- If the fill material is to be used in Flood Zone "A°, it is understood that a drainage plan addressing a
"compensating volume" will be submitted at time of permitting which is prepared by a professional engineer
licensed by the State of Florida.
If the fill material is to be used in Flood Zone "A" in connection with a permitted building using stem wall
construction, I certify that fill will be used only to fill the area within the stem wail,
If fill material is to be used in any area, I certify that use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversely affect adjacent properties, the owner may be cited for violating
the conditions of the building permit issued tender the attached permit application,. for lots less than one (1)
acre which are elevated by fill, an engineered drainage plan is required.
If I am the AGENT FOR THE OWNER, I promise in good faith to Inform the owner of the permitting conditions set forth in this affidavit prior to
commencing construction. I understand that a separate permit may be required for electrical work, plumbing, signs; welts, pools, air conditioning, gas, or
other installations not specifically included in the application. A pormit issued shall be construed to be a license to proceed with the work and not as
authority to violate, cancel, alter, or set aside any provisions of the technical codes, nor shall issuance of a permit prevent the Building Official from
thereafter requiring a correction of errors in plans, construction or violations of any codes. Every permit issued shelf become Invalid unless the work
authorized by such permit is commenced within six months of permit Issuance, or if work author: re€i by the permit is suspended or abandoned for a
period of six (6) months after the time the work is commenced. An extension may be requested, In writing, from the Building Official for a period not to
exceed ninety (90) days and will demonstrate justifiable cause for the extension, if work ceases for ninety (90) consecutive days, the job Is considered
abandoned.

This Work Agreement to Master Trade Partner Agreement (Land) (Owner -Contractor) (Base Agreement)
(the "Work Agreement") is entered into this _ day of 20 _, by and between Lennar
jjo.mes,LLC3("0wner") and Waterline Constrution Inc., a Corporation, ("Contractor") whose address is
4408 N, Grady Ave,, Tampa, FL 33614 telephone number (813) 806-1977.
A. Owner and Contractor entered into that certain Master Trade Partner Agreement (Land) (Owner -
Contractor) Base Agreement on or about 11/20/2020 Contract No. TPUFLL1120WAC (the
"Agreement"). As used herein, and except as hereby expressly provided, all capitalized words and
phrases shall have the same meanings as defined in the Agreement,
BOwner desires to have Contractor perform work within the Project referenced above (the "Project")
as set forth in Contractor's bid package for the Project, which was submitted to Owner via Owner's
electronic bid system, and is hereby approved by Owner and incorporated herein by reference (the
"Work").
C, Owner and Contractor desire to amend the Agreement as more particularly set forth herein,
TERMS AND CONDITIONS
NOW, THEREFORE, in consideration of the foregoing recitals, and the covenants and conditions contained
herein, and for other good and valuable consideration, the receipt of which is hereby acknowledged, Owner
and Contractor hereby agree to amend the Agreement as follows:

1 Pricing as approved for construction of the Work, as set forth in the Scope of Work set forth in
Exhibit "A" hereto, is set forth in the "Pricing Schedule" attached hereto as Exhibit "B".

1 The list of Subcontractors shall be as set forth in Exhibit "D" hereto.
4, The Plans and Specifications for the Work shall be as set forth in Exhibit "E" hereto.

