NOTICE OF DEED RESTRICTIONS: The undersigned understands that this permit may be subject to "deed" restrictions"
which may be more restrictive than County regulations. The undersigned assumes responsibility for compliance with any
applicable deed restrictions.
UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES: If the owner has hired a contractor or
contractors to undertake work, they may be required to be licensed in accordance with state and local regulations. If the
contractor is not licensed as required by law, both the owner and contractor may be cited for a misdemeanor violation
under state law. If the owner or intended contractor are uncertain as to what licensing requirements may apply for the
intended work, they are advised to contact the Pasco County Building Inspection Division —Licensing Section at 727-847-
8009. Furthermore, if the owner has hired a contractor or contractors, he is advised to have the contractor(s) sign
portions of the "contractor Block" of this application for which they will be responsible. If you, as the owner sign as the
contractor, that may be an indication that he is not properly licensed and is not entitled to permitting privileges in Pasco
County.
TRANSPORTATION IMPACT/UTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands
that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings, change of
use in existing buildings, or expansion of existing buildings, as specified in Pasco County Ordinance number 89-07 and
90-07, as amended. The undersigned also understands, that such fees, as may be due, will be identified at the time of
permitting. It is further understood that Transportation Impact Fees and Resource Recovery Fees must be paid prior to
receiving a "certificate of occupancy" or final power release. If the project does not involve a certificate of occupancy or
final power release, the fees must be paid prior to permit issuance. Furthermore, if Pasco County Water/Sewer Impact
fees are due, they must be paid prior to permit issuance in accordance with applicable Pasco County ordinances,
CONSTRUCTION LIEN LAW (Chapter 713, Florida Statutes, as amended): If valuation of work is $2,500.00 or more, I
certify that 1, the applicant, have been provided with a copy of the "Florida Construction Lien Law —Homeowner's
Protection Guide" prepared by the Florida Department of Agriculture and Consumer Affairs. If the applicant is someone
other than the "owner", I certify that I have obtained a copy of the above described document and promise in good faith to
deliver it to the "owner" prior to commencement.
CONTRACTOR'S/OWNER'S AFFIDAVIT: I certify that all the information in this application is accurate and that all work
will be done in compliance with all applicable laws regulating construction, zoning and land development. Application is
hereby made to obtain a permit to do work and installation as indicated. I certify that no work or installation has
commenced prior to issuance of a permit and that all work will be performed to meet standards of all laws regulating
construction, County and City codes, zoning regulations, and land development regulations in the jurisdiction. I also
certify that I understand that the regulations of other government agencies may apply to the intended work, and that it is
my responsibility to identify what actions I must take to be in compliance. Such agencies include but are not limited to:
Department of Environmental Protection -Cypress Bayheads, Welland Areas and Environmentally Sensitive
Lands, Water/Wastewater Treatment.
Southwest Florida Water Management District -Wells, Cypress Bayheads, Wetland Areas, Altering
Watercourses,
Army Corps of Engineers -Seawalls, Docks, Navigable Water -ways,
Department of Health & Rehabilitative Services/Environmental Health Unit -Wells, Wastewater Treatment,
Septic Tanks.
US Environmental Protection Agency -Asbestos abatement.

I understand that the following restrictions apply to the use of fill:
Use of fill is not allowed in Flood Zone "W unless expressly permitted.
If the fill material is to be used in Flood Zone "A", it is understood that a drainage plan addressing a
"compensating volume" will be submitted at time of permitting which is prepared by a professional engineer
licensed by the State of Florida.
If the fill material is to be used in Flood Zone "A" in connection with a permitted building using stem wall
construction, I certify that fill will be used only to fill the area within the stem wall.
If fill material is to be used in any area, I certify that use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversely affect adjacent properties, the owner may be cited for violating
the conditions of the building permit issued under the attached permit application, for lots less than one (1)
acre which are elevated by fill, an engineered drainage plan is required.
If I am the AGENT FOR THE OWNER, I promise in good faith to inform the owner of the permitting conditions set forth in
this affidavit prior to commencing construction. I understand that a separate permit may be required for electrical work,
plumbing, signs, wells, pools, air conditioning, gas, or other installations not specifically included in the application. A
permit issued shall be construed to be a license to proceed with the work and not as authority to violate, cancel, alter, or
set aside any provisions of the technical codes, nor shall issuance of a permit prevent the Building Official from thereafter
requiring a correction of errors in plans, construction or violations of any codes. Every permit issued shall become invalid
unless the work authorized by such permit is commenced within six months of permit issuance, or if work authorized by
the permit is suspended or abandoned for a period of six (6) months after the time the work is commenced. An extension
may be requested, in writing, from the Building Official for a period not to exceed ninety (90) days and will demonstrate
justifiable cause for the extension. If work ceases for ninety (90) consecutive days, the job is considered abandoned.

