charge of Fifteen and 00/100 Dollars ($15.00) shall be made for each trip for each trade:
a. Wrong Address
b. Condemned work resulting from faulty construction.
c. Repairs or corrections not made when inspection called.
d. Work not ready for inspection when called.
e. Permit not posted on job site.
f. Plans not at job site.
g. Work not accessible.
The payment of inspection fees shall be made before any further permits will be issued to the person owning
same.

A. NOTICE OF DEED RESTRICTIONS
The undersigned understands that this pertit lay be subject to .deed restrictions" which lay be lOre restrictive than City
regulations. The undersigned assUles responsibility for COIpliance with any applicable deed restrictions.
B. UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES
If the owner bas hired a contractor or contractors to undertake work, they lay be required to be licensed in accordance with
state and local regulations. If the contractor is not licensed as required by law, both the owner and contractor lilY be
cited for a lisdeJeanor violation under state law. If the owner or intended contractor are uncertain as to what licensing
requirlllents lay apply for the intended work, they are advised to contact the City of Zephyrhills Building DepartJent, (813)
788-6611.
FurtherlOre, if the owner has hired a contractor or contractors, he is advised to have the contractor(s) sign portions of the
"Contractor Sections" of this application for which they will be responsible. If you, as the owner sign as the contractor,
you are indicating that you, rather than the contractor, are responsible for the work. If the contractor wishes you to sign
as contractor that lilY be an indication that he is not properly licensed and is not entitled to pertitting privileges in the
City of Zepbyrbills.

addressing a uCOlpensating volUleu will be sublitted which is prepared by a professional engineer registered in the State of
Florida prior to perlit issuance.
A pertit issued sball be construed to be a license to proceed with tbe work and not as authority to violate, cancel alter, or
set aside any provisions of the technical codes, nor shall issuance of a pertit prevent the Building Official frol thereafter
requiring a correction of errors in plans, construction, or violations of any code. Every perlit issued shall beCOle invalid
unless the work authorized by such pertit iscOlleDced within six IOnths of issuance, or if work authorized by the pertit is
suspended or abandoned for a period of six IOnths after the tile the work is coaenced. One 90 day extension of tile, lilY be
allowed for tbe peIlit with fee charge of $15.00. Tbe extension shall be requested in writing to the Building Official. An
approved inspection lUst be logged during eacb six IOnth period, or the project will be considered abandoned.

DRIVEWAY CONSTRUCTION SPECIFICATIONS
1. DRIVEWAYS SHALL FOLLOW 'THE CONTOUR FROM CENTER OF STREET TO
BOTTOM OF SWALE.
2. EXCAVATION SHALL BE SIX (6) INCHES FROM SQUARE CUT STREET EDGE
TO BOTTOM OF SWALE, THEN TAPER TO FOUR (4) INCHES AT PROPERTY
LINE.
3. ALL VEGETATION SHALL BE REMOVED, RAKED, LEVELED AND TAMPED
UNDER TOTAL AREA TO BE PAVED.
4. WIRE MESH IS REQUIRED FROM STREET TO PROPERTY LINE, AND
FASTENED TOGETHER WITH TIES EVER FOUR (4) FEET.

RIGHT-OF-WAY USE PERMIT

No construction under ground or above ground of any facility shall be accomplished on any
City Rights of Way without written approval from the City Building Department. Any application
for approval of cQnstruction operations on any City owned Rights of Way shall furnish a detailed
.drawing showing the complete scope of the proposed work in triplicate to the office of the Build-
ing Official for approval prior to the commencement of any work. A copy of this permit is to be
kept readily available at the site of the work at all times. :

1. The construction and maintenance of such utility shall not interfere with be property
and rights of a prior occ~pant.
2. All work shall be done in keeping with the standards of the City Streets Department and
under the supervision of the City Building Official, who will be notified at least one day prior
to commencement of construction.
3. All materials and 'eq~ipment shall be subject to inspection by the City Building Official.
4. During construction all safety regulations of the State Road Department shall be observed
and tqe City shall be relieved of all responsibility from damage of any nature arising from this
permit: Subject to the same terms and conditions, the permit holder may take such safety measures,
inclUding placing and display of caution signs, as it may deem necessary in conduct of construction
and maintenance work hereunder.
5. All private and all City property shall be restored to its original condition as far as
practical in the opinion of the City BUilding Official.
6~ All underground CI:lCIssing installations shall be laid at su~h depth as may be specified
by the City Building Official.
7. The sketch covering the details of this installation shall be made a part of this permit.
This permit is granted with the understanding that the applicant has notified all other utility
users in the area covered by the permit and takes full responsibility for any damage incurred by
prior installations as a result of his operations in order that they may safe guard their interests.
B. It is expressly stipulated that this permit is a license for permissive use only and
that the placing of ~~~ilit:ies upon public property pursuant to this permit shall not operate to
'create or to vest any property right in said holder.
9. Whenever the City decides to further exploit the City Rights of Way, any or all of said
poles, ~ires, pip.s, cables, or other facilities and appurtenances authorized hereunder, shall be
immediately removed from said Rights of Way, or reset or relocated thereon as required by the City
BUilding Official at the expense of the holder of this permit.
10. The holder shall save and keep the City harmless from any and all damages, Claims, or
injuries that may occur bt..reason of the construction, maintenance, and operation of said facility.
11. . The holder ahall complete the requested work within days or this permit shall
become null and void. In the event the work requested is not completed upon the expiration date
of this permit, the City shall have the right to complete such work and to charge the holder of
the pe~t for all casts incurred in completi~g said work.
. 12. The holder shall post a cash or surety bond with ~he City Manager in the sum of $
dollars to guarantee performance of the obligations herein, and to guarantee maintenance of the
right-of-way d~scribed,herein for a period of one (1) year following completion of the requested
,installation. In the event a surety bond is posted, the said surety bo~d shall be made payable to
the City and shall obligate the surety to hold the City harmless in the event the holder of this
pe~t should fail to. meet any of its obligations hereunder. The bond shall also indemnify the
City for all court costs and reasonabl~ attorneys fees in the event legal action is required to
collect on said bond.
