TRANSPORTATION IMPACTIUTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands
that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of now buildings, change of use In existing buildings, or
expansion of existing buildings, as specified in Pasco County Ordinance number 89-07 and 90-07, as amended, The undersigned also understands, that

Use of fill is not allowed in Flood Zone 'V" unless expressly permitted.
If the fill material is to be used in Flood Zone "A", it is understood that a drainage plan addressing a
.Compensating volume" will be submitted at time of permitting which is prepared by a professional engineer
licensed by the State of Florida.
If the fill material is to be used in Flood Zone "A" in connection with a permitted building using stem wall
construction, I certify that fill will be used only to fill the area within the stem wall,
If fill material is to be used in any area, I certify that use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversely affect adjacent properties, the owner may be cited for violating
the conditions of the building permit issued under the attached permit application, for lots less than one (1)
acre which are elevated by fill, an engineered drainage plan is required,
If I am the AGENT FOR THE OWNER, I promise in good faith to inform the owner of the permitting conditions set forth in this affidavit prior to
commencing construction. I understand that a separate permit may be required for electrical work, plumbing, signs, wells, pools, air conditioning, gas, or
other installations not specifically included in the application. A permit issued shall be construed to be a license to proceed with the work and not as
authority to violate, cancel, after, or set aside any provisions of the technical codes, nor shall issuance of a permit prevent the Building Official from
thereafter requiring a correction of errors in plans, construction or violations of any codes. Every permit issued shall become invalid unless the work
authorized by such permit is commenced within six months of permit issuance, or if work authorized by the permit is suspended or abandoned for a
period of six (6) months after the time the work is commenced. An extension may be requested, in writing, from the Building Official for a period not to
exceed ninety (90) days and will demonstrate justifiable cause for the extension. If work ceases for ninety (90) consecutive days, the job Is considered
abandoned.

PAYMENT MAY BE AVAILABLE FROM THE FLORIDA HOMEOWNERS'CONSTRUCTION RECOVERY FUND IF YOU LOSE MONEY ON A PROJECT
PERFORMED UNDER CONTRACT, WHERE THE LOSS RESULTS FROM SPECIFIED VIOLATIONS OF FLORIDA LAW BY A LICENSED CONTRACTOR.

to install and complete the project in a timely manner, customer understands that there are no deadlines or timetables associated with this

LIEN: Upon delivery of certified notice to owner, the buyer has 15 days to issue payment for the below amounts, if no payment has been
received within the 15days, Sunlight has the right to place a lien on the property. Once payment of cancellation completed the lien shall be
removed promptly.
1. SUNLIGHT makes nowritten orverba|representations with regard to: State ofFlorida solar rebates, U.S.Federal innometmxcredits
or utility incentives ("Incentives"). CUSTOMER shall be solely responsible for determining the applicability ofsuch incentives, and
should consult with CUSTOMER'S attorney, financial advisors and | officials in this regard. CUSTOMER releases
SUNLIGHT from any liability regarding the applicability ofsuch |ncentives.| Jf91_99SOnitia|
2. SUNLIGHT makes no guarantees or representations regarding the effectiveness of the solar systems that it installs. In some
instances, SUNLIGHT may provide anestimated as tohow much power and/or production aCUSTOMER may expect tosave asa
result of their contract, however, and ultimately the power output is entirely dependent upon factors that are outside the control
ofSUNLIGHT, ie,the amou that reaches the CVSTONlERSEVpanels (due topos�iunin�amount ofson|ightshading
due totrees and/other o�ects)�j�i���nitia|.
3. In the event that the merchandise or installation are defective or nonconforming, the UabUuv ofSUNLIGHT shall be limited to
repairing or replacing the non -conforming merchandise or installation pursuant to the terms of the Limited Warranty stated herein
and there shall benoreduction orabatement ofthe amAwr4ue under this Contract. SUNLIGHT SHALL NOT 8ELIABLE FOR ANY

4. Payment in full is due upon labor installation date. Payments not received uptoseven business day after labor installation date
will be charged an additional $500 penalty fee. The balance will be applied with a b|-week|yinterest charge ofOj%.shall besubject

parts thereof are property of SUNLIGHT. Any equipment and parts thereof are entitled to repossession by Sunlight if full payment