6. Work approved for construction is set forth in Contractor's bid package for the Project described
above,
7. Prior to commencing the Work, Contractor shall comply with all insurance requirements set forth in
Schedule 'A" to the Agreement which requirements are incorporated herein by this reference as
though set forth herein, including, without limitation and if applicable, the "OCIP Addendum" if
attached thereto, If the "Insurance Requirements" are added to or amended by this Work
Agreement, they are set forth in the"Insurance Requirements" attached thereto as Exhibit"G",
8. The Pricing Schedule attached hereto shall be effective on the date set forth above, Any changes
following such price guaranty period, and any changes to the Work, or the Pricing Schedule, shall
become effective only upon execution by all parties hereto of an amendment to the Work
Agreement (the "Amendment"), Contractor Rates and Rental Rates are as set forth in Exhibit "H"
hereto,
9, Owner may, from time to time, issue written notices to proceed/purchase order requests identifying
specific locations on which the Work is to be performed ("Scheduling Notice", "Received Order", or
"Notice to Proceed"), Owner shall have no obligation to issue any Scheduling Notice(s) during the
term of this Work Agreement, THIS WORK AGREEMENT IS NOT AN AUTHORIZATION TO
PROCEED WITH WORK, AND SHALL NOT BECOME EFFECTIVE WITH RESPECT TO THE
WORK OF CONTRACTOR UNLESS AND UNTIL OWNER ISSUES SCHEDULING NOTICE(S)
TO CONTRACTOR AUTHORIZING THE SPECIFIC WORK OF CONTRACTOR TO BE
PERFORMED, AND THIS WORK AGREEMENT SHALL BE BINDING ONLY AS TO THE WORK
SO AUTHORIZED BY OWNER. CONTRACTOR SHALL PERFORM NO WORK WITHOUT
RECEIVING OWNER'S WRITTEN SCHEDULING NOTICE(S) FOR SUCH WORK.
CONTRACTOR AGREES TO COMMENCE SUCH WORK AS MAY BE AUTHORIZED BY
OWNER BY MEANS OF EACH SCHEDULING NOTICE(S), AND SUCH SCHEDULING
NOTICE(S) SHALL BE EFFECTIVE IMMEDIATELY UPON ISSUANCE BY OWNER WITHOUT
NEED FOR FURTHER ACCEPTANCE THEREOF BY CONTRACTOR, If the Work authorized by
a Scheduling Notice is not commenced on the date specified in the Scheduling Notice, Owner may,
in its sole discretion, declare such Scheduling Notice null and void. Owner may, but shall not be
obligated to, furnish Contractor with a progress schedule for all or any portion of the Work which, if
furnished, may be amended from time to time by Owner and shall be considered a part of this Work
Agreement, Contractor acknowledges that neither this Work Agreement nor the issuance by Owner
of a progress schedule constitute any representation by Owner that a minimum or specified number
of Scheduling Notices will be issued. As to that portion of the Work covered by a Scheduling Notice,
such schedule shall control over any other provision of this Work Agreement or progress schedule
regarding time for performance,
Except to the extent the Agreement is supplemented by this Work Agreement or such other Work
Agreements that may have been entered into by the parties hereto, the terms and conditions of the
Agreement shall remain unmodified and in full force and effect. In the event of conflict between the terms

JOB RULES
1. Notwithstanding any contrary language in any agreement between Contractor and any third person,
including any labor organization, neither Contractor nor any Subcontractor shall allow any person access to
the Project/Job Site unless they are directly and actively engaged in the performance of the Work. No
unauthorized persons (e.g,, members of the general public, off -duty employees, family members, solicitors,
visitors, vendors, sales persons, officials of labor organizations, prospective homeowners, etc,) shall be
allowed on the Project or Job Site without Owner's written consent. Access by any unauthorized person shall
be considered unlawful trespass and a material breach of this Agreement.
I Hard-hats, eye and hearing protection, proper scaffolding, equipment safety devices, and all other
appropriate clothing and safety devices are required at all times and in all locations at the Project or Job Site
for Contractor and any Subcontractor, If Contractor observes any other contractor violating this requirement,
Contractor shall immediately report such violation to Owner,
3, CONSUMPTION OF ALCOHOL OR ILLEGAL SUBSTANCES EITHER BEFORE OR DURING
PERFORMANCE OF THE WORK IS CAUSE FOR IMMEDIATE SUSPENSION OF WORK AND
TERMINATION OF THIS AGREEMENT. Contractor shall not allow any person acting by, through or under
Contractor to be on the construction site or perform any part of the Work (a) while under the influence of
alcohol or any drugs or controlled substances, or (b) if there is (i) any suspicion of alcohol or drug use,
possession, or impairment involving such employees, agents, or Subcontractors, or hi) an incident where
drug or alcohol use could have been a contributing factor. Owner may require Contractor to remove any
person that Owner reasonably suspects is in any way impaired and, in such case, such person may only
be considered for return to work after Contractor certifies as a result of a for -cause test, conducted
immediately following removal, that person was in compliance with this Agreement, Contractor shall not
allow on the Job Site any person that either refuses to take, or tests positive in, any alcohol or drug test.
Owner may, without prior notice, search or require Contractor to search the person, possessions, and
vehicles of any person acting by, through or under Contractor that are on the Job Site or any premises
owned, controlled, or managed by Owner. Any person who refuses to cooperate with such search will be
immediately removed from the premises and not allowed to return. Contractor will comply with all applicable
federal, state, and local drug and alcohol related laws and regulations (e,g,, Department of Transportation
regulations, Department of Defense Drug -Free Workforce Policy, Drug -Free Workplace Act of 1988),
Owner may perform unannounced audits of Contractor's alcohol and drug program to verify that
Contractor's policy and its enforcement are acceptable to Owner. In addition, possession of alcohol, drugs,
illicit or unprescribed controlled substances, firearms, explosives, weapons, and hazardous substances or
articles is not permitted on the Job Site or other premises owned, controlled, or managed by Owner. Owner
does not warrant or represent that Contractor's policy meets the requirements of any applicable law, code,
rule or regulation, nor does Owner warrant that the proper enforcement of Contractor's policy will ensure
that no accidents or injuries will occur. In addition, any action taken or not taken by Owner under this
Paragraph shall not diminish or affect any of Contractor's obligations under applicable law or this
Agreement, or Contractor's status as an independent Contractor.
4. Contractor and its employees, agents, and Subcontractors shall not speed or otherwise operate their
automobiles in a dangerous or offensive manner within the Job Site, and shall otherwise comply with all traffic
laws, ordinances, rules or regulations. The speed limit within the Job Site is 5 MPR
5. Contractor will provide portable toilet facilities for the use of Contractor's employees.
6. Owner shall have the right to control or restrict parking of all vehicles and equipment on the
Project/Job Site, and Contractor shall comply with any such parking restrictions.

