REINSPECTION FEES: When extra inspection trips are necessary due to anyone of the following reasons, a
charge of Fifteen and 00/100 Dollars ($15.001 shall be made for each trip for each trade:
a. Wrong Address
b. Condemned work resulting from faulty construction.
c. Repairs or corrections not made when inspection called.
d. Work not ready for inspection when called.
e. Permit not posted on job site.
f. Plans not at job site.
g. Work not accessible.
The payment of inspection fees shall be made before any further permits will be issued to the person owning
same.

you are indicating that you, rather than the contractor, are responsible for the work. If the contractor wishes you to si;n
as contractor that lay be an indication that he is not properly licensed and is not entitled to perlitting privileges in the
City of Zephyrhills.

A perlit issued shall be construed to be a license to proceed with the Mork and not as authority to violate, cancel alter, cr

THIS AGREEMENT is made and entered into the date above written by and
between GUDE BROTHERS CONSTRUCTION COMPANY, the Contractor (CONTRACTOR), and the
above-named owner(s) (OWNER), wherein IT IS MUTUALLY UNDERSTOOD, STIPULATED,
COVENANTED AND AGREED BY AND BETWEEN THE PARTIES HERETO AS FOLLOWS:
1. Description of Work: The CONTRACTOR shall perform the following described
work in accordance with the contract, plans and specifications, herein
called "Contract Documents", at the above-described job location. SUMMARY
OF WORK TO BE PERFORMED: As per the attached Proposal dated July 6, 1994,
including alternate.
2. Agreed Payment to Contractor: The OWNER agrees to pay the CONTRACTOR for
the work described the total payment or contract sum of:
Ten thousand, thirty and no/100s DOLLARS ($10,030.00).
The OWNER shall make progress payments on the account of the contract sum
to the CONTRACTOR on the basis of applications for payment submitted to
the OWNER or the OWNER'S Architect by the CONTRACTOR as the work
progresses. The OWNER agrees to pay to the CONTRACTOR the fOllowing
percentages of the contract sum upon completion of the following phases of
construction: 90% upon Substantial Completion, 10% upon Final Completion
Payment due within 10 days of invoice
3. Contract Period: The CONTRACTOR shall commence work under this contract
within fifteen (15) days of the issuance of the building permit, and
CONTRACTOR shall work diligently to complete the work as soon as possible,
consistent with principles of good workmanship and with no unnecessary
delays other than those caused by the weather and acts of God.
4. Contractor's Affidavit: Upon completion of this project, the CONTRACTOR
shall furnish to the OWNER a Contractor's Affidavit affirming that all
labor, materials and equipment for the project and all subcontractors have
been paid in full. CONTRACTOR will further furnish a complete release of
all liens arising out of the contract, or in the alternative, a bond
satisfactory to the OWNER indemnifying the OWNER against such liens.

5. Duties and Rights of Owner: OWNER shall have the right to expect all work
to be performed in a workmanlike fashion, consistent with present industry
standards. OWNER agrees to communicate directly with the CONTRACTOR, and
not with subcontractors or laborers at the work site. OWNER agrees to pay
for all extra work not specifically set forth in the "Contract Documents".
6. Duties and Rights of Contractor: CONTRACTOR shall be solely responsible
for the supervision of construction, including the techniques, sequences,
procedures and means, and for coordination of all work. The CONTRACTOR
shall be responsible for maintaining discipline among his employees and
subcontractors. The CONTRACTOR shall provide and pay for all labor,
ma terials and equipment, including tools, construction 4~quipment and
machinery. In addition, CONTRACTOR agrees to pay for all utilities,
including water, transportation, and all other facilities and services
necessary for the proper completion of work on the project in accordance
with the "Contract Documents". In addition, the CONTRACTOR shall comply
with all laws and ordinances, and the rules, regulations or orders of all
public authorities relating to the performance of the work. During the
course of construction, the CONTRACTOR agrees to keep the workplace and
adjoining ways free of waste material and rubbish caused by the
CONTRACTOR'S work, and further agrees to remove all such waste material
and rubbish upon completion of the job.
7. Work Changes: The OWNER reserves the right to order work changes in the
nature of additions, deletions or modifications without invalidating the
contract, and agrees to make corresponding adjustments in the contract
price. All changes will be authorized by a written change order signed by
the OWNER and the CONTRACTOR. All change order shall provide for payment
within ten (10) days of performance of the work, unless otherwise agreed
in writing.
8. Insurance: The CONTRACTOR agrees to keep in force at CONTRACTOR'S own
expense during the entire period of construction on the project, workmens
compensation insurance to cover all employees of the CONTRACTOR while they
are working on the job. In addition, CONTRACTOR shall keep and maintain
in force a general liability insurance policy which will protect the
CONTRACTOR and the OWNER from any liability as a result of any negligent
acts or omissions of the CONTRACTOR, it's agents and employees. Proof of
such insurance shall be furnished by the CONTRACTOR to the OWNER upon
request by the OWNER. The OWNER agrees to maintain at OWNER'S expense
during construction of the project property damage insurance on the work
at the site to it's full insurable value, including interest of the OWNER,
CONTRACTOR and subcontractors, against fire, vandalism or other perils
ordinarily included in extended coverage. Losses under such insurance will
be adjusted with and made payable to the OWNER as Trustee for the parties
insured as their interest appear. The OWNER shall furnish to the
CONTRACTOR a copy of such insurance policy upon request.

