A. NOTICE OF DEED RESTRICTIONS
The undersigned understands that this perlit 'lay be subject to "deed restrictions" which aay be lore restrictive than City
regulations. The undersigned assules responsibility for coapliance with any applicable deed restrictions.
B. UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES
If the owner has hired I contractor or contractors to undertake work, they lay be required to be licensed in accordance with
state and local regulations. If the contractor is not licensed as required by law, both the owner and contractor lay be
cited for a lisdeleanor violation under state law. If the owner or intended contractor are uncertain as to Khat licensing
requirelents lay apply for the intended work, they are advised to contact the City of Zephyrhills Building Departlent, (813)
788-6611.
Furtherlore, if the owner has hired a contractor or contractors, he is advised to have the contractorls) sign portions of the
"Contractor Sections" of this application for which they will be responsible. If you, as the owner sign as the contractor,
you are indicating that you, rather than the contractor, are responsible for the work. If the contractor wishes you to sign
as contractor that lay be an indication that he is not properly licensed and is not entitled to perlitting privileges in the
City of Zephyrhills.

addressing a "co.pensating volu.e" will be sublitted which is prepared by a professional engineer registered in the State of
Florida prior to perlit issuance.
A perlit issued shall be construed to be a license to proceed with the Mork and not as authority to violate, cancel alter, or
set aside any provisions of the technical codes, nor shall issuance of a pertit prevent the Building Official frot thereafter
requiring a correction of errors in plans, construction, or violations of any code. Every perlit issued shall becole invalid
unless the work authorized by such perlit is cottenced within six tonths of issuance, or if work authorized by the per.it is
suspended Dr abandoned for a period of six lonths after the ti.e the work is cot.enced. One 90 day extension of ti.e, .ay be
allowed for the per.it with fee charge of $15.00. The extension shall be requested in writing to the Building Official. An

A G R E E MEN T
THIS AGREEMENT, made and entered into this ~fi day of

ZEPHYRHILLS, hereinafter referred to as the "City" and L.B. McKell
Construction Company, a Florida company, herein after referred to as
the "Contractor" for rental rehabilitation improvements to 4 rental
units 38608 5th Ave., 38604 5th Ave., 38608 5th Ave., 5317 10th St.
FOR CONSIDERATION and mutual covenants contained herein, the
parties agree as follows:
1. The Contractor will commence and complete the rental
rehabilitation improvements as specified in the attached bid.
2. The Contractor will furnish all materials, supplies, tools,
equipment, labor and other services necessary for the construction
and completion of the above-referenced project.
3. The Contractor will commence the work required

4. The Contractor agrees to perform all of the work necessary
to complete the improvements in accordance with the bidding
specifications. In addition, the Contractor shall perform all work
described in the advertisement for bid, information for bidders and

5. All work by the Contractor shall be performed in a good and
workmanlike manner.
6. The City and property owner (on a 50/50 split) shall pay to
the Contractor th~ sum of Six Thousand Two Hundred Seventy Dollars
($6,270.00) payable within

days of final inspection and
acceptance by the City.
7. The Contractor's total compensation shall be reduced by
$200.00 for each day or fraction of a day that the installation is
not completed commencing at 12:01 a.m. on~, A.D., 1993.

8. This Agreement shall be pinding upon all parties hereto and
their respective heirs, executors, administrators, successors and
assigns.

Sealed bids will be received at the Office of the City
Manager, 5335 8th Street, Zephyrhills, Florida 33540, until 4
p.m., Thursday, November 5, 1992, for the following:
Rental Rehabilitation Program
(Improvements to a quadplex)
Bids must be in a sealed envelope plainly marked "Bids on
Rental Rehabilitation Program". Bids will be opened on
Thursday, November 5, 1992, at 4 p.m. at the Council
Chambers, 5335 8th Street, Zephyrhills, Florida 33540. The
bid opening is open to the public.
The City of Zephyrhills reserves the right to reject any
and/or all bids or any portion thereof, and/or accept the bid
deemed to be in the best interest of the City. Bids may be
accepted in whole or part. Any bid received later than 4
p.m. November 5, 1992 will be returned unopened.
SCOPE OF WORK:
The work specified herein consists of furnishing all required
labor, materials, supplies and implements necessary for the
improvements to the quadplex as specified.
ADDITIONAL INFORMATION:
Improvements will be made to a quadplex (two one-bedroom
apartments and two two-bedroom apartments) requiring specific
rehabilitation. Required improvements are available from the
Building Department at 788-6611 and include electrical work,
carpentry, installation of central heat and air conditioning,
flooring, window replacement, etc. The quadplex is located at
5317 10th St., and 38605 5th Ave., Zephyrhills, Florida
33540. Inspections are by appointment only.
The City of Zephyrhills is a subrecipient of the Rental
Rehabilitation funds provided by the U.S. Department of
Housing and Urban Development (HUD) and are disbursed through
Pasco County, Florida. This is a federally funded program
and federal guidelines must be adhered to. Davis-Bacon wage
rates do not apply to residential projects.
INSURANCE:
Those submitting bids shall furnish evidence of liability
insurance, workman's compensation insurance and unemployment
insurance if applicable. Eligibility will be required to
accept federal compensation and bidding firms will be checked
to insure they are not on the Federal Debarred List.
BID FORM:
Each unit bid must specifically address proposed improvements
and bids shall be submitted on a bid form provided for this
project.
BID BOND:
Each proposal must be accompanied by a certified check or bid
bond executed on the prescribed bid form, payable to the City
of Zephyrhills in an amount not less than 5 percent of the
bid amount.
Bid bonds will not be returned until after a contract is
awarded and signed or all bids are rejected by the City
Council of the City of Zephyrhills. A recommendation to
award the bid will be placed before the City Council at their
regular Council meeting Monday, November 9, 1992, at 7 p.m.
