NOTICE OF DEED RESTRICTIONS: The undersigned understands that this permit maybe subject to "deed" restrictions" which maybe more restrictive
than County regulations. The undersigned assumes responsibility for compliance with any applicable deed restrictions.
UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES: if the owner has hired a contractor or contractors to undertake work, they
may be required to be licensed in accordance with state and local regulations. If the contractor is not licensed as required by law, both the owner and
contractor may be cited for a misdemeanor violation under state law. if the owner or intended contractor are uncertain as to what licensing requirements

TRANSPORTATION IMPACT(UTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands
that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings, change of use in existing buildings, or
expansion of existing buildings, as specified in Pasco County Ordinance number 89-07 and 90-07, as amended. The undersigned also understands, that
such fees, as may be due, will be identified at the time of permitting. It is further understood that Transportation Impact Fees and Resource Recovery
Fees must be paid prior to receiving a "certificate of occupancy" or final power release. if the project does not involve a certificate of occupancy or final
power release, the fees must be paid prior to permit issuance. Furthermore, if Pasco County Water/Sewer Impact fees are due, they must be paid prior
to permit issuance in accordance with applicable Pasco County ordinances.
CONSTRUCTION LIEN LAW (Chapter 713, Florida Statutes, as amended): If valuation of work is $2,500.00 or more, I certify that 1, the applicant,
have been provided with a copy of the "Florida Construction Lien Law —Homeowner's Protection Guide" prepared by the Florida Department of
Agriculture and Consumer Affairs. If the applicant is someone other than the "owner", I certify that I have obtained a copy of the above described
document and promise in good faith to deliver it to the "owner" prior to commencement.
CONTRACTOR'S10W NER'S AFFIDAVIT: I certify that all the information in this application is accurate and that all work will be done in compliance with
all applicable laws regulating construction, zoning and land development. Application is hereby made to obtain a permit to do work and installation as
indicated. I certify that no work or installation has commenced prior to issuance of a permit and that all work will be performed to meet standards of all
laws regulating construction, County and City codes, zoning regulations, and land development regulations in the jurisdiction. I also certify that 1
understand that the regulations of other government agencies may apply to the intended work, and that it is my responsibility to identify what actions I
must take to be in compliance. Such agencies include but are not limited to:
- Department of Environmental Protection -Cypress Bayheads, Wetland Areas and Environmentally Sensitive
Lands, Water/Wastewater Treatment.
Southwest Florida Water Management District -Wells, Cypress Bayheads, Wetland Areas, Altering
Watercourses.
Army Corps of Engineers -Seawalls, Docks, Navigable Waterways.
Department of Health & Rehabilitative Services/Environmental Health Unit -Wells, Wastewater Treatment,
Septic Tanks.

I understand that the following restrictions apply to the use of fill:
- Use of fill is not allowed in Flood Zone "V" unless expressly permitted.
- If the fill material is to be used in Flood Zone "A", it is understood that a drainage plan addressing a
"compensating volume" will be submitted at time of permitting which is prepared by a professional engineer
licensed by the State of Florida.
- If the fill material is to be used in Flood Zone "A" in connection with a permitted building using stem wall
construction, I certify that fill will be used only to fill the area within the stem wall.
- If fill material is to be used in any area, 1 certify that use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversely affect adjacent properties, the owner may be cited for violating
the conditions of the building permit issued under the attached permit application, for lots less than one (1)
acre which are elevated by fill, an engineered drainage plan is required.
If I am the AGENT FOR THE OWNER, I promise in good faith to inform the owner of the permitting conditions set forth in this affidavit prior to
commencing construction. I understand that a separate permit may be required for electrical work, plumbing, signs, wells, pools, air conditioning, gas, or
other installations not specifically included in the application. A permit issued shall be construed to be a license to proceed with the work and not as
authority to violate, cancel, alter, or set aside any provisions of the technical codes, nor shall issuance of a permit prevent the Building Official from
thereafter requiring a correction of errors in plans, construction or violations of any codes. Every permit issued shall become invalid unless the work
authorized by such permit is commenced within six months of permit issuance, or if work authorized by the permit is suspended or abandoned for a
period of six (6) months after the time the work is commenced. An extension may be requested, in writing, from the Building Official for a period not to
exceed ninety (90) days and will demonstrate justifiable cause for the extension. if work ceases for ninety (90) consecutive days, the job is considered
abandoned.

The Customer shall pay the Contractor the total agreed upon sum of $ 28,590 ("Contract Price") for and in consideration of all material and
labor that is provided and performed under this Sales Contract. This Contract is not contingent upon financing or other lender approval. Contractor
does not provide financing but will assist Customer in securing outside financing at the specific request of the Customer in accordance with Article
23 herein. Should Customer request financing assistance, Contractor makes no warranties or representations regarding Customer's ability to secure
outside financing, its terms, rates or conditions.
BUYER'S RIGHT TO CANCEL. This is a home solicitation sale, and if you do not want the goods or services, you may cancel this
agreement by providing written notice to the seller in person, by telegram, or by mail. This notice must indicate that you do not
want the goods or services and must be delivered or post -marked before midnight of the third business day after you sign this
agreement. If you cancel this agreement within the three business days alotted, the seller may not keep all or part of the cash

Exclusions from Work: Except asexplicitly set forth inthis Agreement, weare not responsible for: (i)changes or
alterations from this Agreement and the Customer Packet that may herequired byapplicable law, requirements of government agencies,
or your Utility; (ii) correction of an inadequate existing electrical supply or structural components of your Property;(iWprior unpennitta]
work atyour Property; (hv)any work required due to your Property not complying with current standards or codes; (v) any trenching or
excavation required for installation; and (vi) the removal of asbestos or the costs of an investigation to determine whether asbestos is
present. Unless expressly set forth in this Agreement, you represent to us that the Property's roofing and electrical systems are adequate
for the System and the work to be performed pursuant to this Agreement. Any work necessary to correct or add the above items will he
considered Extra Work.

