NOTICE OF DEED RESTRICTIONS: The undersigned understands that this permit may be subject to "deed" restrictions' which may be more restrictive
than County regulations. The undersigned assumes responsibility for compliance with any applicable deed restrictions.
UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES: If the owner has hired a contractor or contractors to undertake work, they
may be required to be licensed in accordance with state and local regulations. If the contractor is not licensed as required by law, both the owner and
contractor may be cited for a misdemeanor violation under state law. If the owner or intended contractor are uncertain as to what licensing requirements

TRANSPORTATION IMPACTIUTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands
that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings, change of use in existing buildings, or
expansion of existing buildings, as specified in Pasco County Ordinance number 89-07 and 90-07, as amended. The undersigned also understands, that
such fees, as may be due, will be identified at the time of permitting. It is further understood that Transportation Impact Fees and Resource Recovery
Fees must be paid prior to receiving a "certificate of occupancy' or final power release. If the project does not involve a certificate of occupancy or final
power release, the fees must be paid prior to permit issuance. Furthermore, if Pasco County Water/Sewer Impact fees are due, they must be paid prior
to permit issuance in accordance with applicable Pasco County ordinances.
CONSTRUCTION LIEN LAW (Chapter 713, Florida Statutes, as amended): If valuation of work is $2,500.00 or more, I certify that I, the applicant..
have been provided with a copy of the "Florida Construction Lien Law —Homeowner's Protection Guide" prepared by the Florida Department of
Agriculture and Consumer Affairs. If the applicant is someone other than the "owner", I certify that I have obtained a copy of the above described
document and promise in good faith to deliver it to the "owner" prior to commencement.
CONTRACTOR'S/OWNER'S AFFIDAVIT: I certify that all the information in this application is accurate and that all work will be done in compliance with
all applicable laws regulating construction. zoning and land development. Application is hereby made to obtain a permit to do work and installation as
indicated. I certify that no work or installation has commenced prior to issuance of a permit and that all work will be performed to meet standards of all
laws regulating construction, County and City codes, zoning regulations, and land development regulations in the jurisdiction. I also certify that I
understand that the regulations of other government agencies may apply to the intended work, and that it is my responsibility to identify what actions I
must take to be in compliance. Such agencies include but are not limited to:
Department of Environmental Protection -Cypress Bayheads, Wetland Areas and Environmentally Sensitive
Lands, WaterfWastewater Treatment.

I understand that the following restrictions apply to the use of fill:
Use of fill is not allowed in Flood Zone 'W unless expressly permitted.
If the fill material is to be used in Flood Zone "A", it is understood that a drainage plan addressing a
"compensating volume" will be submitted at time of permitting which is prepared by a professional engineer
licensed by the State of Florida.
If the fill material is to be used in Flood Zone "A" in connection with a permitted building using stem wall
construction, I certify that fill will be used only to fill the area within the stem wall.
If fill material is to be used in any area, I certify that use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversely affect adjacent properties, the owner may be cited for violating
the conditions of the building permit issued under the attached permit application, for lots less than one (1)
acre which are elevated by fill, an engineered drainage plan is required.
If I am the AGENT FOR THE OWNER, I promise in good faith to inform the owner of the permitting conditions set forth in this affidavit prior to
commencing construction. I understand that a separate permit may be required for electrical work, plumbing, signs, wells, pools, air conditioning, gas., or
other installations not specifically included In the application. A permit issued shall be construed to be a license to proceed with the work and not as
authority to violate, cancel, alter. or set aside any provisions of the technical codes, nor shall issuance of a permit prevent the Building Official from
thereafter requiring a correction of errors in plans, construction or violations of any codes. Every permit issued shall become invalid unless the work
authorized by such permit is commenced within six months of permit issuance, or if work authorized by the permit is suspended or abandoned for a
period of six (6) months after the time the work is commenced. An extension may be requested, in writing, from the Building Official for a period not to
exceed ninety (90) days and will demonstrate justifiable cause for the extension. If work ceases for ninety (90) consecutive days, the job is considered
abandoned.

