NOTICE OF DEED RESTRICTIONS: The undersigned understands that this permit may be subject to "deed" restrictions" which may be more restrictive
than County regulations. The undersigned assumes responsibility for compliance with any applicable deed restrictions.
UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES: If the owner has hired a contractor or contractors to undertake work, they
may be required to be licensed in accordance with state and local regulations. If the contractor is not licensed as required by law, both the owner and
contractor may be cited for a misdemeanor violation under state law. If the owner or intended contractor are uncertain as to what licensing requirements
may apply for the intended work, they are advised to contact the Pasco County Building Inspection Division —Licensing Section at 727-847- 8009.
Furthermore, if the owner has hired a contractor or contractors, he is advised to have the contractor(s) sign portions of the "contractor Block" of this
application for which they will be responsible. If you, as the owner sign as the contractor, that may be an indication that he is not properly licensed and is
not entitled to permitting privileges in Pasco County.
TRANSPORTATION IMPACTIUTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands
that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings, change of use in existing buildings, or
expansion of existing buildings, as specified in Pasco County Ordinance number 89-07 and 90-07, as amended. The undersigned also understands, that
such fees, as may be due, will be identified at the time of permitting. It is further understood that Transportation Impact Fees and Resource Recovery
Fees must be paid prior to receiving a "certificate of occupancy" or final power release. If the project does not involve a certificate of occupancy or final
power release, the fees must be paid prior to permit issuance. Furthermore, if Pasco County Water/Sewer Impact fees are due, they must be paid prior
to permit issuance in accordance with applicable Pasco County ordinances.
CONSTRUCTION LIEN LAW (Chapter 713, Florida Statutes, as amended): If valuation of work is $2,500.00 or more, I certify that I, the applicant,
have been provided with a copy of the "Florida Construction Lien Law —Homeowner's Protection Guide" prepared by the Florida Department of
Agriculture and Consumer Affairs. If the applicant is someone other than the "owner", I certify that I have obtained a copy of the above described
document and promise in good faith to deliver it to the "owner" prior to commencement.
CONTRACTOR'S/OWNER'S AFFIDAVIT: I certify that all the information in this application is accurate and that all work will be done in compliance with
all applicable laws regulating construction, zoning and land development. Application is hereby made to obtain a permit to do work and installation as
indicated. I certify that no work or installation has commenced prior to issuance of a permit and that all work will be performed to meet standards of all
laws regulating construction, County and City codes, zoning regulations, and land development regulations in the jurisdiction. I also certify that I
understand that the regulations of other government agencies may apply to the intended work, and that it is my responsibility to identify what actions I
must take to be in compliance. Such agencies include but are not limited to:
- Department of Environmental Protection -Cypress Bayheads, Wetland Areas and Environmentally Sensitive
Lands, Water/Wastewater Treatment.
- Southwest Florida Water Management District -Wells, Cypress Bayheads, Wetland Areas, Altering
Watercourses.
- Army Corps of Engineers -Seawalls, Docks, Navigable Waterways.
- Department of Health & Rehabilitative Services/Environmental Health Unit -Wells, Wastewater Treatment,
Septic Tanks.
- US Environmental Protection Agency -Asbestos abatement.
- Federal Aviation Authority -Runways.
I understand that the following restrictions apply to the use of fill:
- Use of fill is not allowed in Flood Zone W" unless expressly permitted.
- If the fill material is to be used in Flood Zone "A", it is understood that a drainage plan addressing a
"compensating volume" will be submitted at time of permitting which is prepared by a professional engineer
licensed by the State of Florida.
- If the fill material is to be used in Flood Zone "A" in connection with a permitted building using stem wall
construction, I certify that fill will be used only to fill the area within the stem wall.
- If fill material is to be used in any area, I certify that use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversely affect adjacent properties, the owner may be cited for violating
the conditions of the building permit issued under the attached permit application, for lots less than one (1)
acre which are elevated by fill, an engineered drainage plan is required.
If I am the AGENT FOR THE OWNER, I promise in good faith to inform the owner of the permitting conditions set forth in this affidavit prior to
commencing construction. I understand that a separate permit may be required for electrical work, plumbing, signs, wells, pools, air conditioning, gas, or
other installations not specifically included in the application. A permit issued shall be construed to be a license to proceed with the work and not as
authority to violate, cancel, alter, or set aside any provisions of the technical codes, nor shall issuance of a permit prevent the Building Official from
thereafter requiring a correction of errors in plans, construction or violations of any codes. Every permit issued shall become invalid unless the work
authorized by such permit is commenced within six months of permit issuance, or if work authorized by the permit is suspended or abandoned for a
period of six (6) months after the time the work is commenced. An extension may be requested, in writing, from the Building Official for a period not to
exceed ninety (90) days and will demonstrate justifiable cause for the extension. If work ceases for ninety (90) consecutive days, the job is considered
abandoned.

