NOTICE OF DEED RESTRICTIONS: The undersigned understands that this permit may be subject to"deed"restrictions"
which may be more restrictive than County regulations. The undersigned assumes responsibility for compliance with any
applicable deed restrictions.
UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES: If the owner has hired a contractor or
contractors to undertake work, they may be required to be licensed in accordance with state and local regulations. If the
contractor is not licensed as required by law, both the owner and contractor may be cited for a misdemeanor violation
under state law. If the owner or intended contractor are uncertain as to what licensing requirements may apply for the
intended work, they are advised to contact the Pasco County Building,Inspection Division—Licensing Section at 727-847-
8009. Furthermore, if the owner has hired a contractor or contractors, he is advised to have the contractor(s) sign
portions of the "contractor Block" of this application for which they will be responsible. If you, as the owner sign as the
contractor, that may be an indication that he is not properly licensed and is not entitled to permitting privileges in Pasco
County.
TRANSPORTATION IMPACT/UTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands
that Transportation Impact Fees and Recourse Recovery Fees.may apply to the:construction of new buildings, change of
use in existing buildings, or expansion of existing buildings, as specified in Pasco County Ordinance number 89-07 and
90-07, as amended. The undersigned also understands, that such fees, as may be due, will be identified at the time of
permitting. It is further understood that Transportation Impact Fees and Resource Recovery Fees must be paid.prior to
receiving a "certificate of occupancy" or final power release. If the project does not involve a certificate of.occupancy or
final power release; the fees must be paid prior to permit issuance. Furthermore, if Pasco County Water/Sewer Impact
fees are due, they must be paid prior to permit issuance in accordance with applicable Pasco County ordinances.
CONSTRUCTION LIEN LAW(Chapter 713, Florida Statutes, as amended): If valuation of work is $2,500.00 or more, I
certify that I, the applicant, have been provided with a copy of the "Florida,Construction Lien Law—Homeowner's
Protection Guide" prepared by the Florida Department of Agriculture and.Consumer Affairs., If the applicant is someone
other than the"owner", I certify that I have obtained a copy of the above described document and promise in good faith to
deliver it to the"owner"prior to commencement.
CONTRACTOR'S/OWNER'S AFFIDAVIT: I certify that all the information in this application is accurate and that all work
will be done in compliance with all applicable laws regulating construction, zoning and land development. Application is
hereby made to obtain a permit to do work and installation as indicated. I certify that no work or installation has
commenced prior to issuance of a permit and that all work will be performed to meet standards of all laws regulating
construction, County and City codes, zoning regulations, and land development regulations in the jurisdiction. I also

unless the work authorized by such permit is commenced within six months of permit issuance, or if work authorized by
the permit is suspended or abandoned for a period of six(6) months after the time the work is commenced. An extension
may be requested, in writing, from the Building Official for a period not to exceed ninety,(90) days and will demonstrate
justifiable cause for the extension. If work ceases for ninety(90)consecutive days, the job is considered abandoned.

We propose to furnish All Labor, Materials and Permits as needed for the New Flat Roof Area.
Work Scope for Flat Roof_ TPO
I. Permits
1. Pull and provide all Permits necessary and provide homeowner with inspection results.
II. Flat Roof TPO
1. Remove current Modified roof and underlayment layers-Currently 1 Layer of Modified.
2. Mechanically install a new ISO Insulation on the roof deck giving it a positive slope and preventing water from
ponding as per manufacture specification and detail.
3. Remove the.old vent stacks, goose neck and a-drip edge.
4. Install 2 3/8" ring shank nails around the perimeter and the decking of the roof at 6" intervals.
5. Remove and Replace any rotten plywood on the roof. Include up to 7 full sheets of plywood.
6. Replace the:rotten fascia along the home—Will replace all the rotten fascia boards found up to 30 LF
7. Install 1 layer of new Single Ply White 60mil Thermoplastic Polyolefin roof membrane with heat welded laps per
manufacture specification and detail.
8. Install New E-Drip around the entire edge of the home color as per owner selection.
9. Install New counter flashing &TPO cover tape around the perimeter and walls.
10. Mechanically fastened aluminum termination bar to secure top of base flashing &Ac units per manufacture
specification'and detail.
11. Install New pipe boots, pith pans, field wrap;TPO one-way vents•and pourable sealant pockets.
III. Insurance and Wind Mitigation
1. Provide and fill out the necessary Wind Mitigation form(s)and provide pictures for the insurance wind mitigation.
2. Provide a Roof Certification that can be provided to the Insurance Company.

IV. Dispose of Debris
1. Cleanup of job site, removal of all demolition debris as well as removal of all construction debris. Keep the area broom swept
clean.
V. Warranty
A 5-year warranty for all labor performed as part of the replacement process provided by Rainer Roofing LLC
A 20-year Manufacture Roofing Warranty.
MATERIAL SCHEDULE
Single Ply 60 Mil TPO/2 3/8" Ring Shank nails for re nailing the deck as per Florida Code. Single Ply 60 Mil TPO ISO
Insulation.
Our price for performing this work is $11,000.000 All our roofing, new construction and home additions come with a
warranty for quality and performance.

