"statement of CoJ:cpletion and Request for Transfer to Operation
Entity" form (copy enclosed) certifying that the project and system
are built in compliancE! with pennitting requirements and requesting
the system qualifies for operation phase authorization.
Every effort should be taken during construction to prevent erosion
and transport and discharge of sediment to wetlands or any property
other than your own.
Please be advised that any person who is substantially affected by
the District's Final Agency Action concerning a Pennit may challerge
this Pennit by requesting an Administrative Hearing in accordance
with Section 120.57, Florida statutes (F.S.), and Part V of Olapter
40D-1, Florida Administrative Code (F.A.C.). A request for hearing
