ake the site suitable for the Roof
Installation. If we do so terminate this agreement,we will refund your initial deposit in full. We may also terminate this agreement after completion of our site
assessment without further obligation if either (a) you breach this agreement, or (b) we discover or become aware of: (1) previously undisclosed legal
encumbrances or easements affecting the service address,(ii)building/zoning code violations at the service address,or(iii)hidden or unforeseen hazardous
conditions such as the presence of mold,asbestos,lead paint,or any other conditions not disclosed to us:
Manufacturer's Warranty:The manufacturer of the materials used in the Roof Installation may warrant those materials against certain defects.,We have
made available to you any such manufacturer's warranties,which govern your rights to seek a remedy from the manufacturer with respect to covered defects.
Limited Warranty: We warrant to you that we conducted the Roof Installation in accordance with (a)the written specifications of the roofing materials
manufacturers and(b)good roofing industry practices,as that term was generally understood as of the date the Roof Installation commenced. Subject to the
following terms,conditions,and limitations,we will,for five years commencing on the day the Roof Installation is completed(the"Warranty Period'),at our
expense,repair or cause to be repaired leaks in your,roof which are the result of defects in our workmanship. Upon expiration of the Warranty Period we will
have no further obligation to make repairs under this limited warranty,and you will have no further'claim against us concerning our workmanship on the Roof
Installation or any materials we used in the Roof Installation. This limited warranty does not cover any leaks in the roof caused by:the acts or omissions of
other trades or contractors;lightning,winds of peak gust speeds of 55 m.p.h.or higher measured at 10 meters above ground,hail storm,flood,earthquake,or
other unusual phenomenon of the elements; structural settlement; failure, movement, cracking or excess deflection of the roof deck; defects or failure of
materials used as a roof substrate over which the roof system is applied;faulty condition of parapet walls,copings,chimneys,skylights,vents,supports,or
other parts of the building; vapor condensation beneath the roof; penetrations for pitch boxes;erosion,cracking, and porosity of mortar and brick; dry rot;
stoppage of roof drains and gutters;penetration of the roof from beneath by rising fasteners of any type;inadequate drainage,slope or other conditions beyond
our control which cause podding or standing of water;termites or other insects;rodents or other animals;fire;or harmful chemicals,oils,acids and the like that
come in contact with the roofing system and cause a leak or otherwise damage the roof system. If the roof fails to maintain a water-tight condition because of
damage by reason of any of the foregoing,this limited warranty shall immediately become null and void. Additionally,this limited warranty shall become null
and void:(a) if you do not provide to us,during the Warranty Period,notice of a covered defect and an opportunity to inspect your roof and make any
repairs required by the terms of this limited warranty'or(b)if work(other than installation of a photovoltaic system by us or our designee)is done on your roof,
including,without limitation,work in connection with flues,vents,drains,sign braces,railings,platforms,or other equipment fastened to or set on the roof,or if
repairs or alterations are made to your roof without first notifying us in writing and giving us a reasonable opportunity to make the necessary roofing application
recommendations with respect to such repairs or alterations,or you fail to comply with any roofing application recommendation we make;or(c)if any area of
the roof is used as a promenade, walkway, or work area or is sprayed or flooded, unless such use was originally specified with a defined area and the
specification was provided in writing to us prior to our commencement of the Roof Installation. You agree to pay us for time and materials expended in making
recommendations or repairs occasioned by the work of others on your roof.
Binding Nature:This agreement shall bind and inure to the benefit of the respective heirs,personal representatives,successors,and permitted assigns of
each party.
Entirety of Contract;Modification;Waiver:This agreement constitutes the complete agreement between you and us concerning the subject matter of this
agreement and supersedes and cancels any and all prior communications,representations,and agreements between you and us,wh ether written or oral,with

respect to the subject matter of this agreement. This agreement may not be amended or modified unless in a writing and executed by you and us. The rights
and remedies of both parties under this agreement are cumulative and not alternative. No waiver of any breach or failure to enforce or insist upon performance
any of the provisions of this agreement by either party at any time will in any way affect,limit,or waive such party's right to enforce and compel strict compliance
with every provision of this agreement.
Severability:The invalidity or unenforceability of any provision of this agreement shall not affect the validity or enforceability of any other provisions of this
agreement,which shall remain in full force and effect.
Assignment:Neither party may assign,transfer or delegate any or all of its rights or obligations under this agreement,without the prior written consent of the
other party,which consent shall not be unreasonably withheld or delayed;provided,however,that(a)we may assign this agreement to any of our affiliates or
to a successor of all or substantially all of our assets through merger, reorganization, consolidation, or acquisition and (b)consent is not required for an
assignment(absolute,collateral,or otherwise),pledge of,or grant of a security interest in,a right to payment under this agreement by either party.
Dispute Resolution:In the event of a dispute,both you and we have the right to have the dispute resolved in a court of law. In this dispute resolution provision;
you and we are agreeing to limit that right.If you have any dispute with us or if we have any dispute with you,then both you and we must seek resolution of
the dispute in either arbitration or small claims.court. If your dispute cannot be resolved in small claims court for any reason,then you must seek resolution of
your dispute in arbitration. The arbitration hearing will be held before a nationally recognized provider of arbitration services at a location of your choice within
your home state. The rules of the arbitration provider will apply.This dispute resolution agreement does not alter any substantive rights that you may have
under state or federal law. By signing below,you agree to comply with this dispute resolution provision,which describes broth your and our only procedure.for
resolving any dispute. Only the arbitrator,not any federal,state,or local court or agency,has the authority to interpret this provision or determine its applicability
or enforceability.If any provision of this dispute resolution provision is found to be unenforceable for any reason, such provision will be severed and the
remainder of this dispute resolution provision will continue in full force and effect. You understand and accept that arbitration awards are subject to challenge
only in limited circumstances.
Governing Law:This agreement shall be governed by and construed in all respects in accordance with the laws of the State of Florida as they apply to a
contract entered into and performed in that state.
Taxes: Customer acknowledges and understands that it is responsible for any and all taxes that arise under this contract,including but not limited to sales
and use taxes. Customer agrees to execute any certificate or other applicable documents relating to sales or use tax exemptions to ensure that it or contractor,
as applicable,may claim exemption from paying or collecting'any sales or use tax that arises under this contract. Customer agrees to pay all applicable sales
or use taxes if such taxes are determined to be due at any time before or after the termination or expiration of this agreement.
FLORIDA HOMEOWNERS'CONSTRUCTION RECOVERY FUND. PAYMENT,UP TO A LIMITED AMOUNT,MAY BE AVAILABLE FROM THE
FLORIDA HOMEOWNERS'CONSTRUCTION RECOVERY FUND IF YOU LOSE MONEY ON A PROJECT PERFORMED UNDER.CONTRACT,WHERE
THE LOSS RESULTS FROM SPECIFIED VIOLATIONS OF FLORIDA LAW.BY A LICENSED CONTRACTOR.FOR INFORMATION ABOUT THE