7. Discretionary Design Change: wemay, inour sole discretion, redesign the System to: (i)exclude one (1)solar
module, nrits equivalent wattage not tpexceed 350watts n,toincrease the size o[the system including, without limitation, byinstalling
more solar modules or increased efficiency solar modules whose wattage may exceed those reflected in the Customer Packet, and/or (ii)
modify the location of the installation of the System on the roof at the Property without a Change Order.

Sales Contract, or to the interpretation, performance, breach or termination thereof may be resolved by binding arbitration administered

Procedures, before asingle arbitrator. Such arbitration shall take place at the offices of the AAA in Miami, Florida or such other location
asthe parties tosuch arbitration may agree. The parties expressly agree that nothing in this Sales Contract will prevent the parties from
applying to a court that would otherwise have jurisdiction for injunctive or other equitable relief. After the arbitrator isselected, he/she
will have sole jurisdiction to hear such applications, except that the parties agree that any relief ordered by the arbitrator may be
immediately and specifically enforced by a court otherwise having jurisdiction over the parties. The arbitrator shall apply the same
substantive law, with the same statutes of limitations and same remedies that would apply if the claims were brought in a court of law.
The decision ofthe arbitrator shall befinal and binding nnthe parties. The decision nfthe arbitrator shall beconclusive and may be
enforced in a court of competent jurisdiction, subject only to appeal to an appellate arbitrator in the event of the arbitrator's
misapplication of the law, no evidence to support the award, or such other grounds for appeal of arbitration awards that exist by statute.
This arbitration provision shall survive closing, breach or termination of this Sales Contract. The parties specifically agree that the FEDERAL

10. UquidatedDamages: The parties hereby agree that, )nthe event Customer rescinds urcancels this Sales Contract
after the expiration nfthree business days, with or without cause, Contractor shall be entitled to liquidated damages in an amount of
$1,500.00 plus costs of all incurred costs and expenses to include but not be limited to permits, surveys, third party inspections, plans,
blueprints, non-refundable product orders, specialty orders and recording fees.

Waiver and Parole Evidence: The waiver by any party hereto of the breach of any provisions of this Sales Contract
shall not operate or be constructed as a waiver of any subsequent breach by any party. This instrument contains the entire agreement of
the parties and may not be changed except by written agreement duly executed by the parties hereto. This Sales Contract shall not be
assignable byCustomer.

voluntarily signed this Sales Contract and understands that he/she is hereby authorizing Contractor tocomplete the work performed

governmental entity; and (viii) unless expressly stated otherwise, all references to articles, sections, subsections, orsubparagraphs are to
provisions ofthis Sales Contract. The parties are sophisticated and were or could have been represented by counsel during the
negotiation ofthis Sales Contract. Customer and Contractor agree and acknowledge that the parties jointly negotiated this Sales Contract
and the party who drafted any provision ofthis Sales Contract shall not suffer any adverse presumptions axaresult ofthat fact.

(A)10-Year Limited Roofing Penetration Warranty. Energy Solutions Direct warrants your roof against damage and water infiltration at each roofing
penetration made by Energy Solutions Direct in connection with the installation of the system and the immediate surrounding area of each such
penetration (collectively, the "Covered Roof Areas"). Energy Solutions Direct will repair damage to your roof and repair or compensate you for actual
physical damage to your property resulting from any water infiltration in the event of failure or defects in the Covered Roof Areas. This 10-Year Limited
Roofing Penetration Warranty will extend for a period of ten (10) years from the date installation of the System is completed by Energy Solutions Direct ;
provided, however, in the event the roof is more than twenty (20) years old (orjudged to be in poor/unacceptable condition by Energy Solutions Direct) at
commencement of the System installation, this Limited Roofing Penetration Warranty will be limited to a period of five (5) years from the date installation
of the System is completed by Energy Solutions Direct unless You provide Energy Solutions Direct with a report from a state licensed roofing contractor
that proves to Energy Solutions Direct's satisfaction that the roof is in an acceptable condition based upon an inspection conducted no more than ninety
(90) days before commencement of the System installation. Energy Solutions Direct assumes no liability for or responsibility in relation to any existing
roofing warranties during or after the installation of the System except to the extent of the 10-Year Limited Roofing Penetration Warranty.
(8)10-Year Limited Labor and Service Warranty. Energy Solutions Direct will provide, at no cost to Customer, reasonable* labor and services support in
connection with the repair or replacement of any material, equipment, product, or component incorporated into and made a part of the System (each a
"System Product" and collectively the "System Products") that is covered under a warranty provided by any supplier, manufacturer, subcontractor,
vendor, or other applicable third party (each a "Supplier" and collectively the "Suppliers") of such System Product. This 10-Year Limited Labor and Service
Warranty will extend for a period of ten (10) years from the date installation of the System is completed by Energy Solutions Direct. *"Reasonable" shall be
defined as one annual service call per year during the warranty period. Supplemental service calls thereafter will be subject to a labor service fee of
