licensed by the State of Florida.
If the fill material is to be used in Flood Zone "A' in connection with a permitted building using stem wall
construction, I certify that fill will be used only to fill the area within the stem wall,
If fill material is to be used in any area, I certify that use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversely affect adjacent properties, the owner may be cited for violating
the conditions of the building permit issued under the attached permit application, for lots less than one (1)
acre which are elevated by fill, an engineered drainage plan is required.
If I am the AGENT FOR THE OWNER, I promise in good faith to inform the owner of the permitting conditions set forth in this affidavit poor to
commencing construction, I understand that a separate permit may be required for electrical work, plumbing, signs, wells, pools, air conditioning, gas, or

period of six (6) months after the time the work is commenced. An extension may be requested, in writing, from the Building Official for a period not to
exceed ninety (90) days and will demonstrate justifiable cause for the extension. If work ceases for ninety (90) consecutive days, the job is considered
abandoned,

This letter is regarding the installation of a new roof mounted Solar PV System on the existing)
residential Stl'Ucture at the address above. I have reviewed the attachment plan and have
deternnined that the roof mounted PV system is in compliance with the applicable secdons of the
following Codes as amended and adopted by the jurisdictiom
2020 Florida Building Code 701 Edition, FBC
ASCE 7 Min, Design Loads for Buildings & Other StFUCtUres
Per 2020 FBC, the Roof Mounted IN system will be subject to the following design criteria:
Design Wind Speed (V,,It) - 140mph 3sec gUSt, Exposure Category - B
The PV System consist of the modules, railing, and connection hardware. The system will add a
dead load of approximately 3 psf to the roof.
The existing roof covering is 24ga. Metal panel with min. 1/2" plywood decking and 2" x 4" roof
trusses 24" O.C. The roofing, decking, and roof trusses are in good condition, The existing structure
will he adequate for supporting the additional PV dead load and wind loads.
The securement method of the PV systern is to be flush mounted to the metal panel roof with the
Ironvidge railing and the Rool"rech Mini ji,'rhe attachments can be attached up to 48" apart in roof
zones 1, 2e, 2n, 2r, & 3e and 24" apart in roof zone 3r. The mounts should be staggered, where
possible, to allow distribution of the design loads evenly to the structure. The mounts shall be
installed using 2 x MS x 60mm stainless steel Lag bolts,

Re -Amortization
With your loan, you have two options for how you use to benefit of this tax credit;
1) You can invest the credit into your loan, which will keep your monthly payments low and help deliver monthly savings more quickly, or
2) You can keep that credit yourself. Regardless of which option you choose, your loam will re -amortize by the 19th month.
If you choose not to make the Incentive Investment or any other pre -payments, the amount of your monthly payments will increase

increase in month 19 and then stay the same through the life of the loan,
