Tim,
Per your instructions I inspected on 8/26/94 the masonry wall
damage to the above referenced facilty.
We have attached sketches documenting how the repairs should be
undertaken. If conditions encountered in the field are different
than shown, our office should be contacted for a reinspection.
We will leave the undamaged elements in position and reconstruct
the damaged elements to meet current code requirements. Any cracked
masonry wall areas should be filled solid by inducing a hole at the
top of the cell in the cracked areas and filling the cell with
concrete.
If any cracked areas or other damage is encountered that is not
reflected on these drawings, our office should be notified prior to
proceeding.
If there are any questions please let me know.

Provide standard hooks at the ends of all column and filled
cell reinforcing. Provide corner bars for all bars at all
corners.
7. FORMS:
Construct formwork for concrete with plywood or other
acceptable panel type materials to provide continuous,
straight smooth surfaces. Coat forms with a commercial
formulation coating compound that will not bond, stain, or
adversely affect concrete surfaces or impair subsequent
treatments. Design land erect formwork to support all loads.
Thoroughly clean and adjust all forms just prior to concrete
placement. Use factory prefabricated snap off or removable
ties.
o. EXECUTION:
a. Support reinforcement on metal bolsters. Masonry may not

b. Formwork supporting concrete beams shall be left in place
for at least 14 days and until the concrete has reached
it's design strength.
c. Consolidate concrete with mechanical vibrat-ing equipment.
d. All slab surfaces are to be coated with liquid cure and
seal compound unless this would affect finishes to be
applied later. In the case of the latter wet cure the
slab. Comply with ACI guidelines for acceptable
protection and curing practices for under all conditions.
e, Provide 1 1/2" keyways at all construction ~I oints .
f. Formwork not supporting the weight of concrete may be
removed after 24 hrs.
g. Place concrete in forms in horizontal layers not
exceeding 18" in depth so as to avoid inclined joints.
h. Wet all forms prior to placing concrete.
i. Provide moisture barrier over prepared base where
indicated.
j. Roughen all existing dried concrete surfaces and coat
with bonding agent prior to placing wet concrete against
the dry surface.
k. Concrete shall not free fall more than two feet into
forms.