7, Contractor shall not interfere with the free vehicular and pedestrian access through any public or
private streets and driveways on the Project/Job Site or located <in the vicinity of the Project/Job Site,

from an officer of Owner at the office address listed in this Agreement, Non -approved work will be removed
or replaced at Contractor's expense, Continued violation of this rule may result in the termination of this

9. Neither Contractor nor any Subcontractor shall, without specific prior written authorization of
Owner's Regional President, give or receive any commission, fee, rebate, gift, loan or anything else of value
to (a) any person or entity, including without limitation any agent or employee of a governmental authority,
in connection with or as a result of Contractor's Work provided hereunder, to influence any decision or gain
any other advantage for Owner or Contractor, or (b) any agent or employee of Owner, Neither Contractor
nor any Subcontractor shall, without prior written authorization of Owner's Regional President, enter into
any business arrangement with any agent or employee of Owner, or any affiliate of any agent or employee
of Owner. No agent or employee of Owner is authorized to engage in any employment or enter into any
contract or agreement that conflicts with Contractor's obligations under this Agreement. Contractor shall
immediately notify the Owner Legal Department in Miami, Florida, if any agent or employee of Owner
requests or suggests that Contractor provide anything of value in violation of this Paragraph, or should
Contractor believe or have reason to believe that an agent or employee of Owner expects to receive, directly
or indirectly, anything of value,

A general permit is granted to any person to construct, extend, or remove a dock or pier and associated
structures as described below:
(a) A private, single-family pier or dock with up to two boat lifts that, together with all existing structures on the
shoreline of the property, does not exceed a total area of 2,000 square feet over surface waters. Such a
structure:
1. Shall not accommodate the mooring,of more than two vessels, either io, the water or orl a boat; Of , Solety, for
purposes of this general permit, up to two personal' watercraft as defined in section 327,02(33), F.S., may be
moored in lieu of either or both allowable vessels of another type. These limits shall not apply to the mooring,
storage or other use of the dock or pier by:
a. N on- motor- powered vessels less than 16 feet in length that are stored on or under the dock or pier, or within
an authorized mooring area; or
b. Personal watercraft, dinghies or similar small vessels that are stowed out of the water, upon a larger parent
vessel that is moored at the dock in compliance with this general permit.

subject to the applicable provisions of chapters 253 and 258, F.S., and the rules adopted thereunder. Portions of
such roofs that overhang beyond the edge of decked portions of the pier or dock shall be included in the
calculation of the total square footage of over -water structure allowed under paragraph (1)(a), above.
(b) A public fishing pier that does not-. exceed a totaterea af,,2,000 square feet provWedthe structure is designed
and built to discourage boat mooring by elevating the fishing pier to a minimum height of five feet above mean
high water or ordinary high water, surrounding the pier with handrails, and installing and maintaining signs that
state "No Boat Mooring Allowed,"
2. This general permit shall be subject to the following specific conditions:
(a) Construction or extension of the boat lift, boat mooring locations, or terminal platform, shall not occur over
submerged grassbeds, coral communities or wetlands. However, the access walkway portion of the pier may
traverse these resources provided it is elevatede minimum of five feet abovemean,hjO water orordinary bigh
wafter, carftns,`hAndriflsAhdt aremd6tainedin such a manner as to prevent use of Vie access watkways for bri
mooring or access, and does not exceed a width of six feet, or a width of four feet in Aquatic Preserves;
(b) There shall be no structures enclosed by walls, screens, or doors on any side;
(c) The dock or pier will not facilitate vessel rentals, charters, or serve any other commercial purpose;
(d) There shall be no fish cleaning facilities, boat repair facilities or equipment, or fueling facilities on the
structures authorized by this general permit. In addition, no overboard discharges of trash, human or animal
waste, or fuel shall occur from any structures authorized by this general permit;
(e) This general permit shall not allthorize the construction or extensionof more than one dock or pier per parcel
of land or IndivduaV lot. For the purposes of, this generat, permit, awltWamity Rving, complexes s4AW be treated as
one parcel of property regardless of the legal division of ownership or control of the associated property; and
(f) Notwithstanding any other provisions of this general permit, the design, construction and operation of the doc
or pier and associated vessels shall not conflict with any manatee protection plan approved and adopted under
section 379.2431(2)(t), F,S.

