UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSiBILITIES: If the owner has hired a contractor or contractors to undertake work, they
may be required to be licensed in accordance with state and local regulations. If the contractor is not licensed as required by law, both the owner and
contractor may be cited for a misdemeanor violation under state law. If the owner or Intended contractor are uncertain as to what licensing requirements

TRANSPORTATION IMPACT/UTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands
that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings, change of use in existing buildings, or
expansion of existing buildings, as specified in Pasco county Ordinance number W07 and 90-07, as amended. The undersigned also understands, that
such fees, as may be due, will be Identified at the time of permitting. It is further understood that Transportation impact Fees and Resource Recovery
Fees must be paid prior to receiving a "certificate of occupancy" or final power release. If the project does not involve a certificate of occupancy or final
power release, the fees must be paid prior to permit issuance. Furthermore, if Pasco County WateUSewer Impact fees are due, they must be paid prior
to permit issuance in accordance with applicable Pasco County ordinances.
CONSTRUCTION LIEN LAW (Chapter 713, Florida Statutes, as n ended), If valuation of work is $2,500.00 or more, I certify that 1, the applicant,
have been provided with a copy of the "Florida Construction Lien Law --Homeowner's Protection Guide" prepared by the Florida Department of
Agriculture and Consumer Affairs. If the applicant is someone other than the "owner, i certify that I have obtained a copy of the above described
document and promise in good faith to deliver it to the 'owner' prior to commencement.
CONTRACTOR'SIOWNER'S AFFIDAVIT: I certify that all the information in this application is accurate and that all work will be done in compliance with
all applicable laws regulating construction, zoning and land development. Application is hereby made to obtain a permit to do work and installation as
indicated. I certify that no work or installation has commenced prior to issuance of a permit and that all work will be performed to meet standards of all
laws regulating construction, County and City codes, zoning regulations, and land development regulations in jurisdiction I also certify that I
understand that the regulations of other government agencies may apply to the intended work, and that if is my responsibility to identify what actions I
must take to be in compliance. Such agencies include but am, not limited to:
- Department of Environmental Protection -Cypress Eayheads, Wetland Areas and Environmentally Sensitive
Lands, Water/Wastewater'Treatment.
- Southwest Florida Water Management District -Wells, Cypress Sayheads, Wetland Areas, Altering

- Use of fill is not allowed in Flood Zone "V' unless expressly permitted.
4 If the fill material is to be used in Flood Zone "A", it is understood that a drainage plan addressing a
"compensating volume" will be submitted at time of permitting which is prepared by a professional engineer
licensed by the State of Florida.
- If the fill material is to be used in Flood Zone "A" in connection with a permitted building using stem wall
construction, I certify that fill will be used only to Till the area within the stem wall.
- If fill material is to be used in any area, I certify that use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversely affect adjacent properties, the owner may be cited for violating
the conditions of the building permit issued under the attached permit application, for lots less than one (1)
acre which are elevated by fill, an engineered drainage plan is required.
If I am the AGENT FOR THE OWNER, I promise in good faith to inform the owner of the permitting conditions set forth in this affidavit prior to
commencing construction. I understand that a separate permit may be required For electrical work, plumbing, signs, yells, pools, air conditioning, gas, or
other installations not specifically included in the application. A permit issued shall be construed to be a license to proceed with the work and not as
authority to violate, cancel, alter, or set aside any provisions of the technical codes, nor shall Issuance of a permit prevent the Building Official from
thereafter requiring a correction of errors in plans, construction or violations of any codes. Every permit issued shall become Invalid unless the work
authorized by such permit is commenced within six months of permit issuance, or R work authorized by the permit is suspended or abandoned for a
period of six (6) months after the time the work is commenced. An extension may be requested, in writing, from the Building Official for a period not to
exceed ninety (90) days and will demonstrate justifiable cause for the extension. If work ceases for ninety (90) consecutive days, the job is considered