RESIDENTIAL SOLAR EQUIPMENT LEASE AGREEMENT
Terms & Conditions
This Lease and any Exhibits, Addenda, and Amendments
(collectively, this "Lease") is the agreement between
you (the ^Customer" or''you''or^you�)and |G5So|a�
LLC(^|GSSolar" ^|GS^ or "we" o"our") covein the
lease of the solar panel system (the "System") that will
be installed at the address listed on the first page of this
Lease (the "Property"). All obligations under this Lease,
unless otherwise specified, will begin on the Transaction
Date listed on the first page of this Lease.

date the System obtains permission to operate from
your electric utility (the "Interconnection Date"), with
each successive 12-month period of the Term being a
"Contract Year."
i During the Term, you will pay IGS Solar for the use
of the System at the rate set forth in the table on
the first page of this agreement.
ii At the time you signthis Lease, you must provide
IGS Solar with a method of payment, which may
be either by credit card, debit card, or ACH. You
can authorize us to use your method of payment
to pay invoices on a recurring basis by completing
our Payment Authorization Form. If you choose
to schedule automatic payments to be deducted
from your bank account or credit card, you
have the responsibility to ensure that there are
adequate funds or credit limit. In addition, IGS
Solar may charge you a billing fee each month
in the amount of $_I.so , which may be
waived, if you are enrolled in automatic payments,
in IGS Solar's sole discretion. Any billing fee, if
charged, will appear on your monthly invoice.
|G3Solar will send anelectronic invoice each month
to the email address you provided for the Monthly
Lease Payment.
C. AUTOMATIC CHARGES; LATE PAYMENTS
You will pay a returned -check fee, which will not
exceed the maximum allowed by law, for any check or
withdrawal that is returned orrefused byyour bank,
and the lesser of(i)alate payment fee equal tol.5Y6per
month on all past -due amounts, or(ii)the maximum
percentage allowed bylaw.

not limited to sales tax, excise tax, property tax. and
other charges. If taxes applicable to this Lease change,
you will pay the updated charged amounts. If, at the
end of the Term of this Lease, you choose to purchase
the System, you will pay any applicable tax on the
purchase price,
A.IGS SOLAR OBLIGATIONS
|GS Solar will perform the following obligations at its
sole cost and expense:
i |GS Solar will construct orcause the construction
ofthe System.
ii. IGC Solar will provide you with revenue -grade
meter to accurately measure the amount of
electricity the System generates.
iii |GS Solar will insure the System against damage
and loss unless the damage or loss is caused
by your gross negligence, or you intentionally
damage the System.
ix|GS Solar will arrange for repair ofthe System.
x |GSSolar will not putelien onyour Property. |GSSolar
may file informational Uniform Commercial [ode
forms or similar statements with the appropriate
secretary ofstate and/or county recorder's office to
give notice of|GSSo|ar'sownership nfthe System.
B. VGS SOLAR'S LIMITS ON OBLIGATION
TO DELIVER
i |GS Solar is not a utility or public service
company and does not assume any obligations
of a utility or public service company to supply
your energy requirements, Nothing in this Lease
will be construed as a dedication of the System
topublic use. The System will not provide power
to the customer in the event that the Property is
disconnected from the Utility power grid.
iiDuring the Term, you may require more electricity
than the System generates.You will be solely
responsible for maintaining access to obtaining,
and paying for that electricity from your utility.
iii. |G5Solar will not pay for electrical panel upgrades
or correction of structural integrity problems,
|GS Solar is not responsible for upgrades to the
Property before orafter installation ofthe System.
C. CUSTOMER OBLIGATIONS
i You will not allow shade to interfere with the
System's performance, and you will keep shrubs,
trees, and other foliage trimmed.
ii You will not clean or make any modif|cations,
impnzvements, evis|ons, or additions to the
System.
iiiYou will promptly notify kS5Solar ifyou suspect

for communication purposes.