GENERAL CONDITIONS:
The following general permit conditions are binding upon the permittee and are enforceable under Part IV of Chapter 373,
F.S. These conditions do not apply to the general permit for stormwater management systems under Section
401814(12), F,S,
1 The general permit is valid only for the specific activity indicated. Any deviation from the specified activity and the
conditions for undertaking that activity shall constitute a violation of the permit and may subject the permittee to
enforcement action and revocation of the permit under Chapter 373, F.S.
2, The general permit does not eliminate the necessity to obtain any required federal, state, local and special
district authorizations prior to the start of any construction, alteration, operation, maintenance, removal or
abandonment authorized by this permit; and it does not authorize any violation of any other applicable federal, state,
Local, or special district laws (including,, but not limited to those governing the 'lake" of listed species).
3. The general permit does not convey to the permittee or create in the permittee any property right, or any interest in
real property, nor does it authorize any entrance upon or activities on property which is not owned or controlled by
the permittee, or convey any rights or privileges other than those specified in the general permit,
4. The general permit does not relieve the permittee from liability and penalties when the permitted activity causes
harm or injury to: human health or welfare; animal, plant or aquatic life; or property. It does not allow the permittee to
cause pollution that violates state water quality standards.
& The authorization to conduct activities under a general permit may be modified, suspended or revoked in
accordance with Chapter 120, F.S., and Section 373,429, F.S.
7The general permit is not transferable to a new third party. To be used by a different permittee, a new notice to use
a general permit must be submitted in accordance with rule 62-330.402, F,A.C. Activities constructed in accordance
with the terms and conditions of a general permit are automatically authorized to be operated and maintained by the
permittee and subsequent owners in accordance with subsection 62-330.340(l), F.A.C. Any person holding the
general permit, persons working under this general permit, and owners of land while work is conducted under the
general permit shall remain liable for any corrective actions that may be required as a result of any permit violations
prior to sale, conveyance, or other transfer of ownership or control of the permitted project, activity, or the real
,property at which the permitted project or activity is located.
1 Upon reasonable notice to the permittee, Agency staff with proper identification shall have permission to enter,
inspect, sample and test the permitted system to ensure conformity with the plans and specifications approved by
the general permit.
9The permittee shall maintain any permitted project or activity in accordance with the plans submitted to the Agency
and authorized in the general permit.
10, A permittee's right to conduct a specific activity under the general; permit is authodzed for a duration of five years.
11Activities shall be conducted in a manner that does not cause or contribute to violations of state water quality
standards. Performance -based erosion and sediment control best management practices shall be implemented and
maintained immediately prior to, during, and after construction as needed to stabilize all disturbed areas, including
other measures specified in the permit to prevent adverse impacts to the water resources and adjacent lands.
Erosion and sediment control measures shall be installed and maintained in accordance with the State of Florida

b. The maximum width of the construction access area shall be limited to 15 feet;
C. All mats shall be removed as soon as practicable after equipment has completed passage through, or work has
been completed, at any location along the alignment of the project, but in no case longer than seven days after
equipment has completed work or passage through that location; and
d. Areas disturbed for access shall be restored to natural grades immediately after the maintenance or repair is
completed.
14. The construction, alteration, or use of the authorized project shall not adversely impede navigation or create a
navigational hazard in the water body.

a. All vessels associated with the project shall operate at "Idle Speed/No Wake" at all times while in the work area
and where the draft of the vessels provides less than a four -foot clearance from the bottom. All vessels will
follow routes of deep water whenever possible.
b. All deployed siltation or turbidity barriers shall be properly secured, monitored, and maintained to prevent
entanglement or entrapment of fisted species.
C. All in -water activities, including vessel operation, must be shut down if a listed species comes within 50 feet of
the work area. Activities shall not resume until the animal(s) has moved beyond a 50-foot radius of the in -water
work, or until 30 minutes elapses since the last sighting within 50 feet, Animals must not be herded away or
harassed into leaving. All on -site project personnel are responsible for observing water -related activities for the
presence of listed species.
d. Any listed species that is killed or injured by work associated with activities performed shall be reported
immediately to the Florida Fish and Wildlife Conservation Commission (FWC) Hotline at 1(888)404-3922. and
ImperiledSpecies@myFWC.com.
e. Whenever there is a spill or frac-out of drilling fluid into waters accessible to the above species during a
directional drilling operation, the FWC shall be notified at lmperiledSpecies@myfwc.com with details of the event
within 24 hours following detection of the spill or frac-out,
