REINSPECTION FEES: When extra inspection trips are necessary due to anyone of the following reasons, a
charge of Fifteen and 00/100 Dollars ($15.00) shall be made for each trip for each trade:
a. Wrong Address
b. Condemned work resulting from faulty construction.
c. Repairs or corrections not made when inspection called.
d. Work not ready for inspection when called.
e. Permit not posted on job site.
f. Plans not at job site.
g. Work not accessible.
The payment of inspection fees shall be made before any further permits will be issued to the person owning
same.

A. NOTICE OF DEED RESTRICTIONS
The undersigned understands that this perlit lay be subject to "deed restrictions" which iay be lore restrictive than City
regulations. The undersigned assules responsibility for cOlpliance with any applicable deed restrictions.
B. UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES
If the owner has hired a contractor or contractors to undertake work, they lay be required to be,licenied in accordance with
state and local regulations. If the contractor is not licensed as required by law, both the owner and contractor lay be
cited for a .isdeleanor violation under state law. If the owner 'c~ intended contractor are uncertain as to what licensing
requirelents lay apply for the intended work, they are advised to contact the City of Zephyrhills Building nepartment, (813l
788-6611.
Furthermore, if the owner has hired a contractor or contractors, he is advised to' have the contractor(sl sign portions of the
"Contractor Sections" of this application for which they will be responsible. If you, as the owner sign as the contractor,
you are indicating that you, rather than the contractor, are responsible for the work. If the contractor wishes you to sign
as contractor that lay be an indication that he is not properly licensed and is not entitled to permitting privileges in the
City of Zephyrhills.

I certify that I, the applicant, have been provided with a copy of "Florida's Construction lien law - Homeowner's Protection
Suide" prepared by the Florida nepartlent of Agriculture and Consu.er Affairs. If the applicant is so.eone other than the
"owner", I certify that I have obtained a copy of the above described doculent and promise in good faith to deliver it to the
"owner" prior to co.mencelent.
E. CONTRACTOR'S/OWNER'S AFFIDAVIT
I certify that all the inforlation in this application is accurate and that all work will be done in compliance with all
applicable laws regulating construction, zoning, and land developlent.
Application is hereby lade to obtain a perlit to do work and installation as indicated. I certify that no work or
installation has co..enced prior to issuance of a pertit and that all work will be performed to meet standards of all laws
regulating construction, City codes, zoning regulations, and land development regulations in the jurisdiction. I also
certify that I understand that the regulations of other govern.ental agencies lay apply to the intended work, and that it is

addressing a "co.pensating volule" will be sublitted which is prepared by a professional engineer registered in the State of
Florida prior to permit issuance.
A perait issued shall be construed to be a license to proceed with the work and not as authority to vioj~te, cancel alter, or
set aside any provisions of the technical codes, nor shall issuance of a permit prevent the Building Uffici~1 from thereafter
requiring a correction of errors in plans, construction, or violations of any code. Every pertit iSSUed ;nall becole invalid
unless the liork authc.rized by such penit is cc..aenced within six months of issuance, c,r if work alltllollIed by the perait is
suspended or abandoned for a period of six lonths after the tile the work is commenced. One 90 day e~te~sJo" of tile, may be
allowed for the permit with fee charge of $15.00. The extension shall be requested in writing to the Building Official. An
approved inspection must be logged during each six lonth period, or the project will be considered dDrliJdoned.

RESULT IN YOUR PAYING TWICE FOR IMPROVEMENTS TO YOUR PROPERTY. IF YOU
INTEND TO OBTAIN FINANCING, CONSULT WITH YOUR LENDER OR AN A1TORNEY

3. It is agreed that this contract shall not become binding upon the Contractor until it is approved by an officer of the

4. Any representations or agreements not contained herein shall not be binding on either part.~. All of the agreements and

5. In the event it becomes necessary for the Contractor to employ an attorney to recover monies due under this agreement, or to
enforce any of the terms and provisions hereof, Owner(s) will be liable for, and agree(s) to pay, all costs and expenses, including
reasonable attorney's fees, necessarily incurred by Contractor as a result, regardless of whether suit is filed or not.
6. All unused malerials shall, under any conditions, remain the property 01 the Contractor.
7. The undersigned warrant(s) that he is (they are) the owner(s) of the above mentioned premises and the legal title thereto
stands of record in his (their) name(s).
8. "BUYER'S RIGHT TO CANCEL": This is a home solicitation sale, and if you do not want the goods or services, you may
cancel this agreement by providing written nollce to the seller in person, by telegram, or by mail. This notice must indicate that
you do not want the goods or services and must be delivered or postmarked belore midnight of the third business day after you
sign this agreement. If you cancel thiS agreement, the seller may not keep all or part of any cash down payment.
9. In the event Owner attempts to cancel this agreement after the date and time allowed for cancellation under paragraph 8 of this
agreement, Owner(s) will be liable for, and agree(s) to pay Contractor, all damages which Contractor sustains as a result of such
cancellation, including loss of profit, Contractor's cost of any materials specially fabricated, and a 15% re-stocking charge for all
materials required for the work specified above.
THE OWNER(S) HEREBY ACKNOWLEDGE(S) RECEIPT OF A COMPLETELY; EXEdutED COpy OF THIS CQNTRACT.