CLIENT acknowledges that he/she has read and received a copy of this contract, including Terms &
Conditions contained, attached, or on the reverse side which are incorporated herein. All work on the
above -mentioned property herein will be performed in a timely and professional manner. Buyer/client may cancel
this contract without any penalty or obligation within three (3) business days from the above date by
providing written notice by US Certified Mail, Return Receipt Requested to Urban Solar Inc. This cancellation
must be clear in intent, written in English and postmarked before midnight of the third business day after

This letter is regarding the installation of a new roof mounted Solar PV System on the existing
residential structure at the address above. I have reviewed the attachment plan and have
determined that the roof mounted PV system is in compliance with the applicable sections of the
following Codes as amended and adopted by the jurisdiction:
2020 Florida Building Code 70, Edition, FBC
ASCE 7 Min. Design Loads for Buildings & Other Structures
Per 2020 FBC, the Roof Mounted PV system will be subject to the following design criteria:
Design Wind Speed (V„it) - 140mph 3sec gust, Exposure Category - C
The PV System consist of the modules, railing, and connection hardware. The system will add a
dead load of approximately 3 psf to the roof.
The existing roof covering is Asphalt Shingle with rain. 1/2" plywood decking and 2" x 4" roof trusses
24" O.C. The roofing, decking, and roof trusses are in good condition. The existing structure will be
adequate for supporting the additional PV dead load and wind loads.
The securement method of the PV system is to be flush mounted to the asphalt shingle roof with the
lronridge railing and QuickMount L-Mount. The attachments can be attached up to 48" apart in roof
zone 1_ and 24" apart in roof zones 2e, 2r & 3. The mounts should be staggered, where possible, to
allow distribution of the design loads evenly to the structure. The mounts shall be installed using
Sf 16" x 4" stainless steel Lag bolts with minimum 2-5J16" thread length.

Contains Phthalates, Methylene Dipheny| /smcyanate (MD[),
Carbon Black or Titanium Dioxide inextricably bound in a
polymer matrix. If in eyes or on skin, rinse with water for at least 15
minutes. If on clothes, remove clothes. If breathed in, move person
to fresh air. If swallowed, call a Poison Control Center or doctor
immediately.

APPLICATION LIMITATIONS
Construction substrates have become complex and diverse
by nature and origin. Substrate chemistries and structures can
interfere with adhesive performances of the sealant. Adhesion
to Substrate Pretest (ASP) is therefore MANDATORY to assess any
adhesion and seating characteristics —see Adhesion to Substrates
Pretest (ASP)section and see Installation Protocol section. This
must be done pre -installation to avoid potential failures. Cati.