GENERAL INSTRUCTIONS-
1, All pads are to be installed flat side down, ribbed side up.
2. The ground under the pads should be leveled as smooth as possible with all vegetation and debris removed. Pads to be placed on evenly
compacted soil, at or below the frost line unless otherwise protected from frost by controlling the temperature and/ or moisture content of the soil
underneath the home.
3. Pier & pad placement will be determined by the manufactured homes' written set-up instructions or any local or state codes.
4. Center blocks on ABS pad and complete pier.
5. The open cells between the ribbing on the upper side of the pads may be filled with soil or sand after installation to prevent any accumulation of
stagnant water in the pads,
6. A pocket penetrometer may be used to determine the unconfined compressive strength of the soil. If no soil testing equipment is available --
use an assumed soil value of 1000 lb& / square foot.
NOTES:
1. All pad sizes shown are nominal dimensions and may vary up to 1/8".
2. The maximum deflection in a single pad is 5/8" measured from the highest point to the lowest point of the top face.
(NOTE: Actual test results were less than 5/8")
3. Pad loads are the same when using single stack or double stack blocks.
4. The maximum load at any intermediate soil value may be interpolated between the next lower and next higher soil values given in the table
below.
5. Any ABS pad configuration may be used to replace a home manufacturer's recommended concrete or wood base pad.
6. Steel Piers.- All pads are tested with steel piers on 1000 PSF soil density unless otherwise noted. If required, attach with
2" #12 x Y2." hex tech screws, Minimum Pier Base 7 1/4". Multi -Pad configurations require a minimum 9 1/4" pier base,
7. Available pads tested on 2000 PSF soil capacity using steel piers are: ID#1055-14, 1055-9,1055-7 and 1055-13.
8. If soil capacities exceed 3000 psf, use the 3000 psf soil values from the table.
9. Any pad may be stacked directly on top of an identical pad. The second pad should also be installed fiat side down. Such a configuration
provides the same allowable load capacity as the single pad.

1.0 EVALUATION SCOPE

2.0 USES
2.1 General:
Oliver Technologies, Inc. ABS Pier Pads, Caps and Shims
are used as part of a foundation footing to distribute
concentrated loads from the frame rail of a manufactured
housing unit, through a pier and distribute the load to the
underlying soil,
2.2 Shims-
ABS Shims are used to distribute concentrated loads from
the frame rail of a manufactured housing unit, to the top of
the pier cap.
2.3 Caps,
ABS Pier Caps on top of the CMU pier are used to distribute
concentrated loads from the frame rail of a manufactured
housing unit, to the top of the pier.
3.0 DESCRIPTION

Oliver Technologies, Inc. ABS Pier Pads, Caps and Shims
are injection molded products and are manufactured using
black ABS and PC/ABS materials. Pier Pads are intended
to be used as footings for manufactured housing as
described in 24 CFR Section 3285.312(a)(3). The pad has
one smooth side intended for contact with the soil and one
side with patterns of ribs designed to stiffen the pad and
transfer load from a pier erected on top of the pad to the soil
the pad bears on. See Figure 1 e Pads are available in sizes
as described in Table I. Pad Caps are intended for use on
top of CMU piers for manufactured housing as described in
24 CFR Section 3285.304(b). Pier Caps are 71/4 inches
wide by 141/2 inches long by 11/2 inches tall. See Figure 2.
Shims are intended for use in manufactured housing on top
of CMU piers and pier caps as described in 24 CFR Section
3285,304(c). Shims are wedge-shaped 31/2 inches wide by
6 inches long and 1 inch deep at the thickest end. Shims
have interlocking texture on adjacent faces when installed,
See Figure 3.
3.2 Materials:
Oliver Technologies, Inc. ABS Pier Pads, Caps and Shims
are injection molded parts using ABS and PCIABS materials
complying with the manufacturer's specifications.
4.0 DESIGN AND INSTALLATION
4.1 Design:
The scope of this report is limited to evaluation of ABS Pier
Pads, Caps and Shims for use in manufactured housing unit
foundation systems. Details related to incorporation of the
product beyond that scope are the responsibility of the
registered design professional. See Table I and 2 for ABS
pier pad models and maximum load capacities for single
and double CMU piers. See Table 3 for maximum allowable
loads for pier Gaps and shims. The combinations of use
between caps, shims and pads is limited to the least
capacity found in the design tables and the design is the
responsibility of the registered design professional.