1. This warranty is in lieu of any other warranty,express or implied.Any implied warranties,including but not limited to,the implied warranty of

2. This warranty shall be null and void and Contractor shall not be liable for any damages or expenses, If Owner does not first grant Contractor

Contractor,if any,shall be deemed null and void if Customer fails to strictly adhere to the payment terms.All warranties and guarantees,if any,are
solely are non-transferable,unless otherwise agreed to by Customer and Contractor in writing.Any express warranty provided,if any,by Contractor is
the sole and exclusive-remedy for alleged construction defects,in lieu of all other remedies,implied or statutory. In no event,whether based on
contract,.warranty(express or implied),tort,federal or state statute or otherwise arising from or relating to the work and services performed under the
Agreement,shall Contractor be liable for special,consequential,or indirect damages,including loss of use,loss of profits,or actions by third parties.
5. Any claims for defects in construction,material,or workmanship are subject to the notice and cure provisions of Chapter 558,
Florida Statutes.Such claims must first be presented in writing to Contractor(and not Contractor's insurance provider)to allow opportunity
for Contractor to inspect and repair.
6, This Agreement shall be governed by the laws of the State of Florida.Venue of any dispute arising out of this Agreement shall be
Hillsborough County,Florida.Should Contractor erriploy an attorney to institute litigation or arbitration to enforce any of the provisions hereof,to
protect its interest in any matter arising out of or related to this Agreement,Contractor shall be entitled to recover from the Customer all of its attorney's
fees,costs and expenses incurred therein,including attorney's fees,costs,and expenses incurred at mediation,administrative,appellate or bankruptcy
proceedings.
7. WAIVER OF JURY TRIAL,THE PARTIES KNOWINGLY,VOLUNTARILY,IRREVOCABLY AND INTENTIONALLY WAIVE THE RIGHT TO
A TRIAL BY JURY IN RESPECT TO ANY LITIGATION ARISING OUT OF OR PERTAINING TO THE AGREEMENT,OR ANY COURSE OF
CONDUCT,COURSE OF DEALINGS,STATEMENTS(WHETHER VERBAL OR WRITTEN)OR ACTIONS OF ANY PERSON OR PARTY RELATED
TO THIS AGREEMENT;THIS IRREVOCABLE WAIVER OF THE RIGHT TO A JURY TRIAL BEING A MATERIAL INDUCEMENT FOR THE
PARTIES TO ENTER INTO THiS AGREEMENT.
8. in no event,whether based on contract,warranty(express or implied),tort,federal or state statute or otherwise arising from or relating to the
work and services performed under the Agreement,shall Contractor be liable for special,consequential,or indirect damages,including loss of use,loss
of profits,or actions by third parties.
9. Should concealed or unknown conditions in an existing structure be at variance with conditions indicated in the description of the work to be
performed from those ordinarily,encountered and generally recognized as inherent in work of the character provided for in this Agreement,the
Agreement price shall be equitably adjusted upon notice thereof from the Contractor to the Customer. Contractor will submit written documentation of
the increased charges to the Customer.SHOULD YOU HAVE ANY QUESTIONS OR COMMENTS,PLEASE CALL US,

DAMAGED OR ROTTEN 3"FASCIA WILL BE AT A RATE OF$3.95 PER LF.6"FASCIA$3.40 PER LF,(Excluding the 30 if included in the agreement.)
SUB FASCIA AND RAFTERS ARE AN ADDITIONAL$5.25 PER LF.ESTIMATED TIME OF COMPLETION WILL BE DETERMINED UPON RECEIPT
OF PERMIT,EXCLUDING SUNDAY AND WEATHER DAYS.
11. IF THERE iS MORE THAN ONE LAYER OF MODIFIED THEN A$35.00 PER SQUARE ADDITION WILL BE ADDED TO THE BALANCE.
A 3.5%CREDIT CARD PROCESSING FEE WILL BE CHARGED ON ALL CREDIT CARD TRANSACTIONS.
This agreement is subject to revision or withdrawal by RAINIER BUILDERS LLC until signed and accepted by Client and executed by an Officer of
RAINIER BUILDERS LLC.This is the complete agreement between the two parties,No prior of contemporaneous oral agreements,and no other written
agreements,except as listed above,shall be binding. The undersigned hereby accepts this Agreement and agrees to be legally bound by all the terms
and conditions set forth on the terms and conditions page.This Agreement shall be governed in accordance with the laws of the state of Florida.Any
action arising under this Agreement shall be brought in the County where RAINIER BUILDERS LLC'principle office is located. Each provision of the

as if both parties mutually drafted this Agreement.If a provision of this Agreement(or the application of it)is held by a
.ourt or arbitrator to be invalid or unenforceable,that provision will be deemed separable from the remaining of the Agreement,will be
