Missed Slab Inspection
Dear Mr. Burgess;
It is our understanding that at the project referenced above the contractor missed the required slab
inspection by City of Zephyrhills personnel. On 7/07/20 Lennar Homes requested that LogistX provide a
slab inspection verification.
Be it known,that a quality assurance inspection,a portion of which encompasses the components to be
inspected during the municipality slab inspection, was conducted by a LogistX inspector under my
supervision on 6/25/20.
Based on the results of this inspection, to the best of my belief and ability, the building components
covered by the municipality inspection were completed and installed in conformance with the approved
plans and the Florida Building Code, 6th Edition.
