Unless otherwise noted on the plans, all construction methods, workmanship, materials and testing shall conform to the "Florida Department of Transportation (FDOT)'s "Standard Specification for Road and Bridge Construction", its current supplements and standard index drawings, and any city, COUNTY, or any other agency's rules and regulations. 2. Survey layout and testing shall be by the Contractor. 3. All work within the right-of-way shall be coordinated with the CITY, County, and/or FDOT. All disturbed areas within the right-of-way shall be restored to the original condition or better in accordance with county, and/or FDOT standards. 4. Locations, elevations and dimensions of existing utilities, structures, and other features are shown according to the best information available at the time of preparation of these plans. The contractor shall verify the locations, elevations and dimensions of existing utilities, structures and other features affecting this work prior to construction. 5. The contractor shall use extreme caution in areas of buried utilities and shall provide at least 48 hours notice to the utility companies prior to construction to obtain field locations of existing underground utilities. The contractor shall protect and maintain operation of existing utilities during construction. 6. The contractor shall check plans for conflicts and discrepancies prior to construction. The contractor shall notify the engineer of any conflicts before performing any work in the affected area. 7. The Engineer shall be notified as to where existing utility services interfere with the proposed construction. 8. The Contractor is responsible for repairing any damage to existing facilities, above or below ground, that may occur as a result of the work performed by the contractor. 9. All underground utilities must be in place and tested or inspected prior to base and surface construction. 10. Utilities installation shall conform to the requirements of the applicable federal, state and local regulations including but not limited to the Florida Department of Environmental Protection (FDEP), Department of Health (DOH), the Occupational Safety and Health Administration (OSHA) Standard and the city. 11. It is the contractor's responsibility to become familiar with the permit and inspection requirements specified by the various governmental agencies and the engineer The contractor shall obtain all necessary permits prior to construction and schedule inspections according to agency instructions. All work performed shall comply with the regulations and ordinances of the various governmental agencies having jurisdiction over the work. 12. At least three (3) working days prior to construction, the contractor shall notify the engineer and appropriate agencies and supply them with the contractor's name, starting date, projected construction schedule, all required shop drawings and other information as required. Any work performed prior to proper notifications may be subject to removal and replacement at the contractor's expense. 13. The site shall be initially graded such that no offsite area will be adversely affected by storm water runoff. Runoff to the adjacent right-of-way shall be minimized. The existing drainage facilities within the right-of-way shall be protected at all times by the use of staked hay bales, temporary grading, revegetation, etc. Any disturbed area within the right-of-way shall be sodded. The contractor shall final grade and hand rake all exposed earthwork areas immediately prior to installation of sod. All areas shall be protected from erosion caused by storm water runoff by vegetation, staked hay bales, temporary grading, etc. 14. Upon completion of construction activities, the storm water management system shall be cleaned of all debris, silts, limerock, etc. 15. The contractor shall verify minimum separation between water and sanitary sewer systems (18" minimum vertical and 10' minimum horizontal separations). Upon completion of construction, the contractor shall provide written verification documenting the minimum separation as constructed. 16. Typical cover over utilities shall be 36". 17. During construction and/or maintenance of this project, all safety regulations are to be enforced. The contractor or his representative shall be responsible for the control and safety of the traveling public and the safety of his personnel. 18. The contractor shall comply with all Federal/State Occupational Safety and Health Act (OSHA) Standards and any other rules and regulations applicable to construction and/or maintenance activities in the State of Florida. The Contractor shall also comply with Chapter 442, Florida Statutes (Toxic Substances in the Workplace), and any CITY, COUNTY, or any other agency's rules and regulations regarding safety. 19. All trenching and excavation operations MUST conform to the requirements stated in the new "Trench Safety Act" incorporated into OSHA standards 29 CFR 1926 Subpart P final rule and the State of Florida Trench Safety Act. Further Contractor operations shall comply with 29CFR 1910.252 and NFPA 51 B for cutting and welding procedures. 21. All persons on (CITY, COUNTY OR STATE) property and in area where the noise level exceeds 85db, must wear hearing protection that complies with ANSI S3.19-74 (ear muffs and/or approved ear plugs). This includes areas where noisy equipment is in use (i.e. jack hammers, electric or air drills, heavy equipment with open cabs, pipe cutting saws, etc.) and in the plant where posted. 22. All work conducted in an elevated position shall comply with 29CFR 1910.269. 