The undersigned understands that this permit may be subject to "deed restrictions" which
may be more restrictive than City regulations. The undersigned assumes responsibility for
compliance with any applicable deed restrictions.
B. UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES
If the owner has hired a contractor or contractors to undertake work, they may be required
to be licensed in accordance with state and local regulations. If the contractor is not
licensed as required by law, both the owner and contractor may be cited for a misdemeanor
violation under state law. If the owner or intended contractor are uncertain as to what
licensing requir:ements may apply [or: the intended wor:k, they are advised to contact the
City of Zephyrhills Building Department, 813-788-6611.
Furthermore, if the owner has hired a contractor or contractors, he is advised to have the
contractor(s) sign portions of the "Contractor Sections" of this application for which they
will be responsible. If you, as the owner signs as the contractor, you are indicating that
you, rather than the contractor, are responsible for the work. If the contractor wishes
you to sign as contractor that may be an indication that he is not properly licensed and is
not entitled to permitting privileges in the City of Zephyrhills.

I certify that I, the applicant, have been provided with a copy of "Florida's Construction
lien Law - Homeowner's Protection Guide" prepared by the Florida Department of Agriculture
and Consumer Affairs. If the applicant is someone other that the "owner", I cerify that I
have obtained a copy of the above described document and promise in good faith to deliver
it to the "owner" prior to corrunencement.

I certify that all the information in this application is accurate and that all work will
be done in compliance with all applicable laws regulating construction, zoning, and land
development.
Application is hereby made to obtain a permit to do work and installation as indicated. I
certify that no work or installation has commenced prior to issuance of a permit and that
all work will be performed to meet standards of all laws regulating construction, City
codes, zoning regulations, and land development regulations in the jurisdiction. I also
certify that I understand that the regulations of other governmental agencies may apply to
the intended work, and that it is my responsibility to identify what actions I must take to
be in compliance. Such agencies include but are not limited to: *Department of
Environmental Regulation-Cypress Bayheads, Wetland Areas and Environmentally Sensitive
Lands, Water/Wastewater Treatment
*Southwest Florida Water Management District-Wells, Cypress Bayheads, Wetland Areas,
Altering Watercourses
*Army Corps of Engineers-Seawalls, Docks, Navigable Waterways
*Department of Health & Rehabilitative Services, Environmental Health Unit-Wells,
Wastewater Treatment, Septic Tanks
*U.S. Environmental Protection Agency-Asbestos abatement
I also certify that, if fill material is to be used in Flood Zone "A" or "A, etc.", it is
understood that a drainage plan addressing a "compensating volume" will be submitted which
is prepared by a professional engineer registered in the State of Florida prior to permit
issuance.
A permit issued shall be construed to be a license to proceed with the work and not as
authority to violate, cancel, alter, or set aside any provisions of the technical codes,
nor shall issuance of a permit prevent the Building Official from thereafter requiring a
correction of errors in plans, construction, or violations of any code. Every permit
issued shall become invalid unless the work authorized by such permit is commenced within
six months of issuance, or if work authorized by the permit is suspended or abandoned for a
period of six months after the time the work is cormnenced. One 90 day extension of time
may be allowed for the permit with fee charge of $15.00. The extension shall be requested
in writing to the Building Official. An approved inspection must be logged during each six
month period, or the project will be considered abandoned.

Dear Mr. Bridges:
The District has reviewed information received on August 26, 1998, for the project referenced above
to determine if the activities proposed are regulated by District rules. Based on the information you
provided and the statutes and rules in effect at the time of this determination, the District has
concluded that an Environmental Resource Permit win not be required for the proposed hangar
addition. Plans and information submitted will be kept on file in the Brooksville Service Office in
support of this decision.
This notification to you does not mean that the District has determined that your project is
permanently exempt from permitting requirements nor does it relieve you of the responsibility of
obtaining other permits or approvals (federal, state or local) that may be required for the project.
This determination may not be valid in the event future changes occur in the applicable statutes or
rules. Additionally, project modifications may necessitate further evaluation or permitting by the
District. Therefore, you are advised to contact the District before beginning any work which is not
specifically described in your submittal.
You or any person whose substantial interests are affected by the District's action regarding your
application may request an administrative hearing in accordance with Sections 120.569 and 120.57,
F .S., and Chapter 28-106, F .A.C., of the Uniform Rules of Procedure. A request for hearing must
(l) explain how each petitioner's substantial interests will be affected by the District's action. or
proposed action; (2) state all material facts disputed by the petitioner or state that there are no
disputed facts; and (3) otherwise comply with Chapter t8-i06. F.A.C.. A request for hearing must
be filed with and received by the Agency Clerk of the District at the District's Brooksville address
within 21 days of receipt of this notice. Receipt is deemed to be the fifth day after the date on
which this notice is deposited in the United States mail. Failure to file a request for hearing within
this time period shall constitute a waiver of any right such person may have to request a hearing
under Sections 120.569 and 120.57, F.S.
Mediation pursuant to Section 120.573, F.S. to settle an administrative dispute regarding the
District's action in this matter is not available prior to the filing of a request for hearing.

