Use of fill is not allowed in Flood Zone 'V' unless expressly permitted.
If the fill material is to be used in Flood Zone "A", it is understood that a drainage plan addressing a
"compensating volume" will be submitted at time of permitting which is prepared by a professional engineer
licensed by the State of Florida,
if the fill material is to be used in Flood Zone "A" in connection with a permitted building using stem wall
construction, I certify that fill will be used only to fill the area within the stem wall.
If fill material is to be used in any area, I certify that use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversely affect adjacent properties, the owner may be cited for violating
the conditions of the building permit issued under the attached permit application, for lots less than one (1)
acre which are elevated by fill, an engineered drainage plan is required.
If I am the AGENT FOR THE OWNER, I promise in good faith to inform the owner of the permitting conditions set forth in this affidavit prior to
commencing construction. I understand that a separate permit may be required for electrical work, plumbing, signs, wells, pools, air conditioning, gas, or
other installations not specifically included in the application. A pen -nit Issued shall be construed to be a license to proceed with the work and not as
authority to violate, cancel, after, or set aside any provisions of the technical codes, nor shall issuance of a permit prevent the Building Official from
thereafter requiring a correction of errors in plans, construction or violations of any codes. Every permit issued shall become invalid unless the work
authorized by such permit is commenced within six months of permit Issuance, or 9 work authorized by the pen -nit is suspended or abandoned for a
period of six (6) months after the time the work is commenced. An extension may be requested, in writing, from the Building Official for a period not to
exceed ninety (90) days and will demonstrate justifiable cause for the extension. It work ceases for ninety (90) consecutive days, the job is considered
abandoned.

25-Year Limited Roofing Penetration Warranty. Efficient Home Services of Florida LLC
warrants Your roof against damage and water infiltration at each roofing penetration
made by Efficient Home Services of Florida LLC in connection with the installation of
the System and the surrounding area of each such penetration (collectively, the
"Covered Roof Areas"). Efficient Home Services of Florida LLC will repair damage to
Your roof and repair or compensate You for actual physical damage to Your property

New Roof Installation
Efficient Home Services of Florida LLC is not liable for consequential damages to the building or
to the contents resulting from any work performed by said company in accordance with the
terms of this agreement. Efficient Home Services of Florida LLC total liability shall not exceed
the original contract price amount. Job must be paid in full or any implied / expressed warranty
becomes invalid. Due to the nature of the roofing work to be done at the customer's request
and the extreme weight of the roofing materials, the customer takes sole responsibility and
Efficient Home Services of Florida LLC disclaims any liability for damaged caused to broken,
cracked, damaged or punctured: (interior), drywall, ceiling tape, nail pops or texture damage,
electrical wires, plumbing lines or HVAC lines that are too close to the roof framing and are
hidden in the attic. (Exterior), walkways, driveways, leaking gutters, septic tanks or drain fields,
Customer is responsible for removing any items / belongings from walls, ceilings and under
work areas which might become dislodged or damaged during the work process do to working
vibrations or a weather mishap. Customer must provide truck access to work site. Customer
must notify Efficient Home Services of Florida LLC of any exposed roof sheathing and therefore
accepts responsibility for nails penetrating any exposed sheathing. Customer understands that
due to the manufacturing process of roofing tiles there can be slight color differences in the
roofing tiles themselves and that Efficient Home Services of Florida LLC is not liable for this
normal occurrence. This contract shall be voidable by the company in the event it becomes
commercially impractical to perform the contract due to substantial labor or material increases,