4.2 Installation:
General: Oliver Technologies, Inc. ABS Pier Pads, Caps
and Shims must be fabricated, identified and erected in
accordance with this report, the approved construction
documents and the applicable code. In the event of a
conflict between the manufacturer's published installation
instructions and this report, this report governs. Approved
construction documents must be available at all times on the
jobsite during installation,
4.2.1 Oliver Technologies, Inc. ABS Pier Pads may be
used to provide a manufactured housing unit foundation
footing to distribute concentrated pier loads to the
underlying soil. Pier pads shall be used with the flat side
bearing on the soil and the ribbed side facing up. Pier pads
may be used on soils with unconfined compressive strength
between 1000 and 3000 psf. The soil under the pad must
be flat and level, and all vegetation must be removed from
the area. Pads may be used with single- or double -stack
CMU pier system when the pier is centered on the pad and
the pad orientation is as designated in the manufacturers
installation instructions, Use of pier pads stacked in
combinations is beyond the scope of this report.
4.2.2 Oliver Technologies, Inc. ABS Pier Shims may be
used with or without ABS Caps or code -prescriptive pier
caps. Shims must be used in pairs and must be driven in
tightly so that they do not occupy more than 1 inch of vertical
height in accordance with 24 CFR Section 3285.306.
Opposing ABS Pier Shims may be installed in a range of
configurations, Shims may range between fully interlocking
along the entire length of the opposing shims to no gap
between thinnest ends,
4.2.3 Oliver Technologies, Inc. ABS Pier Caps may be
in acco ance with 2A CFR Section 3285.306 and this
report. The maximum gap between shims when driven
together may not exceed 1 inch and must be used in pairs
5.0 CONDITIONS OF USE
The Oliver Technologies, Inc. ADS Pier Pads, Caps and
Shims described in this report comply with, or are suitable
alternatives to what is specified in, those codes listed in
Section 1.0 of this report, subject to the following conditions:
5.1 Product is produced and labeled in a manufacturing
location identified in this report under a quality control
program with inspections by ICC-Es.
5.2 Installation must comply with this report, the
manufacturer's installation instructions and the
approved construction documents.
53 The design pier load may not exceed the lesser of
the Pier Pad capacity, the unconfined compressive
strength of the soil, or the pier capacity.
5.4 The design pier load must not exceed the capacity of
the Pier Cap when used, The design pier load must not
exceed the Pier Cap capacity. Where required by the
authority having jurisdiction, manufactured housing
unit structures using Pier Caps must be designed by a
registered design professional to ensure allowable
loads are not exceeded. Pier Caps are designed for
use on 8-inch square end regular CMU blocks. Pier
Caps used on single block piers must be centered over
the top block with the entire perimeter bearing on the
block. Pier Caps used on double stacked blocks must
span the joint between the blocks and the perimeter
must be bearing on the blocks.
&5 The design pier load must not exceed the capacity of
the Pier Shim when used independently. The design
pier load must not exceed the lesser of the Pier Shim
capacity or Pier Pad Cap capacity when used together.
Where required by the authority having jurisdiction,
manufactured housing unit structures using Pier Shims
to fill the gaps between a pier and the frame must be
designed by a registered design professional to ensure
allowable loads are not exceeded. Pier Shims must be
used in pairs, fully supported along the bottom surface
by the pier. Total height of the gap filled by a Pier Shim
set must not exceed 1 -inch.
5.6 ABS Pier Pads, Caps and Shims, when installed, must
be shielded from direct exposure to UV light.
5.7 ABS Pier Pads are rated for the loads listed in Tables
I and 2 for use on soils having unconfined
compressive strength from 1000 psf to 3000 psf,
Actual soil bearing strength must be verified at the
depth of installation.
5.8 Pier and Pier Pad spacing is the responsibility of the
registered design professional and must be specified
in the written set-up instructions provided by the
housing manufacturer.
5.9 In freezing climates, the footing pad must be installed
below the frost line or be otherwise protected from the
effects of frost heave as permitted in 24 CFR Section
3285.
5.10 ABS Footing Pads, Pier Caps and Shims must not be
used to resist lateral or uplift loads.
5.11 ABS Footing Pads, Pier Caps and Shims have not
been evaluated for resisting or transmitting seismic
loads
5.12 ABS Footing Pads, Pier Caps and Shims must not be
field modified.