23. All design application, installation, maintenance, removal of all traffic control devices, warning devices and barriers necessary to protect the public and workmen from hazards within the projects shall be in accordance with the minimum standards as set forth in the current edition of the State of Florida and Traffic Design Standards. 24. All traffic control markings and devices shall conform to the provision set forth in the "Manual on Uniform Traffic Control Devices" prepared by the U.S. Department of Transportation, Federal Highway Administration. 25. All Contractor owned or controlled vehicles and/or equipment which will be operated on or within ten (10) feet of the roadway shall be equipped with a minimum of one amber 360 degree Class I warning device. This device must meet minimum standards for utility construction purposes, such as a minimum of 500,000 candlepower and visible from 360 degrees of mounting. The warning device(s) must be in operation at all times that a vehicle/equipment if on the roadway or within the ten (10) feet of runoff area and not in a "normal" travel status. 26. Area designated by the Owner shall be utilized for equipment storage and cover material staging. Materials and supplies shall be stored only in those areas approved by the owner. 27. In regard to the inspection of the project, Southwest Florida Water Management District (SWFWMD) and the city inspectors shall have the same right of review and inspection as the Engineer of Record. Although it is appropriate for the Engineer of Record to resolve questions, diffic�ities, and disputes relative to interpretation of contract documents or construction; SWFWMD and the city reserve the right of final concurrence on such matters. 28. The city's Engineering inspector shall be notified, and the Engineer's approval shall be obtained, on all proposed field changes, alterations or additions to the approved plans by the Contractor or the Engineer of Record prior to the implementation of such changes, alterations or additions. 29. The contractor shall obtain a separate permit for any on -site burning. 30. If during construction activities any evidence of historic resources including but not limited to aboriginal or historic pottery, prehistoric stone tools, bone or shell tools, historic trash pits, or historic building foundation, are discovered, work shall come to an immediate stop and the Florida Department of Historic Resources (State Historic Preservation Officer) and Pasco County shall be notified within two working days of the resources found on the site. 31. All proposed signs must be applied for, approved and permitted on an individual basis apart from ultimately approved site plan. Approval of this site plan does not constitute approval of any signage. 32. Handicap parking spaces will be Properly signed and and striped in accordance with Florida Statute 316, the Manual on Uniform Traffic Control Devices, or other applicable standards. 33. The Engineer certifies that the site has been designed in accordance with the American Disability Act. 34. All on -site parking spaces will be striped and signed in accordance with the Manual on Uniform Traffic Control Devices Latest Edition. Parking spaces, directional arrows, and stop bars shall be striped in white. It shall be the Owner/Developers responsibility to properly sign and stripe the site in accordance with applicable standards. 35. All clear -site areas shall be kept free of any signage plantings, trees, etc. In excess of three and one-half feet in height. 36. All structures including buffer walls, retaining walls, fencing, signage etc. require separate building permits. 37. No irrigation system or landscaping shall be installed in any city right of way without the appropriate right of way permit. 38. The contractor shall submit shop drawings on all precast and manufactured items to the engineer for approval. During construction and/or maintenance of this project, all safety regulations are to be enforced. The contractor or his representative shall be responsible for the control and safety of the traveling public and the safety of his personnel. 11. The contractor shall comply with all Federal/State Occupational Safety and Health Act (OSHA) Standards and any other rules and regulations applicable to construction and/or maintenance activities in the State of Florida. The Contractor shall also comply with Chapter 442, Florida Statutes (Toxic Substances in the Workplace), and any County, City, or any other applicable agency's rules and regulations regarding safety. 12. Contractor operations shall comply with 29CFR 1910.252 and NFPA 51 B for cutting and welding procedures. 13. All persons on public property and in area where the noise level exceeds 85db, must wear hearing protection that complies with ANSI S3.19-74 (ear muffs and/or approved ear plugs). This includes areas where noisy equipment is in use (i.e. jack hammers, electric or air drills, heavy equipment with open cabs, pipe cutting saws, etc.) and in the building where posted. 14. All work conducted in an elevated position shall comply with 29CFR 1910.269. 15. All design application, installation, maintenance, removal of all traffic control devices, warning devices and barriers necessary to protect the public and workmen from hazards within the project shall be in accordance with the minimum standards as set forth in the current edition of the State of Florida and Traffic Design Standards. 16. Area designated by the Owner shall be utilized for equipment storage and cover material staging. Materials and supplies shall be stored only in those areas approved by the Owner. 17. The County Engineering inspector shall be notified, and the Engineer's approval shall be obtained, on all proposed field changes, alterations or additions to the approved plans by the Contractor or the Engineer of Record prior to the implementation of such changes, alterations or additions. 18.
