Skyetec Engineering Services inspected the CMU, lintels,and dowels for this structure. The
inspection failed on April 6 due to a cracked lintel in the bearing area and missing dowels. On
April 8,Skyetec again inspected the lintel for replacement and for the installation of the previously
missing dowels. The reinspection passed. However,the(previous)site construction manager
scheduled the concrete pour before the municipality's inspectors could complete their inspection.
Please accept this letter and the attached inspection reports,including photographs,as evidence
that the inspection was completed to the approved plans and specifications and has passed.
We find that all aspects conform to the approved plans.
