o If fill material is to be used in Flood Zone "A" in connection wilh a permitted building using stem wall construction, I certify that fill will be used only

If I am the AGENT FOR THE OWNER, I promise in good faith to infonn the owner of the pennitting conditions set forth in this affidavit prior to commencing
construction.
I understand that a separate penn it may be required for electrical work, plumbing, signs, wells, pools, air conditioning, gas, or other installations not
specifically included in the application.
A permil issued shall be consll'ued to be a license to proceed wilh the work and nol as authority 10 violate, cancel, alter, or set aside any provision of
Ihe l8d1nicaJ codes, nor shall issuance of a permit prevent the Building Official from thereatl8r requiring a correction 01 errors In plans, construction, or
violations of any code. Every permil issued shall become invalid unless Ihe work aulhorized by such permills commenced wilhin six months of permit
issuance, or if work aulhorized by the permit Is suspended or abandoned for a period of six monlhs after Ihe time the work is commenced. One or
more extensions of time, for periods not io exceed 90 days each, may be allowed for Ihe permit The extension shall be requested In writing to Ihe
Building Official. An approved inspection must be logged during each slx-monlh period, or the project will be considered abandoned.
