We spoke regarding the sign permit application for 38128 North Avenue when the wall cabinet was
denied. We have revised the layout to channel letters, which Gail Hamilton has signed off on. Included

that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings, change of rise in existing buildings, or
expansion of existing buildings, as specified In Pasco County Ordinance number 89-07 and 90-07, as amended. The undersigned also understands, that
such fees, as may be due, will be identified at the time of pemitting. it is further understood that Transportation impact pees and Resource Recovery
Fees must be paid prior to receiving a "certificate of occupancy or final power release. if the project does not involve a certificate of occupancy or final
power release, the fees must be paid prior to permit issuance. Furthermore, 1 Pasco County Water/Sewer Impact fees are due, they must be paid prior
to permit issuance in accordance with applicable Pasco County ordinances.
CONSTRUCTION LIEN LAW (Chapter 713, Florida Statutes, as amended): If valuation of work is $2,500.00 or more, I certify that I, the applicant,
have been provided with a copy of the"Florida Construction Lien Law -Homeowner's Protection Guide" prepared by the Florida Department of
Agriculture and Consumer Affairs. If the applicant is someone other than the "owner", 1 certify that i have obtained a copy of the above described
document and promise in good faith to deliver it to the "owner" prior to commencement.
CONTRACTOR'S/OWNER'S AFFIDAVIT: I certify that all the information in this application is accurate and that all work will be done in compliance with
all applicable laws regulating construction, zoning and land development. Application is hereby made to obtain a permit to do work and installation as
indicated, I certify that no work or installation has commenced prior to issuance of a permit and that all work will be performed to meet standards of all
laws regulating construction, County and City codes, zoning regulations, and land development regulations in the jurisdiction. I also certify that I
understand that the regulations of other govemment agencies may apply to the intended work, and that it is my responsibility to identify what actions i
must take to be in compliance. Such agencies include but are not limited to:

TERMS AND CONDITIONS
These Terms and Conditions (the Agreement) is hereby incorporated into that certain proposal or statement of work (the SOW, and such services
described in the SOW, the Services) entered into between COMMERCIAL FIRE AND COMMUNICATIONS, INC,, a Florida corporation (CFC), and you
(Customer),
SERVICES
This Agreement shall become effective when the SOW is signed by both Customer and CFC: (such date, the Effective Date). CFC shall perform the
Services set forth in the SOW pursuant to the terms and conditions of this Agreement and the SOW.
CANCELLATION, TERMINATION
Customer may not terminate this Agreement andlor the SOW except upon CFCs written consent. CFC may terminate this Agreement and the SOW
immediately and without notice in the event (a) Customer breaches any term of this Agreement and/or the SOW, (b) any receivership, bankruptcy,
assignment for the benefit of creditors or any other form of insolvency proceedings are filed or instituted by or against Customer, (c) Customers premises
or fire alarm system are attached, liened or seized by process of law and such attachment or lien is not vacated, or such seizure is not terminated, within
ten (10) days after its occurrence, or (d) there is any act or omission on the part of Customer whereby CFC is prevented from completing the Services by
the delivery date(s) and completion date(s) set forth in the SOW. In the event CFC terminates this Agreement and/or the SOW in accordance with the
foregoing sentence or in the event Customer terminates this Agreement and/or the SOW for any reason, an amount equal to the value of the Services
already provided by CFC and all fees and expenses incurred or earned in connection with the performance of the Services through the effective date of
termination of this Agreement and/or the SOW plus an amount equal to all remaining payments for the Services set forth in the SOW shall immediately
become due and payable by Customer to CFC.
Customer agrees to compensate CFC as set forth in the SOW, and Customer will be billed as set forth in the SOW and in the amount set forth in the SOW.
In addition, Customer shall reimburse CFC its actual out-of-pocket expenses as reasonably incurred by CFC in connection with its performance of the
Services. All fees, charges and other amounts payable to CFC hereunder do not include any sales, use, excise, value added or other applicable taxes,
tariffs or duties (whether foreign or domestic), payment of which shall be the sole responsibility of Customer.
In addition to the prices and compensation specified in the SOW, Customer shall pay for all extra work performed by CFC and requested by Customer or
made necessary because of incompleteness of or inaccuracy in the plans or other information submitted by Customer with respect to the location, type of
occupancy or other details related to the Services to be performed hereunder. Customer acknowledges that the prices, delivery date(s) and completion
date(s) set forth in the SOW are based upon information contained in CFCs records as of the Effective Date and provided by Customer with respect to the
layout of Customers facilities. In the event the layout of Customer's facilities has been altered from the information contained in CFCs records as of the
Effective Date and provided by Customer, or is altered prior to completion of this Agreement and/or the SOW, Customer shall advise CFC of any
alterations, and such prices, delivery date(s) and completion date(s) set forth in the SOW shall be changed by CFC as may be required because of such
alterations. Unless otherwise agreed in writing by CFC, the prices applicable to the extra work performed by CFC pursuant to this paragraph will be billed to
Customer at CFC's then -current rates and prices.
In the event there are any changes, alterations or additions to the plans, specifications or construction schedule related to the Services to be performed

Unless otherwise specified by Customer in writing, all installation work in connection with the Services will be performed during normal business hours. If
Customer shall require any overtime labor in connection with the Services, Customer agrees to pay CFC for the overtime premium on same at CFCs then -
current rates and prices for such overtime labor,
NO WAIVER
No waiver of any breach of thisAgTeement or the SOW shall constitute ewaiver ofosubsequent breach.

SURVIVAL
The termination urexpiration nfthis Agreement or the SOW will not relieve the parties from complying with any provisions of this Agreement and/or the
SOW that contemplate performance subsequent to termination or expiration (including, but not limited to, limitation of liability and indemnification
