Jackie,City of Zephyrhills.Today I received a return check from our bank indicating
funds not sufficient for payment of a one-time extension granted for inspection to be
done at the address 387219th St by#21489. I need for the company Brevard Solar to
come to the Building dept located at 5335 81h St and take care of the return check
matter or we will have to turn the matter over to our Police dept for further
investigation. I hope that it will not have to come to that.
The amount paid was for$20.00 in which we received a check#8422 on January 17,
2020 the return check charge is$50.00 total need to clear this matter will be$70.00
you may mail in cashier check if so choose to attn: building Dept-5335 81h St Zephyrhills

having problems procuring panels.
We do expect to have this job completed within the next thirty days but would rather do a
ninety-day extension just to be.safe.

NOTICE OF DEED RESTRICTIONS: The undersigned understands that this permit may be subject to "deed" restrictions"
which may be more restrictive than County regulations. The undersigned assumes responsibility for compliance with any
applicable deed restrictions.
UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES: If the owner has hired a contractor or
contractors to undertake work, they may be required to be licensed in accordance with state and local regulations. If the
contractor is not licensed as required by law, both the owner and contractor may be cited for a misdemeanor violation
under state law. If the owner or intended contractor are uncertain as to what licensing requirements may apply for the
intended work, they are advised to contact the Pasco County Building Inspection Division—Licensing Section at 727-847-
8009. Furthermore, if the owner has hired a contractor or contractors, he is advised to have the contractor(s) sign
portions of the "contractor Block" of this application for which they will be responsible. If you, as the owner sign as the
contractor, that may be an indication that he is not properly licensed and is not entitled to permitting privileges in Pasco
County.
TRANSPORTATION IMPACT/UTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands
that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings, change.of
use in existing buildings, or expansion of existing buildings, as specified in Pasco County Ordinance number 89-07 and
90-07, as amended. The undersigned also understands, that such fees, as may be due, will be identified at the time of
permitting. It is further understood that Transportation Impact Fees and Resource Recovery Fees must be paid prior to
receiving a "certificate of occupancy" or final power release. If the project does not involve a certificate of occupancy or
final power release, the fees must be paid prior to permit issuance. Furthermore, if Pasco County Water/Sewer Impact
fees are due, they must be paid prior to permit issuance in accordance with applicable Pasco County ordinances.
CONSTRUCTION LIEN LAW(Chapter 713, Florida Statutes, as amended): If valuation of work is $2,500.00 or more, I
certify that I, the applicant, have been provided with a copy of the "Florida Construction Lien Law—Homeowner's
Protection Guide" prepared by the Florida Department of Agriculture and Consumer Affairs. If the applicant is someone
other than the "owner", I certify that I have obtained a copy of the above described document and promise in good faith to
deliver it to the"owner' prior to commencement.
CONTRACTOR'S/OWNER'S AFFIDAVIT: I certify that all the information in this application is accurate and that all work
will be done in compliance with all applicable laws regulating construction, zoning and land development. Application is
hereby made to obtain a permit to do work and installation as indicated. I certify that no work or installation has
commenced prior to issuance of a permit and that all work will be performed to meet standards of all laws regulating
construction, County and City codes, zoning regulations, and land development regulations in the jurisdiction. I also
certify that I understand that the regulations of other government agencies may apply to the intended work, and that it is
my responsibility to identify what actions I must take to be in compliance. Such agencies include but are not limited to:
- Department of Environmental Protection-Cypress Bayheads, Wetland Areas and Environmentally Sensitive
Lands, Water/Wastewater Treatment.
- Southwest Florida Water Management District-Wells, Cypress Bayheads, Wetland Areas, Altering
Watercourses.
- Army Corps of Engineers-Seawalls, Docks, Navigable Waterways.
- Department of Health & Rehabilitative Services/Environmental Health Unit-Wells; Wastewater Treatment,

Thank you for the update on the companies mishaps. In order for your company to move forward in doing the work in
our jurisdiction the company must be registered to do work here in the City of ZephVrhills . Brevard Solar has only one
building permit issued at the address of3O721Qmave this permit will expire onepri| 15m this date was given based on
9O day hard ship lx. If the work will not be completed there would need to be a new set of plan given and another plan
review needed. We will need another noc because the one given is showing contractor as Brevard.The plans we have on
file are ok you would not need to submit new plans only ifthe permit expires would m/e require that.
We can do a contractor change on permit and keep the same permit#without new issuance for the cost of$30 payable
in check or cash...if check make check payable to the City ofZephyrhi||s.

Good afternoon,
I am sure by now most of you have heard that Brevard Solar went out of business and
left about 250 of their customers hanging.
I was with Brevard Solar for five years and didn't see this coming. Just plain old poor
financial management and closed office on MLK Jr. Day in January.
The electrical sub that had worked with Brevard Solar for about eight years lost a TON of
money as well as a lot of clients. He is still stepping up to the plate to "make things
right" with the customers and we will do our best to do just that. We don't want Brevard
Solar to soil the word "solar" as their are about 1200 system out there already and they
are quality work and the clients are very happy with them. The people that just have rail
on their homes now are NOT so very happy!
The electrical sub is whom I am working for now, and we are going back and trying to
get the Brevard Solar clients finished up for a reasonable price.
I have spoken to a few of the jurisdictions and they are all very understanding and are
helping us as best they can in regard to existing permits. We will be changing
contractors with those clients that wish to move forward with us and still have the same
quality work that Brevard Solar provided for eight years and over 1200 homes.
My questions are:
1. Do we just need a "change of contractor form" along with the new NOC?
2. May we use copies of the, plans, I didn't remove any original permits since they were
not my property. Any that were in Voltage Pro's names were taken out by me, only
because they belonged to Voltage Pro's. We have scanned copies of many of them, we
are hoping that the files we recovered (computer) will suffice and we can print them on
11x17 paper so the drawings can be read. Right now they are in the computer all as 8
1/2 x 1111 as that was the only scanner I had access to at Brevard Solar.
3. We will be doing these changes individually since we don't know who all is going to
move forward with us. Some have moved on with other companies and just paid the
heavier price to get it over with. We understand. It is going to be some lean times
coming up and any help from the jurisdictions to keep their residents happy helps us as
well. We are going to be working on a tight budget to get this fixed and hope to be able
to write it off as "marketing" and move on with new photovoltaic solar contracts in the
future.
I have met most of you on my visits all over the state over the years and can honestly
say everyone has been extremely helpful in the past. I hope we can continue this good
relationship now that I am on board with Voltage Pro's trying to clean up the mess that
Brevard Solar left behind.
If you can please send me exactly what will be needed for the change of contractor, I
would be most appreciative. So far it has been a change of contractor form, new NOC
and a letter from Voltage Pro's stating that Brevard Solar is gone and we will be taking
over the permit number (listing permit number and address etc.).

Of course I will have to register Voltage Pro's with each of you and I will request the
insurance certificate be forwarded to you immediately upon knowing which customers
are moving forward with you and also setting up a new online account if necessary.
There may be some that are coming up for expiration or are already expired, we really
don't know because we were not granted access to any of this information. The doors
were just closed. Many AJH's are waiving the fees and allowing us to renew permits
since the owner of Voltage Pro's was in Asia until about a week ago.
I will be watching out for all emails to update each AJH's "profile or cheat sheet" that I
have built.
Some of you may even have permits that haven't been picked up yet, we will need to

time being.
