NOTICE OF DEED RESTRICTIONS: The undersigned understands that this permit may be subject to `deed" restrictions" which may be more restrictive
than County regulations, The undersigned assumes responsibility for compliance With any applicable deed restrictions.
UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES: If the owner has hired a contractor or contractors to undertake work, they
may be required to be licensed in accordance with state and local regulations, If the contractor is not licensed as required by law, both the owner and
contractor may be cited for a misdemeanor violation under state law. If the owner or intended contractor are uncertain as to what licensing requirements
may apply for the intended work, they are advised to Genteel the Pasco County Building Inspection Division --Licensing Section at 727-847- 8009.
Furthermore, K the owner has hired a contractor or contractors, he is advised to have the contractor(s) sign portions of the "contractor Block' of this
application for which they will be responsible. If you, as the owner sign as the contractor, that may be an indication that he is not property licensed and is
not entitled to permitting privileges in Pasco County.
TRANSPORTATION IMPACT/UTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands
that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings, change of use in existing buildings, or
expansion of existing buildings, as specified in Pasco County Ordinance number 89-07 and 90-07, as amended. The undersigned also understands, that
such fees, as may be due, will be identified at the time of permitting. It is further understood that Transportation Impact Fees and Resource Recovery
Fees must be paid prior to receiving a 'certificate of occupancy" or final power release. If the project does not involve a certificate of occupancy or final
power release, the fees must be paid prior to permit issuance. Furthermore, if Pasco County Water/Sewer Impact fees are due, they must be paid prior
to permit issuance in accordance with applicable Pasco County ordinances.
CONSTRUCTION LIEN LAW (Chapter 713, Florida Statutes, as amended): If valuation of work is $2,500.00 or more, I certify that 1, the applicant,
have been provided with a copy of the "Florida Construction Lien Law —Homeowner's Protection Guide' prepared by the Florida Department of
Agriculture and Consumer Affairs, If the applicant is someone other than the "owner", I certify that I have obtained a copy of the above described
document and promise in good faith to deliver it to the "owner" prior to commencement.
CONTRACTOR'S/OWNER'S AFFIDAVIT: I certify that all the information in this application is accurate and that all work will be done in compliance with
all applicable laws regulating construction, zoning and land development. Application is hereby made to obtain a permit to do work and installation as
Indicated. I certify that no work or installation has commenced prior to issuance of a permit and that all work will be performed to meet standards of all
laws regulating construction, County and City codes, zoning regulations, and land development regulations in the jurisdiction. I also certify that I
understand that the regulations of other government agencies may apply to the intended work, and that it is my responsibility to identify what actions I
must take to be In compliance. Such agencies include but are not limited to:
- Department of Environmental Protection -Cypress Bayheads, Welland Areas and Environmentally Sensitive
Lands, WaterNVastewater Treatment.
- Southwest Florida Water Management District -Wells, Cypress Bayheads, Wetland Areas, Altering
Watercourses.
- Army Corps of Engineers -Seawalls, Docks, Navigable Waterways.
- Department of Health & Rehabilitative Services/Environmental Health Unit -Wells, Wastewater Treatment,
Septic Tanks.
- US Environmental Protection Agency -Asbestos abatement.
- Federal Aviation Authority -Runways.
I understand that the following restrictions apply to the use of fill:
- Use of fill is not allowed in Flood Zone W" unless expressly permitted.
- If the fill material is to be used in Flood Zone "A", it is understood that a drainage plan addressing a
"compensating volume" will be submitted at time of permitting which is prepared by a professional engineer
licensed by the State of Florida,
- If the fill material is to be used in Flood Zone "A" in connection with a permitted building using stem wall
construction, I certify that fill Will be used only to fill the area within the stem wall.
- If fill material is to be used in any area, I certify that use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversely affect adjacent properties, the owner may be cited for violating
the conditions of the building permit issued under the attached permit application, for lots less than one (1)
acre which are elevated by fill, an engineered drainage plan is required.