Certification: I have reviewed the engineering testing reports for the racking and attachments to
be used on this project and I certify that the products are capable of supporting the code
required loads and are suitable for this installation when installed in strict compliance with the
manufacturers printed instructions.
Regarding the solar panel array installation on the above referenced project please note that
,An inspection was performed and analysis of the existing structure was conducted. There is
adequate structural capacity for the installation of the array with the following
recommendations:
1. The array will be installed on the existing roof. The roof is constructed of 2"x4"wood trusses
@ 24" O.C. spanning 142"with a top chord length of 17'4" with intermediate supports at 1/3rd
points making the effective longest span 69"with 1/2" OSB plywood sheathing. The new
array (See Site map by contractor)will add 2.63 Lb./ Sf. overall to the roof.No additional
structure support is required for the existing roof structure.
2. The system shall be secured to the roof using the "Bcofasten Solar""Rock-IT" ®attachment
system. The installation shall be in strict compliance with manufacturers printed instructions.
The attachment system shall be UL 1703 approved tested. The attachments shall be min. 5/16"
x 4" long stainless-steel lags with a min. embedment of 3.25"into the rafters. Attachments shall
be @ 48"o.c. Max. in roof wind zone 1 and @ 24"o.c. Max. in roof wind zone 2 and 3 lags to
alternate rafters. Provide 6 mil.vapor barrier between dissimilar metals. Provide water tight
gasket and sealant at all penetrations. Attachments shall follow panel rows as specified by the
system manufacturer's installation manual. The panel angle shall match the roof slope.

spacing.
• The Rock-It Array'Skirt sections are the width of a typical 60 cell

Note:The distance between the rows of mounts is calculated by the.module.dimension N-S plus 1 3/8°-
-(35mm).Lag screw should be installed as.close to center of exposed shingle as possible.The minimum

when torqued.

insert the lag bolt through the EPDM bonded washer,the Rock-it slide,the gasketed
hole in the flashing and into the rafter.
Torque-The range Is between 100-140 torquednch-pounds depending on the type
of wood and time ofyear.The visual indicator for proper torque is when the EPDM
on the underside ofthe'bonded washer begins to push out the sides as the washer
compresses.If using an Impact wrench to Install the fasteners be.careful not to over
torque the fastener.You may need to stop and use a ratchet to finish the install.

1 Locate all parts
-Locate the Rock-it Clip SS,micro-inverter/power optimizer,:and the section of the module frame In which
you will be mounting the.micro-inverter/poweroptimizer.

2 Install the Rock it Clip SS
• Slide the Rock-it Clip SS onto-the llp:of the module frame.
• Slide the micro-Inverter/power optimizer Into the opposite lip of the Rock-it Clip SS.
• Tghten the bolt to-150 In-lb minumum to damp the Rock-It Clip SS to the module frame-and the
micro-Inverter/power optimizer.to the Rock-It Clip SS.
• Ensure thatthe lip on the clip is tight against the frame and that the.micro-inverter/power optimizer flange Is
tight against the dip.flange to avoid notation during tightening.

The EcoFasten Solar Rock-it System is a rooftop PV racking system consisting of 6000 Series Aluminum and 300 Series
Stainless Steel components.The Rock-It System Includes the rack components but does not include the PV panels,
inverters or electrical components.The PV modules to be used with Rock-it shall be certified under UL 1703.The
system shall be used an steep slope roofs mounted over a Class A fire rated roofing material and attached to the roof
structure using 5/16"diameter,minimum C long 300 series Stainless Steel lag bolts with minimum thread
embedment depth of 2.1 Ar Into the roof structure.
Periodic:re4rlspection for loose.components
The system Is subject to re-Inspection as required by the PV module-manufacturer or by the Authority Having
Jurisdiction.Re-inspection,as required,should include evaluation of any loose components or loose fasteners.-All
loose components and fasteners should be secured in accordance with these Instructions.The system should also.be
evaluated for any evidence of corrosion.Any corrosion should be removed.Any affected part should be cleaned or
replaced in accordance with these instructions.