5. Limited Warranty:
(a) For a period of thirty (30) years from the date of receipt of equipment or installation, SUNLIGHT will repair or replace
Photovoltaic electric systems (PV) which is defective because of poorworkmanship only. The (30) yearwarranty will also cover
leaking directly caused by Sunlight Solar roof penetration, but not age deterioration of roof or outside the act of Sunlight's
Labor. This limited warranty applies only to Photovoltaic electric system (PV) and does not cover DHW, Solar pool panels, solar
pool pumps and other non-PV items. Product warranty on all Non-PV related items shall be per manufacturer's warranty and
SUNLIGHT shall provide labor warranty ofZyears from the installation day on those Non-pv items. This warranty does not
cover defects which result from misuse, abuse, fire, acts of God, improper handling of the equipment by persons not in the
employ of SUNLIGHT, vandalism, water damage or freeze damage, THIS LIMITED WARRANTY DOES NOT COVER INCIDENTAL
OKCONSEQUENTIAL DAMAGES, AND IS IN LIEU OF ANY OTHER WARRANTY, EXPRESS OR IMPLIED. This warranty gives you
specific legal rights, and you may also have other rights which vary from state to state.
(b) CUSTOMER understands and acknowledges that proper operation of the equipment purchased hereunder is dependent upon
CUSTOMER'S existing roof, plumbing, electrical, structural, mechanical and /or pool equipment being in good order and
operation. SUNLIGHT shall have no obligation or liability for the condition thereof or any other part of CUSTOMER'S existing

roof, plumbing, electrical, structural, mechanical and/or pool equipment. SUNLIGHT shall further have noobligation inthe
event that the equipment purchased hereunder is unable to properly function due to the condition of CUSTOMER'S then
existing roof, plumbing, electrical, structural, mechanical and or/pool equipment.
(c) It is understood and agreed byCUSTOMER that SUNLIGHT'S liability, and CUSTOMER'S sole remedy, whether in contract or
tort, shall not exceed the ngum to CUSTOMER of the amount of the invoice actually paid by CUSTOMER, reduced by
SUNLIGHT'S expenses relating tothe providing ofequipment and services to CUSTOMER. Under no circumstances yhoU
SUNLIGHTbe liable for any special, incidental, consequential, orotherclamages incurred byCUSTOMER, itsagents, employees,
contractors, representatives, heirs, successor, assigns or administrators. Such waiver shall also include damages for personal
injury, property damages, lost profits, lost wages, repair or replacement of equipment, or any other damages.
6. For any solar electric system to have viable system monitoring, it is required to have avalid intemetconnecdon in the home
SUNLIGHT shall provide hardwire connection at no extra cost. If client prefers to use a wireless connection, the customer is
assuming that any changes to the internet provider, router or password will lose connection and thabilitytomonitor
the solar electric system. A service fee will be applied to schedule a visit to reconnect ittothe intem itia|
7. Consent of HOA: As a homeowner I hereby acknowledge that pursuant to Florida Statute 163.04, homeowner and condominium
associations are expressly prohibited from preventing its members from installing energy devices based on renewable energy
resources on buildings erected on the lots or parcels covered by the declaration or binding agreement. Homeowners further
acknowledge that such associations are permitted to determine the specific location where such devices may be installed.
Notwithstanding the foregoing, homeowners have elected to install Solar PV panels on their property prior to receiving any written
approval by their association or governing body. Homeowner agrees to indemnify, defend and hold harmless Sunlight Enterprises,
Inc DBA Sunlight Solar and its members, managers, affiliates, employees, officers, representatives, agents, successors and assigns
(collectively, the "Indemnified Parties"), from and against all claims, demands, lawsuits, liabilities, damages, judgements, fines, loss
to persons o,property, costs and expenses, including reasonable attorneys' fees and costs, and any other amounts reasonably
incurred orsuffergp-Wicluding attorneys' fees) in connection with Sunlight Enterprises, Inc DBA Sunlight Solar's installation of the
Solar PVpanel itia|
8. eysigning below, the parties acknowledge and agree:
a. I understand that the sales rep visited today to educate me about my opportunity to take advantage of a Federal Tax
Credit and save energy byinstalling solar products onmyhome.
b. The rep did not state or imply that our company is affiliated with a government entity or electric utility.
c. The rep did not assert or guarantee any utility costs increases in the future and I understand that no one can know what
will happen with future utility costs.
d. Upon finishing the installation, and submitting the project tobeinterconnected tnthe local power grid, Sunlight shall
not be responsible for any timeline/timeframe that the utility company takes to have the customer's project approved
and/or connected tothe local power grid,
e, Independent of power production, my electric company will always charge me at least for a meter fee and applicable
taxes.
f. Sunlight will not provide any payment reductions when there is a delay in interconnection due to customer's inaction.
& The rep did not assert or guarantee any specific savings on my future electric bills. I understand that my actual saving
will depend upon myown energy -use habits.
h. I understand that a tax credit from the Federal Government for installing solar products may reduce my income tax
obligation, but if I don't pay enough in income taxes, the tax credit may take more than one year to receive in full.
i. I am in full understanding that any and all rebates, incentives or tax credits are the sole responsibility of me, the
homeowner, to inquire about, apply for and receive; not that of Sunlight Enterprises Inc.
j� If I'm applying for financing SUNLIGHT is not affiliated or an agent of any financing company and the terms and
conditions of any financing agreement are not apart of this subject contract.
9. General Stipulations:
a. Sunlight is not responsible for county delays on permit approval, inspection schedule or for the electric company
timeframetoapprove the interconnection process.
b. If the customer is enrolled in a budget billing program with his/her electric company, he/she needs to cancel it as soon
aspossible tocollect the solar energy benefits. Sunlight isnot responsible for losses related tothat.
c. The customer has up to 72h to request an installation date change without any penalty after confirmed. if the customer
requests achange after that a$98Ofee will beapplied
d. If the customer approves a Wi-Fi connection for the solar system and decides to change the provider or password a
service call fee will beapplied tufix itonce isnot covered bvwarranty.
e. Sunlight Solar isnot responsible for Double billing ifaproject delays based oncounty u,electric company processes out
ofSunlight control