obtain your electrical usage history from your
utility. VVewill treat those bills and usage history
as confidential.
ix, You represent and warrant to |6S Solar that you
are the owner ofthe Property and that you are at
least lOyears ofage.
D. REPAIRS OR IMPROVEMENTS
You must notify |GS Solar 30 days before making
repairs or improvements toyour Property that could
interfere with the System. You will not materially
alter the location where the System is installed. You
will continue to pay all Lease Payments during any
downtime.
E. ACCESS TO THE SYSTEM
i You grant to |GSSolar and its contractors the
right to access all of the Property as reasonably
necessary for:
l. Installing, operating, maintaining, repairing,
removing, and replacing the System and
making any additions to the System and
installing complementary technologies on or
about the location of the System;
2.Using and maintaining electric Unes, invedeo,
and meters necessary to interconnect the
System toyour Property's e|ectricxystemandto
the utility's electric distribution system; and
S.Takingany other action reasonably necessary
in connection with insta||ing, opeeting,
maintaining, repairing, removing, and replacing
the System, and enforcing any remedies of
|G5Solar under this Lease.
ii. Our right to access will continue for 90 days after
the Term expires to provide IGS Solar with time to
remove the atthe end ofthe Lease if|GS
Solar chooses todoso. |G3Solar will have access
rights aLall times and you will not interfere with
or permit any third party to interfere with such
rights or access.
|ii |G5 Solar will attempt to provide you with
reasonable notice of our need to access the

G. CUSTOMER WAIVERS
Your obligation (opay Lease Payments and all other
amounts you owe under this Lease will beabsolute
and will not be subject toany abatement, defense,
counterc|aim, setoff, recoupment, or reduction
for any meson whatsoever. All amounts owed by
you under this Lease will be payable in all events,
including by your heirs and estate. You waive all
rights you may have to revoke acceptance of the
System nrtogrant asecurity interest in the System,
and except asset forth in the following parts ofthis
Lease, you waive all rights to cancel this
Leese: Section 26 of this Lease' the Notice of Right
toCancel onthe Signature Page, and Exhibit 1tothis
Lease.
A. THE FOLLOWING ITEMS MUST BE
COMPLETE PRIOR TO INSTALLATION
i |GSSolar will complete anengineering site audit,
the final System design and real estate due
diligence toconfirm the suitability ufthe Property,
including any required changes orimprovements
that must bedone by you, for the construction,
installation, and operation ofthe System;
ii |GS Solar will obtain all necessary governmental
zoning' land use, and building permits (this

Material changes to this Lease may only be made in a
written amendment tothis Lease signed by you and |GS
Solar. You authorize |GSSolar tomake corrections toany
utility paperwork in order toconform tothis Lease, and
any amendments or addenda.
|GS Solar may assign' seU, or otherwise transfer the
System and this Lease, nrany part, without your consent;
however, your rights will not beaffected.
The System is personal property comprised of removable
equipment and will not be considered a fixture or in any
way a part of your Property. IGS Solar owns the System,
including all data generated from the System, for all
purposes. You will keep the System free and clear of,
and indemnify and defend IGS against, all claims, levies,
liens, and other encumbrances on the System, except for
any created by IGS Solar,
You will own the electricity generated bythe System.
Except for the electricity
Solar will own everything produced by or otherwise
related to the System, including but not limited to all tax
credits, incentives, renewable energy credits, green tags,
carbon offset credits, utility rebates, allowances, benefits,
grants, and environmental attributes (collectively,
"Benefits"), Throughout the Term, IGS Solar will have the
exclusive right to use, trade, and/or sell all such Benefits.
You will notenter into any agreementwith yourutility that
would entitle your utility to claim any Benefits. Subject
to applicable law, you will reasonably cooperate with IGS
Solar so that IGS Solar may claim all Benefits, including
entering into net metering agreements, interconnection
agreements, and filing renewable energy/carbon offset
credit registrations and/or applications for rebates from
the federal, state, and/or local government, or a local
utility, and giving those Benefits to IGS Solar.
As long as you are not in default of this Lease you
may purchasethe System, as is atany time onorafter
the 5-year anniversary of the Interconnection Date. To
purchase the System, you must deliver notice to IGS
Solar of your intent to purchase and pay the greater of:
A. THE MINIMUM PURCHASE PRICE
The Minimum Purchase Price will be equal to the
present value of the forecasted Lease Payments over
the Term then remaining, discounted at a rate of5%;
or
B. THE FAIR MARKET VALUE FOR THE SYSTEM
purchaseThe FMV will be calculated by an independent
appraiser who is selected by agreement between
you and IGS Solar. IGS will be responsible for the cost
of the appraisal.