If I am the AGENT FOR THE OWNER, I promise in good faith to inform the owner of the permitting conditions set forth in this affidavit prior to
commencing construction. I understand that a separate permit may be required for electrical work, plumbing, signs, wells, pools, air conditioning, gas, or
other installations not specifically included in the application. A permit issued shall be construed to be a license to proceed with the work and not as
authority to violate, cancel, alter, or set aside any provisions of the technical codes, nor shall issuance of a permit prevent the Building Official from
thereafter requiring a correction of errors in plans, construction or violations of any codes. Every permit issued shall become invalid unless the work
authorized by such permit is commenced within six months of permit issuance, or if work authorized by the permit is suspended or abandoned for a
period of six (6) months after the time the work is commenced. An extension may be requested, in writing, from the Building Official for a period not to
exceed ninety (90) days and will demonstrate justifiable cause for the extension. If work ceases for ninety (90) consecutive days, the job is considered
abandoned.

1. This contract is valid for fifteen (15) days. Signing this contract, and or making a deposit, initiates this contract
and illustrates the acknowledgement of these terms and conditions by the client. Any modification or alteration
desired from the above specifications must be in writin , and may accrue additional costs; no oral agreements or
statements are legally binding.
2. CASH DEALS ONLY- Upon the effective date, the client shall pay to SOLARWISE ENERGY SOLUTIONS, a
$2,000 deposit. No work will commence until the $2,000 deposit is received, The second payment, due at permit
submittal, is equal to 50% of the total contract price. The third payment, due after the solar system passes
inspection, is the remaining balance of the total project cost. Utility approval is coordinated after final payment is
3. Installations must be completed in accordance with all laws, codes and other requirements applicable under
federal, state and local authority (e.g. permits, inspections).
4. SOLARW|SE ENERGY SOLUTIONS will require the necessary client information to proceed with mandatory
government and State applications.
5. SOLARWISE ENERGY SOLUTIONS and its staff will require access to your property to proceed with initial
project assessment, construction, inspection and required maintenance. The client will be contacted prior to
SOLARWISE ENERGY SOLUTIONS visits, and will need to arrange property access allowing SOLARWISE ENERGY
SOLUTIONS to fulfill its portion of the agreement. SOLARWISE ENERGY SOLUTIONS will require all necessary
forms filled and full client cooperation to complete its part of the contract.
6. We recommend the o|kant to seek the appropriate consultation concerning any tax liabilities that may be
associated with the receipt of the rebate.
7. SOLARVV|SE ENERGY SOLUTIONS, its representatives, parents, subsidiaries, affiliates, officers, employees,
contractors and agents:
- Are not responsible for any tax liabilities that eclient may incur.
- Assume no responsibility for the performance of the equipment or equipment warranty,
- Do not endorse, guarantee or warrant any particular brand, manufacturer or third party services.
8. SOLARWISE ENERGY SOLUTIONS guarantees the first year of solar production. If solar production falls short of
estimated annual production, following a full year (365 days) of system activation. SOLARWISE ENERGY
SOLUTIONS will compensate the kwh cost difference at the current retail utility rates. No savings, energy
production or income generation are guaranteed after the first year as a result of the purchase and installation of
eligible equipment by SOLARWISE ENERGY SOLUTIONS. Refer to the solar manufacturer's websites for further
information on the 25-30 year linear power warranties specifically, the degradation schedule.
9. The client is responsible for the application prooeam, from initiation through completion unless mentioned
otherwise by representative ofS{]LARVV|SE ENERGY SOLUTIONS. The client may designate a primary contact;
however, the client is responsible for ensuring that the guidelines, terms and conditions, requirements, and
program processes are adhered to.