The Agreement includes a Solar Insure Component Monitoring and Warranty that coverscomponent defects
to the solar powered photovoltaic system, roof penetration within 3 inches of the solar roof attachment and
related labor costs. Customer agrees togive Solar Insure site access totheir system'smonitoring data. Internet
connectivity must be enabled throughout the coverage term. Full product details will be provided to the
Customer separate from this document in their monitoring and limited warranty,
The limited warranty provided bySolar Insure is separate and apart from Contractor's limited warranties as
may be set forth in the Agreement and while coverage under the Solar Insure Warranty solely rests with Solar
Insure, Customer should contact Contractor for coordination of Solar Insure warranty requests and services.
Customer agrees and acknowledges that Solar Insure is a supplier of component monitoring and warranty
and not an affiliate of Contractor and that any and all warranty obligations of Solar Insure are not the
obligations ofContractor.
Except as set forth in this Addendum, the undersigned reaffirm the terms and conditions of the Agreement.
This Addendum and the foregoing terms and conditions are hereby agreed to, and the undersigned
acknowledge receipt.

This letter is regarding the proposed installation of a rooftop -mounted Solar PV system on the existing residential
structure at the subject address. i have reviewed the attachment plan and have determined that the rooftop -mounted PV
system is in compliance with the applicable sections of the following Codes as amended and adopted by the jurisdiction
when installed in accordance with the manufacturer's installation instructions:

Design Criteria: Design Wind Speed(Vult) - 140 mph 3sec gust, Exposure Category- C, Risk Category 11
The rooftop -mounted photovoltaic panel system has been designed in accordance with FRG R324,4. When roof
penetrations are necessary, they shall be flashed and sealed in accordance with the manufacture's installation instructions,
11905.17.3. The PV system consist of the modules, railing, and connection hardware. Refer to the specific roof type
calculation pages for PV dead loads. The portions of the existing structure covered with solar panels will be adequate for
supporting the roof loads per R324,4,1.1.
The securement method of the the PV system is to be mounted parallel to the structure with the site specific railing and
attachments according to the designed plans, The site specific wind load calculations for the module and their supports are
attached with this document. Fasteners shall be installed to the designated roof member with the proper torque from the
manufactures installation instructions.
The design wind pressures for rooftop solar panels located on enclosed or partially enclosed buildings of all heights, with
panels parallel to the roof surface with a tolerance of 2' and with a max height above the roof surface, h2, not exceeding 10
in. A arin gap of 0.25 in shall be provided between all panels with the spacing of gaps between panels not exceeding 6.7 ft in
addition the array shall be located at least 2h4 from the roof edge, a gable ridge, or a hip ridge.
It is the contractors responsibility to review all drawings for accuracy and notify the FOR of any discrepancies prior to
beginning construction. To the best of my knowledge, the plans and specifications comply with the minim rn r�t i��r�i� t ��
