To whom it may concern;
The above listed Jacobsen manufactured home is constructed with a structural attachment beam, or
"host beam". The design of this host beam allows an attachment to be made directly to the included
structural member in lieu of the typically required "fourth wall construction". Please refer to the
serialized floor plan provided by Jacobsen Homes to verify the location of the host beam,
This manufactured home is required to be set-up or installed, on -site, in accordance with the Jacobsen
Homes "Set -Up Manual" and its "Addendum". Site requirements must be followed to attach a structure to
the host beam provided. Maximum sizes allowed for any attached structure along with the
requirerrients for perimeter blocking along the entire sidewall area where the host beam is installed, are
included in these manuals.
All site installed structures shall be designed by a Professional Engineer (PE) registered in the State of
Florida or as allowed ky the Local Authorit y Having Jurisdiction (LAH]). The design shall comply with all

Limitations and Condition of Use
1, Code Compliance
a. The product assembly described herein has demonstrated compliance with the Florida
Building Cede 71 Edition (I BC), Section 1709.2.
2. Large and small missile impact resistance has Nof been tested to or evaluated for in this
approval. In HVE-1 , this product shall be used in structures "not to be considered living areas"
per Section laid unless impact resistance in accordance to the HVHZ requirements are met.
3. Each product listed above shall be installed in strict compliance with its respective Product
Evaluation Document and site -specific engineering along with all components noted herein.
4. Use of each product shall be in strict accordance with its Product Approval Evaluation and
Limitations of Use.
5. Composite panels shall be constructed using type 3003-H154 aluminum facings, l or 2 PCF
ASTM C-578,Dyplast Products I..LC brand FIBS town insulation (NOA No. 10-1129. 5) adhere
to aluminum facings with Ashland Chemical 2020D ISO grip. Fabrication to be by Elite panel
products only in accordance with approved fabrication methods.
6. Elite roof panels maintain a UL 1715 (int) class V (ext) rating and are ER- 01 approved,
7, This specification has been designed and shall be fabricated in accordance with the requirements
of the F'C, composite panels comply with Chapter 7 Section 720, Chapter 8 Section 803, Class
A interior finish, and Chapter 26 Section 2603. All local building code amendments shall be
adhered to as required.

Midge, Vision Pro, Shadow Ridge, Forest Ridge, Castle Ridge, Somerset Beaded, GP
Park Sap , Compass, Truewall Series 3800, Truewall Series 4000, Trucwall' ` Series

conformance with ASTM D 3679-17 as required Florida da ildin ode 2020,

rational analyses to be in conformance with Florida Building Code 2020, paragraph
1404.8 and Section 2605. For configurations, thicknesses, nail hem types, horizontal and
vertical nail spacings, plus allowable design wind pressures, see table on page 3 of this
report.
Structural testing to ASTM D 5206 was done at Architectural Testing, Inca's main
labomtory located at 130 Deny Court, York, PA 17402 and is reported in 01-38984.01
(test date-3/15/01), 01-3956101 (test date-6/27/01), 99352.04-109-44 (test data
03/26/10), and 99352.05-109-44 (test date03/25/10). Additional fire testing at
Southwest Research Institute located at 6220 Culebra Road, San Antonio, Texas as
reported in projects 01-2303.135 and 01-2303.136, in confonnance with FBC 2612.2, has
determined that, these products have passed the-ASTM D 1929 Self Ignition test and
attained a class C-2 rating in the ASTM D 635 Rate of Bum test.

of these products have been detennined in a separate 8 page structural analyses titled
VINYL, PLASTIC & STEEL SIDING ANALYSES FLORIDA EVALUATIONS & 2020
FBC UPD,4TE. The Pressure Equalization factor was got used in the analyses of steel
siding products. In order to achieve the pressures in the analyses, all studs must have
nails in thera and all nails must be in studs, the exception of the vertical steel siding.
Which requires the nails to only go through the sheathing or plywood, Studs must be at