10. SOLARWISE ENERGY SOLUTIONS warranties workmanship for a period of 10 years following the date of project
completion. A workmanship warranty protects the client from labor -related defects and constitutes the attachment,
fastening, connecting and condition of electrical devices that are part of the original installed system. During the
workmanship warranty period, SOLARWISE ENERGY SOLUTIONS will repair or replace defective parts with new parts
or, at the option of SOLARWISE ENERGY SOLUTIONS, serviceable used parts that are equivalent or superior to new
parts in performance. This workmanship warranty extends only to products purchased from SOLARWISE ENERGY
(a) as a result of accident, misuse or abuse (b) as a result of an act of god (c) by operation outside the usage
by modification of the product (f) a result of war or terrorist attack (9) as a result of service by anyone other than a
SOLARWISE ENERGY SOLUTIONS representative (h) or any vandalism,
11. SOLARWISE ENERGY SOLUTIONS warranties that there will be no intrusion from natural water into any installed
penetrations for 25 years following the completion of the installation. SOLARWISE ENERGY SOLUTIONS will be
responsible for repairing the intrusion areas and damages caused, by means of waterproofing damage repair or
replacement. The client agrees that the age of the existing roofing should be recognized before installation and that
SOLARWISE ENERGY SOLUTIONS is not responsible for existing conditions. Detection of any leaking areas must be
reported within twenty-four (24) hours to SOLARWISE ENERGY SOLUTIONS. Any areas affected by the intrusion will be
assessed by SOLARWISE ENERGY SOLUTIONS staff only. No warranty or liability will be assumed for leaks and/or
damage to the roof and/or substructure and/or interior building materials and/or contents caused by roof penetrations
in, attachments to, or walking on roofs deemed to be in poor condition prior to or during the installation — the client will
be notified of this condition upon discovery. Work performed on roofs deemed to be in good condition will be included
in SOLARWISE ENERGY SOLUTIONS standard warranty.
12. Tile roofs have an inherent risk of damage during installation, SOLARWISE ENERGY SOLUTIONS is not responsible

tile on -site. If a spare is not available, SOLARWISE ENERGY SOLUTIONS will only caulk the cracked tile. The client
hereby agrees that SOLARWISE ENERGY SOLUTIONS shall not be held responsible for any damage to the tile roof
during installation.
13. The client hereby agrees to indemnify, defend and hold harmless, SOLARWISE ENERGY SOLUTIONS, its
representatives, parents, subsidiaries, affiliates, officers, employees, contractors and agents from any and all liability
associated with this program/project except for the labor warranties provided� Please refer to the manufacturers spec
sheet and online warranty information for details. Manufacturer is responsible for all solar components warranties,
SOLARWISE ENERGY SOLUTIONS is responsible for all labor warranties unless stipulated otherwise.

14. It is the responsibility of SOLARWISE ENERGY SOLUTIONS to ensure that all appropriate permits (e.g. local,
state) have been obtained including ensuring that Deed Restrictions or Home Owner Association Restrictions do
not prohibit the installation of the proposed photovoltaic system.
15. Federal, State, Local, Utility program guidelines and rebate levels are subject to change without notice, and
may be terminated by executive, legislative, judicial or regulatory action at any time. Utility rates are also subject
to change. SOLARWISE ENERGY SOLUTIONS is not held accountable for loss of rebate, income, savings or delays
resulting from such a situation.
16. Prior to final pricing, SOLARWISE ENERGY SOLUTIONS reserves the right to conduct a site visit/audit to
verify that all satellite calculations were exact. Materials offered are subject to change without notice. Quality and
efficiency offered will remain equivalent or better.
17. All payments must be made according to the scheduled agreement. Personal Check, Wire Transfer, all major
Credit/Debit Cards (only for initial deposit), and Bankers Draft are all acceptable methods of payment. All goods
remain the property of the SOLARWISE ENERGY SOLUTIONS until final payment has been completed. Payments
outstanding beyond their respective due dates will incur interest at the rate of 3% on the outstanding balance,
until the balance is paid in full and final settlement has been reached.
18. SOLARWISE ENERGY SOLUTIONS does not, will not, and cannot hold responsibilities in relation to any form
of damage caused by accumulated snow and/or ice and/or hail on or around the solar array.
19. The timeline proposed in this offer cannot be guaranteed by SOLARWISE ENERGY SOLUTIONS an
SOLARWISE ENERGY SOLUTIONS cannot be held accountable for any delays resulting from situations not under
the control of SOLARWISE ENERGY SOLUTIONS (rain, snow, application approval delays etc.).