building code modifications, or other circumstances beyond the control of the company of a
similar or dissimilar nature, of this contract. Drone disclaimer Customer agrees that Efficient
Home Services of Florida LLC and/or its agents or affiliates may use a drone or other imaging
device to take photos and/or videos of Customer's property. Customer hereby allows Efficient
Home Services of Florida LLC to use any media obtained for educational, promotional, legal, or
any other use as desired by Efficient Home Services of Florida LLC. If any additional work that is
NOT included in this proposal is requested after acceptance of this proposal, it will be
considered a change order. It will not be included in this contract unless both parties agree in
writing. 3rd nail, soffit, aluminum fascia, gutter work is NOT a part of this contract. Replacement
of up to 3 sheets of damaged plywood, 4x 8".Replacement of additional plywood to cost $100
per 4'x 8'.Any extra wood replacement excluding Plywood will be charged at 20$ per Linear
Foot.
Tax Credit Disclaimer
Efficient Home Services of Florida LLC is not responsible for obtaining rebates or tax
credits related to your solar project. Regardless of whether you independently apply for
and receive a government rebate or tax credit related to your solar project, you are
responsible for repaying the full amount of your loan. Efficient Home Services of Florida
LLC makes no guarantees regarding the effectiveness of your solar investment, nor do
we guarantee you will receive rebates or tax credits related to your project. We
recommend consulting a tax expert with any questions regarding your tax liability.
Cancellation
Any projects canceled after the 3 day right of rescission will be charged a cancellation
fee of 10% of the agreement. Any deposits will be refunded 2-4 after the cancellation
date.
Right to Terminate Service Agreement
Efficient Home Services of Florida LLC reserves the right to terminate any service
contract to customers who repeatedly treat Efficient Home Services employees and
contractors in a foul or rude manner. Direct disrespect to our staff and employees will
not be tolerated via in person or over any phone communication. A written and verbal
termination will be assessed for repeat offenders.
By signing this contract, the customer also express y ac nowledges an• agrees t at -
this contract constitutes the entire agreement of the parties hereto concerning its
subject matter and supersedes all prior and contemporaneous representations,
proposals, discussions, and communications, whether oral or in writing: and (11) no other
promise or agreement of any kind has been made to or with the customer by any person
only in writing and shall be enforceable per its terms when signed by each of the parties
hereto.
You, the buyer, may cancel this transaction at any time before midnight of the third
business day after the date • this transaction. See the attached •: of cancellation

This letter is regarding the proposed installation of a rooftop -mounted Solar PV system on the existing residential
structure at the subject address. I have reviewed the attachment plan and have determined that the rooftop -mounted
PV system is in compliance with the applicable sections of the following Codes as amended and adopted by the
jurisdiction when installed in accordance with the manufacturer's installation instructions:

Design Criteria: Design Wind Speed(Vult) - 140 mph 3sec gust, Exposure Category - C, Risk Category 11
The rooftop -mounted photovoltaic panel system has been designed in accordance with FRC R324.4. When roof
penetrations are necessary, they shall be flashed and sealed in accordance with the manufacture's installation
instructions, R905.17.3. The PV system consist of the modules, railing, and connection hardware. Refer to the specific
roof type calculation pages for PV dead loads. The portions of the existing structure covered with solar panels will be
adequate for supporting the roof loads per R324.4.1.1.
The securernent method of the the PV system is to be mounted parallel to the structure with the site specific railing
and attachments according to the designed plans. The site specific wind load calculations for the module and their
supports are attached with this document, Fasteners shall be installed to the designated roof member with the proper
torque from the manufactures installation instructions.
The design wind pressures for rooftop solar panels located on enclosed or partially enclosed buildings of all heights,
with panels parallel to the roof surface with a tolerance of 2' and with a max height above the roof surface, h2, not
exceeding 10 in. A min gap of 0.25 in shall be provided between all panels with the spacing of gaps between panels not
exceeding 6.7 ft. in addition the array shall be located at least 2h2 from the roof edge, a gable ridge, or a hip ridge.
It is the contractors responsibility to review all drawings for accuracy and notify the FOR of any discrepanices prior
to beginning construction, To the best of my knowledge, the plans and specifications comply with the minimum
requirements of the latest Florida Building code,
