A. NOTICE OF DEED RESTRICTIONS
The undersigned understands that this perlit may be subject to "de~d restricti~ns" which ~ay b~ ~~re restrictive than City
regulations. The undersigned assules responsibility for compliance with any applicable deed restrictions.
B. UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES
If the owner has hired a contractor' or contractors to undertake work, they lay be required t~ be licensed in accordance with
state and local regulations. If the contractor is not licensed as required by law, both the owner and contractor ~ay be
cited for a lisdeneanor violation under state law. If the owner ~r intended contractor are uncertain as to what licensing
requirelents lay apply for the intended work, they are advised to contact the City of Zephyrhills Building Department, (BI3)
788-6611.
Furthermore, if the owner has hired a contractor or contractors, he is advised to have the contractor(sl sign portions of the
'Contractor Sections' of this application for which they will be r~sponsible. If you, as the ofln~r sign as the contractor,
you are indicating that you, rather than the contractor, are responsible for the work. If the contractor wish~s you to sign
as contractor that lay be an indication that he is not properly licensed and is not entitled te, permitting privileges in the
City of Zephyrhills.

MARTELL, his wife and JOHN M. BOMMARITO and THOMAS M. BOMMARITO, as joint
tenants with the right of survivorship and not as tenants in common, are
aware that the 1992 Real Estate Tax Roll has been not published, and that
any shortage or overage due to the proration of the taxes on the basis of
the 1991 Tax Roll are not the responsibility of ASSOCIATED LAND TITLE