Using 6 Anchor Assemblies
Lock Down Anchor Assembly Installation Instructions
The Ground Anchor with Strut Attachment Cap will take the place of one perimeter anchor
and align with a sidewall vertical strap. Install this anchor at a slight angle, not to exceed
15 degrees. This assembly includes a stabilizer plate that is installed with the anchor and
attached with a U-bolt and two nuts. The stabilizer plate should be driven into the ground
when the anchor is about 2/3 installed, Attach the stabilizer to the anchor shaft using the
U-bolt and nuts then complete the install of the anchor until the strut attachment cap is
flush to the ground or slightly recessed into the ground, no more than W'. The Strut

Attachment Cap should be installed within Y," or flush to the top of the stabilizer plate,

NOTE: State of Florida allows a 2' pilot holes to be drilled to assist anchor installation in extremely hard soil conditions, if this process is used during installation
of the Ground Anchor with Strut Attachment Cap the soil must be placed back in the pilot hole and compacted at 6" intervals, This process should take place
when the anchor is 8"-10" from fully installed - allowing room for the soil to be placed back in the pilot hole and properly packed.

Once the ground anchor assembly is fully installed, attach longitudinal and lateral struts according to the diagram under anchor assembly placement (page 1j. The
lateral arm fasteners are to be installed tightly (nuts and bolts). The Beam Clamp assembly has two holes for the bolt and nut placement. Depending an the width of
the beam, the corresponding hole should be used that pushes the front of the metal plate as close as possible to the upright of the beam. See the illustration on page
2, #5,
Lateral struts should be installed in line with the center anchor head and attached at the top of the I-beam,
Lateral strut angles must fall within 10 degree minimum - 60 degree maximum, Longitudinal strut angles must fall within 10 degree minimum - 50 degree maximum.
After the Lock Down Anchor Assemblies have been fully installed and all permiter anchors and straps fully installed and tightened — each strut has 4 self tapping
screws that must be installed as the final step. Predrilled holes are provided on the outer strut indicating placement. If the predrilled holes can not be accessed,
install the screws as close to the predrilled holes as possible - making sure they are tapping into the inner strut.
The Lock Down Anchoring System should only be used for homes in the state of Florida, This anchoring system is not designed, tested or approved for use in any
other state,

Home Pride warrants its Lock Down Anchor System (HP LDS) against defects in workmanship and material at the site of its initial installation, The HP LDS is specifically
designed and approved for use in the state of Florida, This product is not approved for use and should not be used outside the State of Florida, This product must
not be reused or reinstalled at any other location. if used in violation of this Warranty or not installed in accordance with installation instructions, all written and
implied warranties arevoid and disclaimed. HP LDS products which are found to have defects will be replaced or repaired at Home Pride's option. This Express Warranty
is limited to replacement of product only and does not cover any labor or installations costs. When the product is replaced, all Warranty rights are extinguished, This
Warranty is only to the benefit of the original purchaser and is not transferable.
There are no other warranties (express or implied) whatsoever which apply to the HP LDS product or to items that are functionally part of the HP LDS product. Home
Pride disclaims any and all other implied warranties, including (but not limited to) warranties of habitability, workmanship, materials and fitness for a particular purpose
to the extent allowed by law and any implied warranty that exists despite this disclaimer is limited to a period of one (1) year from the effective date of this Warranty.
These limitations shall be enforceable to the extent permitted by law,
HP LDS products which are examined and are found not to be defective will be returned to user and all costs associated with examination of the anchor product will be
incurred by the user. The user is responsible for all maintenance of the HP LDS product including regular monitoring of stability and integrity of HP LDS products,

