charge of Fifteen and 00/100 Dollars ($ 25.00) shall be made for each trip for each trade:
a. Wrong Address
b. Condemned work resulting from faulty construction.
c. Repairs or corrections not made when inspection called,
d. Work not ready for inspection when called.
e. Permit not posted on job site.
f. Plans not at job site.
g. Work not accessible,
The payment of inspection fees shall be made before any further permits will be issued to the person owning
same.

CONDITIONS OF PERMIT AFFIDAVIT
A. NOTICE OF DEED RESTRICTIONS
The undersigned understands that this permit may be subject to "deed restrictions" which
may be more restrictive than City regulations. The undersigned assumes responsibility for
compliance with any applicable deed restrictions.
B. UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES
If the owner has hired a contractor or contractors to undertake work, they ~Y; be requ~r~d
to be licensed in accordance with state and local regulations. If the contractor is not'
licensed as required by law, both the owner and contractor may be cited for a misdemea~or
violation under state law. If the owner or intended contractor are uncertain as'to'what'
licensing requirements may apply for the intended work, they are advised to contact the
City of Zephyrhills Building Department, 813-788-6611.
Furthermore, if the owner has hired a contractor or contractors, he is advised t9 have,tl1e
contractor(s) sign portions of the "Contractor Sections" of this application for which they
will be responsible. If you, as the owner signs as the contractor, you are indicia,ti!ng\.t:9:~t
you, rather than the contractor, are responsible for the work. If the contractor wishes
you to sign as contractor that may be an indication that he is not properly licensed and is
not entitled to permitting privileges in the City of Zephyrhills.

lien Law - Homeowner's Protection Guide" prepared by the Florida Department of Agriculture
and Consumer Affairs. If the applicant is someone other that the "owner", I cerify that I
have obtained a copy of the above described document and promise in good faith to deliver
it to the "owner" prior to commencement.

understood that a drainage plan addressing a "compensating volume" will be submitted which
is prepared by a professional engineer registered in the State of Florida priorto,'permit
issuance.
A permit issued shall be construed to be a license to proceed with the work and not ~ as.i
authority to violate, cancel, alter, or set aside any provisions of the technical codes,
, nor shall issuance of a permit prevent the Building Official from thereafter requiring a
correction of errors in plans, construction, or violations of any code.Every~i'p~~';t""rf~'r.;;'l
issued shall become invalid unless the work authorized by such permit is commenced within
six months of issuance, or if work authorized by the permit is suspended or abandoned for a
period of six months after the time the work is commenced. One 90 day extension of~tim~t
may be allowed for the permit with fee charge of $15.00. The extension shall be reques~ed ;
in writing to the Building Official. An approved inspection must be logged during each' six
month period, or the project will be considered abandoned.

We hereby propose to furnish materials and labor necessary for the completion of:
1. For the shingled portions of the home, remove old roofing materials to wood, taking precautions to protect
the building and the landscaping. Groom the deck and reset existing decking nails.
2. Replace bad wood other than herein agreed for at $36.00 per man-hour plus materials marked up a 25%
contractors fee,
3. Install _6" white eaves drip with all edges sealed with plastic cement.
4. Install 2 layers of ASTM 15 lb. asphalt shingle underlaymenL
5. Install galvanized valley metal for the length of all valleys. First side shingles will extend over the center of
the valley a minimum of 12" The second side shingles shall be installed and cut parallel with the center of the
valley and approximately 2" back from the center of the valley and the valley points shall be cut off.
6. Install new lead boots over vent piped and replace metal vents with new.
7. Chalk lines shall be struck to assure proper shingle exposure. Insta1l3~ year Royal Soverien Class A
self sealing fungus resistant fiberglass shingles. Six 11/4" corrosion resistant nails sha I be installed pJr

9. Hurricane nail the deck to the rafters option to meet Current SBCCI Code. See pricing section.

CONTRACTOR WARRANTY ,
UPON COMPLETION OF THE WORK. AND PAYMENT OF ALL MOINES OWED. CONTRACTOR
SHALL ISSUE A WRITTEN FIVE (5) YEAR WARRANTY FOR WORKMANSHIPS LIMITED TO LEAKS
CAUSED BY ANY COMPONENT INSTALL BY THE CONTRACTOR, SHINGLE MANUFACTURER
SHALL PROVIDE A cJ~ YEAR LIMITED WARRANTY.

Deck Over wind turbines, four sheets of plywood 16' 2 x 4 6' 1 x 6 6' 1 x 4 6' white alc soffit. 6 Man-hours of
labor has been allowed for wood repair.

GENERAl, CONDITION
All work shall be carefully supervised and completed by worlanen skilled and knowledgeable in methods
needed to produce high quality work. The job site shall be kept clean daily for the duration ofthe job and the
grounds shall be left clean of all roof related debris after completion. The yard shall be swept with a magnet.
Collection costs is any, together with interest shall be added to the contract price if payment default occurs.
Cancellation of the contract after the 72 hour grace period shall incur a nominal fee. Permit, worlanan
compensation, and general liability insurance shall be provided by the contractor. Carpentry, authorized change
orders and work which is not covered under the scope of work outlined herein shall be performed on a time and
materials basis unless otherwise agreed upon,