1 . All calculations for properties of panels are cakulated in accordance with
Specifications for the Design of Light Gauge Cold Fanned Steel Structurat
Members published by the American Iran and Sleet Institute (AlSJ) and
guidelines eSlablished by the Metal Building Component Manufacturers
Association (MBCMA). Wind loads and deRection loads are limited by a
deRection ratio of L/180.
2, Values shown as allowable loads are based on ponels COYering three (3) or
more equal continuous spans, Multiply values shown by 0.8 for two (2)
span condition,
3. Allowable loads (deRection) for wind shown have been increased by 331fJ'X,.
4. For wind load stress only, multiply live load (stress) by 133.
5. Weight of panels must be deducted from values to ablain net allowable load.

SuBSTRATES:
The most popular substrates in the metal building industry are
GAlVAlUME and galYanized. Both il1YOlve CXlCIfing \he base melal in
a hot-dip process. Galvanized substrate utilizes zinc as a p-cIec:INe
material, while GAlVALUME utilizes an allay cJ zinc and aluminum.
SUBSTRUCTURAL COMPONENTS:
McElroy manufactures a complete line of CEES, lEES, EAVE
STRUTS, ANGLES, and SUB-GIRTS. For more information,

McElROY PAINT SYSTEMS:
Standard colors for Mega-Rib utilize a Kynar 500 paint sys1em.
Mega-Rib is also oycilable with siliconized paIyester and Plastisol
(PVC) systems. Minimum order quantities may apply. Contact a
McElroy Representative for oycilability.
ME&A-RIB TRIM I ACCESSORIES:
Mega-Rib Trim is produced on state-of-the-art, computerized
equipment. Like all McElroy products, it is oycilable in a wide
assortment of attractive colors and the proven performance of

(Wall Sheeting) (Liner) (Fascia) (Soffit) panel as manufactured by
McElroy M.elal, Inc. 1500 Hamilton ReI., Bossier Oty, LA 71111,
800-950-6531. The CX)Iltroc:lor shan furnish all maIeriaI, labor, and
equipment to execute the wen in a timely & satisfaclory manner in
ac:oordanc:e with plans and specifications.

2. MATERIALS AND COATlNas: Base melal shall be (29) (26) (24)
(22) Gauge GAl.VAlUME steel produced in ac:oordanc:e with /!SfM
A-792 /1.Z55 or (20) (18) Gauge, G-9O Galvanized steel producsd
in accordance with ASTM A-AA6. Grade selected from
manufac:turel's standard.
I. FINISHES: The finish coating shall be (bare GALVALUME)
(bare Galvanized) (McElroy Premium Kynar 500) (McElroy
siliconized polyester) (McElroy Plastisol). Color to be selected from
manufac:turel's standard.
4. PANELS I FLASHIN&: Panels to be roll formed wilh 1112" rib
height, Z2" rib spacing, and 36" c:overage. Panels may be IappecI
horizontally as required. Maximum panel length is AO feet. Panel
lexIure shall be smaoIh. All Bashing to be manufactured from 26
gauge malerial. Color selected from manufac:turel's standard.
5. SEALANn I CLOSURES: Closures shall be pre-moIded, dosed
cell poIye!hylene foam to match the panel CX)Il~guration. Closures
located per manufac:turer's recommendations, When panels are used
as Roof Sheeting, butyl rubber tape sealant shall be applied at side
laps and end laps to assure weathertightness per manu'
focturel's recommes ldations.

and panel joints shall be caulked where required to prevent