9. Arbitration: All claims and disputes relating to this contract or arising
out of this contract shall be subject to arbitration in accordance with
the arbitration rules of the American Arbitration Association for the
construction industry then obtaining. Written notice of demand for
arbitration shall be filed with the other party to the contract and with
the American Arbitration Association, within a reasonable time after the
dispute has arisen. The arbitration decision shall be final and binding
upon both parties to this contract.
10. Venue and Litiqation: The laws of the State of Florida shall govern all
matters arising out of or related to this contract. Venue for any cause
of action arising out of or related to this contract shall be in Pasco
County, Florida. In the event of any litigation arising out of this
contract or in any way pertaining to this contract, the prevailing party
shall be entitled to recover against the other party a reasonable
attorneys fee, together with costs of any litigation.
IN WITNESS WHEREOF, the parties have executed this agreement in Pasco
County, Florida, on the date last above written.

2. Install new drywall and paint to all walls in Dr. Jacobs office.
3. Re-work ceiling as needed throughout building.
4. Install new ceiling grid and tile in group rooms.
5. Fill in hole from bathroom to kitchen.
6. Fill in door from medical records to nurse's station.
7. Patch and paint walls as needed throughout building.
8. Re-arrange existing lights and add two new lights in group rooms. No new
switches added.
9. Remove existing wall sinks and cut plumbing to below slab or in walls as
needed.
10. Remove some interior walls as discussed.
11. Add two dutch doors at nurse's station.
Does not include any of the following: Flooring of any kind, cabinets or cabinet
removal, air conditioning, engineering or architect fees, impact fees (permits
are included), any items required by governmental agencies not listed above,

machinery. In addition, CONTRACTOR agrees to pay for all utilities,
including water, transportation, and all other facilities and services
necessary for the proper completion of work on the project in accordance
with the "Contract Documents". In addition, the CONTRACTOR shall comply
with all laws and ordinances, and the rules, regulations or orders of all
public authorities relating to the performance of the work. During the
course of construction, the CONTRACTOR agrees to keep the workplace and
adjoining ways free of waste material and rubbish caused by the
CONTRACTOR'S work, and further agrees to remove all such waste material
and rubbish upon completion of the job.
7. Work Chanqes: The OWNER reserves the right to order work changes in the
nature of additions, deletions or modifications without invalidating the
contract, and agrees to make corresponding adjustments in the contract
price. All changes will be authorized by a written change order signed by
the OWNER and the CONTRACTOR. All change order shall provide for payment
within ten (10) days of performance of the work, unless otherwise agreed
in writing.
8. Insurance: The CONTRACTOR agrees to keep in force at CONTRACTOR'S own
expense during the entire period of construction on the project, workmens
compensation insurance to cover all employees of the CONTRACTOR while they
are working on the job. In addition, CONTRACTOR shall keep and maintain
in force a general liability insurance policy which will protect the
CONTRACTOR and the OWNER from any liability as a result of any negligent
acts or omissions of the CONTRACTOR, it's agents and employees. Proof of
such insurance shall be furnished by the CONTRACTOR to the OWNER upon
request by the OWNER. The OWNER agrees to maintain at OWNER'S expense
during construction of the project property damage insurance on the work
at the site to it's full insurable value, including interest of the OWNER,
CONTRACTOR and subcohttactots~ against fire, vandalism or other perils
ordinarily included In extended coverage. Losses under such insurance will
be adjusted with and made payable to the OWNER as Trustee for the parties
insured as their interest appear. The OWNER shall furnish to the
CONTRACTOR a copy of such insurance policy upon request.

9. Arbitration: All claims and disputes relating to this contract or arising
out of this contract shall be subject to arbitration in accordance with
the arbitration rules of the American Arbitration Association for the
construction industry then obtaining. Written notice of demand for
arbitration shall be filed with the other party to the contract and with
the American Arbitration Association, within a reasonable time after the
dispute has arisen. The arbitration decision shall be final and binding
upon both parties to this contract.
10. Venue and Litigation: The laws of the State of Florida shall govern all
matters arising out of or related to this contract. Venue for any cause
of action arising out of or related to this contract shall be in Pasco
County, Florida. In the event of any litigation arising out of this
contract or in any way pertaining to this contract, the prevailing party
shall be entitled to recover against the other party a reasonable
attorneys fee, together with costs of any litigation.