Step 1...
Laying the Bottom Channel Components
A level or plumb line should be used to establish the location of the Bottom
Channel, The taller backside of the Bottom Channel should be located directly
beneath the outside bottom edge of ,,V — r-1, � '-j ,
skirting and provide a non-shiffing base for the ground spikes, 9" asphalt roof starter
should be installed around the home. The roof starter also reduces the possibility of the

holes (seefigure 2). Spikes are required every 24 inches. ..extra holes are provided
in the Bottom Channel component for convenience. Another spike or a drift punch
may be used to drive spikes in completely. To allow for expansion, leave a 1/2" gap
between each section of the Bottom Channel component. To form clean, attractive
corners, the Bottom Channel component can be notched with snips (seefigure 3) an
then bent to the desired angle. (Attached to the ground as shown infigut

channel to treated wooden stakes. For installation on concrete, use 3/4" masonry nail
instead of ground spike. "Liquid Nail" cement or other similar methods of setting a
fastener directly to concrete can also be used.
Step 2...
Mounting Top Trim Back Components
First determine where the Top Trim Back component(s) will mount on the lower part
of the home, The bottom edge of the Top Trim Back component can extend below
the bottom edge of the home if there is a solid support for nailing and a solid bearing
for the Top Trim Front component against the side of the home. It is helpful to mark
a line around the bottom of the home with a chalk line or other method to assure a
straight line where the Top Trim Back component is to be installed,
The Top Trim Back component is installed by driving the fasteners in the middle of
every slot (seefigure 5). Do notfasten tightly! (see fastening instructions on front
page.) Do not cut Top Trim Back components at the corners. Gently bend over a
sharp edge of a cutting table or a similar surface to form a corner (seefigure 6),
If the installation is made in extremely cold weather, the vinyl should be warmed to
room temperature before bending. Warming will avoid the likelihood of cracking,
Step 3...
Cutting Top Trim Back Components
The Top Trim Back component is constructed with two parallel ridges at intervals
below the nailing slots (seefigure7). These ridges may be used to measure the
distance from the_�ground to the lower ridge. In cold w4mhavw
In warm weather, measure to the bottom ridge. If the ground is level, several panels
may be cut at one time using a hand power saw. Remember, if a power saw is to be
used, mount afine-toothed blade in reverse position (seefigure 8).
Locking the Skirting Panels
A snali lock tool is used to punch locking tabs on the outside bottom edge of each skirt-
ing panel (seefigure 9). When the panel is installed, it becomes locked in the Bottom
Channel comyonent. This feature assures retention of the panel in
Note: When installing in a high wind area, you may also punch locking tabs at the
top of the panel for added locking strength.
Installing the Skirting Panels
Panel can be installed by setting into the Bottom Channel and leaning against the top
trim back. Lock the next panel as shown in figure 10, Panels should not be cut but
bent around corners as shown in figure 11.
Fitting Skirting Panels Around Service Connections
Cutting and fitting to virtually any shape or radius is easily done with Vinyl Skirting,
Using aviation snips, cut the panel to fit around the connection. Cut the panel from
the side — not from the top or bottom. Keep snip points open as if cutting cloth to
avoid cracking panels.
Proper installation of manufactured home skirting requires
that the Top Back component be fastened loosely so the panel
will slide freely in the nail slots. This can be accomplished by
leaving the fastener 1/ 16" to 1/8 " from the face of the panel
The fastener must penetrate a solid surface by 3/4".
Do not place outdoor cookers near the vinyl skirting
because the heat wil I distort the panels. Any heat source

Step 4...
Instafling the Top Thm Front Component
The Top Trim Front component(s) installs easily by snapping the top edge of its
spring lock into the installed Top Trim Back component. Be sure to push the Top Trim
Front component all the way into the TopThm Back component until it 4Snaps" into place.
Each of the 15 pieces of the Top Trim Front components in the trim kit are notched 2"
on one end (seefigure 12) to permit overlapping. Overlap ends of adjoining Top Trim
Front component(s) approximately I ".
Cutting Additional Corners
If inside comers are needed, trim strips can be easily cut with aviation snips to form
attractive corner joints by cutting a 450 mitre on adjacent ends and butting. If extra
outside corners are required (for porches or add -on rooms), notch the trim strips as
shown (see figure 13), bend around the corner and snap into place. Allow at least 3' of
trim strip on each side of the comer,
Easy Access
Access can be gained at virtually any point by simply lifting the Top Trim Front com-
ponent and sliding out the desired number of panels. Accessibility to the area under
the unit is available whenever desired,
Final Configuration
Please review the drawing to the right that shows how the components and skirting
panels look after installation is complete.
