REINSPECTION FEES: When extra inspection trips are necessary due to anyone of the following reasons, a
charge of Fifteen and 00/100 Dollars ($15.00) shall be made for each trip for each trade:
a. Wrong Address
b. Condemned work resulting from faulty construction.
c. Repairs or corrections not made when inspection called.
d. Work not ready for inspection when called.
e. Permit not posted on job site.
f. Plans not at job site.
g. Work not accessible.
The payment of inspection fees shall be made before any further permits will be issued to the person owning
same.

A. NOTICE OF DEED RESTRICTIONS
The undersigned understands that this per.it lay be subject to "deed restrictions" which may be more restrictive than City
regulations. The undersigned sssules responsibility for compliance with any applicable deed restrictions.
B. UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES
If the owner has hired a contractor or contractors to undertake work, they eay be required to be licensed in accordance with
state and local regulations. If the contractor is not licensed as required by law, both the owner and contractor ~ay be
cited for a misdemeanor violation under state law. If the owner or intended contractor are uncertain as to what licensing
requirelents may apply for the intended work, they are advised to contact the City of Zephyrhills Building Department, (813)
788-6611.
Furtherlore, if the owner has hired a contractor or contractors, he is advised to have the contractor(sl sign portions of the
'Contractor Sections" of this application for which they wil.l be responsible. If you, as the owner sign as the contractor,
you are indicating that you, rather than the contractor; are responsibie for the work. If the contractor wishes you to sign
as contractor that lay be an indication that he is not properly licensed and is not entitled to permitting privileges in the
City of Zephyrhills.

I certify that I, the applicant, have been provided with a copy of "Florida's Construction Lien Law - Ho~eowner's Protection
Guide" prepared by the Florida Departlent of Agriculture and Consumer Affairs. If the applicant is so~eone other than the
"owner", I certify that I have obtained a copy of the above described document and promise in good faith to deliver it to the
"owner" prior to cOlmence~ent.

I certify that all the information in this application is accurate and that all work will be done in co~pliance with all
applicable laws regulating construction, zoning, and land development.
Application is hereby aade to obtain a permit to do work and installation as indicated. I certify that no work or
installation has co&menced prior to issuance of a perlit and that all work will be performed to meet standards of all laws
regulating construction, City codes, zoning regulations, and land development regulations in the jurisdiction. I also
certify that I understand that the regulations of other governmental agencies may apply to the intended work, and that it is

set aside any provisions of the technical codes, nor shall issuance of a permit prevent the Building Offici,;1 frol thereafter

2. Work will beQinwithin approximately after credit is approved if any amount of this contract
is to be financed. Otherwise, work will begin within approximately of signing this contract. Work will be
completed within approximately working days. Contractor will not be liable for delays beyond his control.
3. It is agreed that this contract shall not become bindinQ upon the Contractor until it is approved by an ollicer of the
company.
4. Any representations or agreements not contained herein shall not be binding on either party. All 01 the agreements and
understandings 01 each party are contained herein.
5. In the event it becomes necessary for the Contractor to employ an attorney to recover monies due under this agreement, or to
enforce any of the terms and provisions hereof, Owner(s) wiil be liable for, and agree(s) to pay, all costs and expenses, including
reasonable attorney's fees, necessarily incurred by Contractor as a result, regardless of whether suit is filed or not.
6. All unused materials shall. under any conditions, remain the property 01 the Contractor.
7. The undersigned warrant(s) that he is (they are) the owner(s) 01 the above mentioned premises and the legal title thereto
stands of record in his (their) name(s).
8. "BUYER'S RIGHT TO CANCEL": This is a home solicitation sale, and if you do not want the goods or services, you may
cancel this agreement by providing written notice to the seller in person, by telegram, or by mail. This notice must indicate that
you do not want the goods or services and must be delivered or postmarked belore midnight of the third business day after you
sign this agreement. If you cancel thiS agreement, the seller may not keep all or part of any cash down payment.
9. In the event Owner attempts to cancel this agreement after the date and time allowed for cancellation under paragraph a of this
agreement, Owner(s) will be liable for, and agree(s) to pay Contractor, all damages which Contractor sustains as a result of such
cancellation, including loss of profit, Contractor's cost of any materials specially fabricated, and a 15% re-stocking charge for all
materials required for the work specified above.
THE OWNER(S) HEREBY ACKN,PWLEDGE,(S) RECEIPT OF A COMPLETELY EXECUTED COPY OF THIS CONTRACT.
