A perlit issued shall be construed to be a license to proceed with the work and not as authority to violate, cancel alter, or
set aside any provisions of the technical codes, nor shall issuance of a perlit prevent the Building Official frOJ thereafter
requiring a correction of errors in plans, construction, or violations of any code. Every perlit issued shall becOle invalid
unless the work authorized by such per.it is cOllenced within six IOnths of issuance, or if work authorized by the perlit is

STANDARD FORM OF AGREEMENT
BETWEEN CONTRACTOR AND OWNER
THIS AGREEMENT made the 5th day of July ;1"} the year
Nineteen Hundred anrl ninety-four by and between J. C . Construction
hereinafter called the Contractor, anrl Kennard Gibbons hereinafter called the
Owner, Witnesseth, that the Contractor and the Owner for the considerations named agree as follows:
ARTICLE 1. SCOPE OF THE WORK
The Contractor shall furnish all of the materials and perform all of the work shown on the Drawings and described in the Specifications
entitled blueprints
prepared by Towson-Rogers Eng. , acting as and in these
Contract Documents entitled the Architect, and shall do everything required by this agreement, the general conditions of the Contract, the
Specifications and the Drawings.
ARTICLE 2. TIME OF COMPLETION
The work to be performed under this Contract shall be commencerl wi thin ten days of permit And shall be
substantially completed within 120 days of permit date. If draw payments are not made
in a timely manner, construction will be delayed.
ARTICLE 3. THE CONTRACT SUM
The Owner shall pay the Contractor for the performance of the Con tract, subject to additions and deductions provided therein, in current
funds as follows: $34,500.00 (Thirty-four thousand, five hundred dollars and nojlOO-)
(State here the lump sum amount, unit prices, or both, 81 desired)
Authorized Change Orders will be submitted by the Contractor for all work not specifically included in this Contract.

ARTICLE 5. ACCEPTANCE AND FINAL PAYMENT

ARTICLE 6. THE CONTRACT DOCUMENTS
The General Conditions of the Contrac~, the Specifications and the Drawings, together with this Agreement, form the Contract, and they

mformation:
Exterior walls painted only. Interior walls around bathrooms metal stuc
with sheetrock with one coat of paint. Standard white fixtures with
one handicap toilet in each bathroom. No wall tile or floor covering
in building. Ceiling tile 9 x 9. Roofing - 20 yr. fiberglass shingles.
All impact fees are to be paid by the owner. Building p~rmit will be
paid by the builder. No air conditioning work is included in this
contract.

The Southwest Florida Water Management District is responsible for
protecting the water resource and its related environment for the citizens
of the District. The District Governing Board has adopted permitting
requirements designed to conserve water resources, preserve water quality,
protect wetlands and reduce flooding. -'.
We have received and reviewed your submittal for the project referenced
above. Pursuant to Chapter 40D-4, Florida Administrative Code, (F,A.C.),
a permit will not be required for the proposed building addition (for
reference see Chapter 400-4.021(7), F.A.C., and Chapter 400-4.041(1),

Plans and information submitted will be kept on file and referenced in
support of this opinion.