requirements. If the buyer meets IGS Solar's credit
requirements, you and your approved buyer will
execute a written assignment of this Lease in a
form approved by IGS Solar. You will execute all
documents necessary to assign the Lease. Under an
assignment approved by IGS Solar, your assignee
will have all of your rights under this Lease and the
Limited Warranty.
C. IF YOU, YOUR ESTATE, OR HEIRS SELL THE
PROPERTY and cannot comply with any of the
options listed above, you will be in default under
this Lease.
D. EXCEPT AS SET FORTH IN THIS SECTION
1, you will not assign, sell, pledge, or in any other
way transfer your interest in this Lease without
IGS Solar's prior written consent, which will not be
unreasonably withheld.

the applicable forms at least 30 days before the

Lease will automatically renew for additional terms
of one year each, with the Lease Payments at the
same amount as Contract Year 25 until:
i. you give IGS Solar notice at least 30 days before
a renewal term that you do not wish to renew; or
ii. we send you a notice cancelling the automatic
renewal.
Upon expiration or termination of this Lease, if you have
not renewed this Lease or purchased the System, IGS
Solar may choose to remove the System and contact
you to schedule a convenient time to have the System
removed from your Property at no cost to you. If IGS
Solar chooses not to remove the System, we will notify
you in writing, and the System will be automatically
conveyed to you AS -IS, WHERE IS, and you will become
solely responsible for payment of all taxes, fees, and
charges, if any, imposed in connection with the transfer
of ownership of the System,
Unless you are negligent or intentionally damage the
System, IGS Solar will bear all of the risk of loss, damage,
or theft of the System. If there is a loss affecting the
System, you will continue to pay on time all Lease
Payments and other amounts you owe under this
Lease and cooperate with IGS Solar to have the System
repaired or replaced.

As part of the Lease, IGSSola
Warranty for the System on your Property. This Limited
Warranty will take effect when the System installation
begins at your Property. This Limited Warranty will not
apply if you purchase the System.
A. SYSTEM WARRANTY
The System will be professionally installed. During
the Term, under normal use and service conditions,
the System will befree from defects inworkmanship
and breakdown ofmaterials orcomponents.
B. ROOFTOP SYSTEMS WARRANTY
If your System is a rooftop system, all of the
penetrations will be watertight for a period of 5 years.
A. OPERATION
IGS Solar guarantees that during thehe System
will operate within manufacturer's specifications,
and if it does not, that IGS Solar will arrange to
repair or replace any defective part and restore
System operation. Any manufacturers' warranties
applicable to the System are for the benefit of IGS
Solar. Regardless of any other provision in this Lease,
including its exhibits and addenda, IGS Solar does
not guarantee that the System will produce a certain
amount of kWh during the Term.
B. MONITORING
During the Tenn' unless you purchase the

our monitoring service at no additional cost. The
monitoring system captures and displays historical
power generation data and consists of hardware

when installed in strict compliance with the manufacturers printed instructions.
Regarding the solar panel array installation on the above referenced project please note that an inspection was
performed by a representative of the Architect/Engineer of Record, and analysis of the existing structure was
conducted. There is adequate structural capacity for the installation of the array with the following recommendations:
1. The array will be installed on the existing roof. The roof framing is constructed of 2"W' wood trusses @24" o.c.
spanning 23' 11 " with a top chord length of 26'with intermediate supports at 1/4th points, making the effective longest
span equals 8'6" with 1/2" plywood sheathing. The new array (See Site map by contractor) will add 2,63 Lb. / Sf.
overall to the roof The existing structure is sufficient to support the new loads associated with the additional weight &
wind resistance. No additional structural support is required for the roof structure,
2. The attachment system shall be secured to the roof and shall be in strict compliance with manufacturers printed
instructions. The attachment system shall be UL 1703 approved tested. Provide water tight sealant at all penetrations.
Attachments shall follow panel rows as specified by the system manufacturer's installation manual. The panel angle