Do not apply over damp, contaminated, loose surfaces (See
Installation Protocol and Surface Preparation), old sealants or
other foreign substances that may impair the adhesion bond.
Avoid air entrapment.
Dampness and substrates with high moisture content will trigger
extensive curing of the sealant within a very short period of time.
This may cause an excess of bubbling and foaming within the
sealant and at the bottom of the bead.
High temperature/humidity can cause the sealant to develop
bubbles during the curing process.
Sealant installation is not recommended when the dew point
of the substrate is close to ambient temperature or a moisture -
vapor transmission condition is present increasing the potential
for bubbling to form during cure.
Porous substrates such as, but not Limited to, marble, limestone,
and granite might absorb components of the Bostik 915FS
Leading to staining of the substrate. ASP with sufficient aging is
mandatory to assess this potential issue.
The ultimate performance of Bostik 915FS depends on proper
joint design and proper application with joint surfaces properly
prepared (See Installation Protocol). Bostik 915FS is not
recommended for joints with dimensions less than or greaterthan
what is recommended below. (See Installation Protocol — Joint
Design section.)
Bostik 915FS must not be used to seal, narrow joints, fillet joints
and face nait holes,
Smearing and feathering Bostik 915FS over joints is not
recommended.
Bostik 915FS is not recommended for horizontal joints or traffic -
bearing joints where abrasion resistance is required (walkways,
driveways, runways, etc.). Please refer to Bostik 955-SL, Bostik
555-SL or Bostik 505 for this application.
Bostik 91SFS11 is not recommended for continuous immersion
in water or any other fluid. When fully cured, avoid exposure,
even incidental, to fuels, chlorinated solvents, acid and alkaline
solutions. Bostik 915FS is not recommended for exterior or interior
seating betowthe waterline; please refer to Bostik 940 Fast Set for
marine applications.
Contact of Bostik 915FS with asphalts (i.e., back coating of window
flashing, etc.) and other fitter compounds impregnated with oil,
asphalt, tar, etc., may deteriorate the cohesive strength of the
substrate and ultimately compromise the seat. Please refer to
Bostik PRO -MS 50 for this application.
During the curing of Bostik 915FS, do not expose to curing silicone
sealants, curing Bostik 2000, Bostik PRO -MS 50, Bostik 2020,
alcohol, acids or solvent -based materials.
Compatibility to copper -based substrates (i.e., flashing) can
vary due to, but not Limited to, age and joint size. Please consult.

Lower relative humidity and temperature will significantly
extend the curingtime. Confined areas, deepjoints and moisture
barrier substrates may also affect the full cure time and extend
it by many days. Apply sealant in ambient airtemperature of 401
F. and rising.
Bostik 915FS may remain tacky for a few hours and attract dust
and dirt from the jobsite which may affect the appearance of
the sealant. Check tack -free time to prevent dirt pickup.
Bostik 915FS is not recommended for glazing applications.
Bond tine strength can be affected by UV rays through the clear
material (glass, acrylic glass, potycarbonate, etc.).
Bostik 915F5 is not an RTV silicone and therefore is suitable
for painting with Latex based paints. Paint chemistries and
flexibility characteristics of the paint films overthe sealant may
affect wetting, adhesion and integrity of the paint layer; and
it is therefore mandatory to pretest the paint or other coating
over the Bostik 915FS to ensure the successful compatibility
between the sealant and the paint/coating after a sufficient
amount of time. See your paint manufacturer for specifications
or Limitations and cati. our Technical Service (1-800-726-
7845) for more information. In general, oil -based paints are
not recommended because of their poor elastic properties
and because of their potential interaction with the sealant
chemistry, which may create non -curing conditions for the
sealant. Do not paint over the polyurethane sealant until it has
futtycured.
The surface of a Bostik 915FS seat when exposed to UV rays
and sunlight will yellow and will not retain its gloss. This
phenomenon can occur within a few weeks after exposure.
The change of color is limited to the surface layer of the sea[
and should not compromise the seating properties ofthe Bostik
915FS if the dimensions of the joint are proper and the sealant
is otherwise property applied. In areas where color retention
is critical, please refer to Bostik 2000, Bostik 2020 and Bostik
PRO -MS 50. Bostik 915FS may remain tacky for a few hours
and attract dust and dirt from the jobsite which may affect the
appearance of the sealant. Check tack -free time to prevent dirt
pickup.
