REINSPECTION FEES: When extra inspection trips are necessary due to anyone of the following reasons, a
charge of Fifteen and 00/100 Dollars ($15.00) shall be made for each trip for each trade:
a. Wrong Address
b. Condemned work resulting from faulty construction.
c. Repairs or corrections not made when inspection called.
d. Work not ready for inspection when called.
e. Permit not posted on job site.
f. Plans not at job site.
g. Work not accessible.
The payment of inspection fees shall be made before any further permits will be issued to the person owning
same.

B. UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES
If the owner bas hired a contractor or contractors to undertake work, they lay be required to be licensed in accordance with
state and local regulations. If tbe contractor is not licensed as required by law, both the owner and contractor lay be
cited for a lisdeleanor violation under state law. If the owner or intended contractor are uncertain as to wbat licensing
requireJents lay apply for the intended work, they are advised to contact the City of Zepbyrbills Building DepartJent, (813)
788-6611.
FurtherlOre, if tbe owner bas bired a contractor or contractors, be is advised to bave the contractor(s) sign portions of the
"Contractor Sections" of this application for wbicb they will be responsible. If you, as the owner sign as the contractor,
you are indicating that you, rather than tbe contractor, are responsible for the work. If the contractor wishes you to sign
as contractor that lay be an indication that he is not properly licensed and is not entitled to perlitting privileges in the
City of Zepbyrbills.

A perlit issued sball be construed to be a license to proceed with the work and not as authority to violate, cancel alter, or
set aside any provisions of the technical codes, nor shall issuance of a perlit prevent the Building Official frOJ thereafter
requiring a correction of errors in plans, construction, or violations of any code. Every perlit issued shall beCOle invalid
unless the work authorized by such perlit is cOllenced within six IOnths of issuance, or if work authorized by the perlit is
suspended or abandoned for a period of six IOntbs after the tile the work is cOllenced. One 90 day extension of tile, JaY be
allowed for the perlit with fee charge of $15.00. Ybe extension sball be requested in writing to the Building Official. An
approved inspection lUst be logged during eacb six IOnth period, or the project will be considered abandoned.

Subject: The Old Mayer's House

Dear Messrs:
On this day, an on-site field inspection was made by me or under my direction for the
purpose of determining the structural condition of the subject building.
There has been removed a section of wall approximately 6 feet in length due to termite
damage. It appears that all of the damaged wall has been removed and is ready for a
replacement wall to be constructed.
The existing foundation consists of piers and spread footings. On the East and West sides of
the building there has been some settlement of approximately 1" to 1-1/2", this settlement is
mainly noticeable due to the relative stability of the building center. There eltists a large
spread footing for a fireplace at the center of the building which settled at a lessor rate than
the perimeter of the building.
The remainder of the building was inspected and found to be in acceptable condition.
It is my professional determination that the subject building is in sound condition and does
retain its structural integrity and is suitable for occupation.
