CONDITIONS OF PERMIT AFFIDAVIT
A. NOTICE OF DEED RESTRICTIONS
The undersigned understands that this permit may be subject to ~deed restrictions" which
'may be more restrictive than City regulations. The undersigned assumes responsibility for
compliance with any applicable deed restrictions.
B. UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES
If the owner has hired a contractor or contractors to undertake work, they may be required
to be licensed in accordance with state and local regulations. If the contractor is not
licensed as required by law, both the owner and contractor may be cited for a misdemeanor
violation under state law. If the owner or intended contractor are uncertain as to what
licensing requirements may apply for the intended work, they are advised to contact the
City of Zephyrhills Building Department, 813-786-6611.
Furthermore, if the owner has hired a contractor or contractors, he is advised to have the
contractor(s) sign portions of the ~Contractor Sections" of this application for which they
will be responsible. If you, as the owner signs as the contractor, you are indicating that
you, rather than the contractor, are responsible for the work. If the contractor wishes
you to sign as contractor that may be an indication that he is not properly licensed and is
not entitled to permitting privileges in the City of Zephyrhills.

I certify that I, the applicant, have been provided with a copy of ~Florida's Construction
lien Law - Homeowner's Protection GuideH prepared by the Florida Department of Agriculture
and Consumer Affairs. If the applicant is someone other that the ~owner", I cerify that I
have obtained a copy of the above described document and promise in good faith to deliver
it to the ~owner" prior to commencement.
E. CONTRACTOR'S!OWNER'S AFFIDAVIT
I certify that all the information in thia application is accurate and that all work will
be done in compliance with all applicable laws regulating construction, zoning, and land
development.
Application is hereby made to obtain a permit to do work and installation as indicated. I
certify that no work or installation has commenced prior to issuance of a pe~it and that
all work will be performed to meet standards of all laws regulating construction, City
codes, zoning regulations, and land development regulations in the jurisdiction. I also
certify that I understand that the regulations of other governmental agencies may apply to
the intended work, and that it is my responsibility to identify what actions I must take to
be in compliance. Such agencies include but are not limited to: *Department of
Environmental Regulation-Cypress Bayheads, Wetland Areas and Environmentally Sensitive
Lands, Water/Wastewater Treatment
*Southwest Florida Water Management District-Wells, Cypress Bayheads, Wetland Areas,
Altering Watercourses
*A~y Corps of Engineers-Seawalls, Docks, Navigable Waterways
*Department of Health & Rehabilitative SerVices, Environmental Health Unit-Wells,

I also certify that, if fill material is to be used in Flood Zone ~A" c.r ~A,etc.", it is
understood that a drainage plan addressing a ~compensating volume~ will, be submitted which
is prepared by a professional engineer registered in the State of Flori.da prior to permit
issuance.
A permit issued shall be construed to be a license to proceed with the work and not as
authority to violate, cancel, alter, or set aside any provisions of the technical codes,
nor shall issuance of a permit prevent the Building Official from thereafter requiring a
correction of errors in plans, construction, or violations of any code. Every pe~it
issued shall become invalid unless the work authorized by such permit is commenced within
six months of issuance, or if work authorized by the permit is suspended or abandoned for a
period of six months after the time the work is commenced. One 90 day extension of time
may be allowed for the permit with fee charge of $15.00. The extension shall be requested
in writing to the Building Official. An approved inspection must be logged during each six
month period, or the project will be considered abandoned.
WARNING TO OWNER: YOUR FAILURE TO RECORD A NOTICE OF COMMENCEMENT MAY RESULT IN YOUR
PAYING TWICE FOR IMPROVEMENTS TO YOUR PROPERTY. IF YOU INTEND TO OBTAIN FINANCING, CONSULT
WITH YOUR LENDER OR AN ATTORNEY BEFORE RECORDING YOUR NOTICE OF COMMENCEMENT. JOBS UNDER

Dear Sir or Madam:
It is the intention of Bank of America to do minor renovations to the existing building. The
project scope is as follows:
1. Address of building:

2. Relocate one drive thru unit and move it to an existing lane doubling this lane.
Provide a new ATM in an island. We will leave a pass thru lane as required by
zonmg.
3. Provide required lighting level meeting the levels for ATM
4. Remove and existing wall cabinet and replace it with a new one.
As evidenced by the enclosed documents, this is a small project and we are respectfully
requesting a building permit review of our drawings. The General Contractor will pull the
permit.
We have provided the following documents:
a. 5 sets of signed and sealed construction documents
b. Application for permit. The general and electrical contractor will request
building permit and electrical permits.
c. Contractor will pay for all required permits
d. A plan review fee check for $15.00

I have forward the Conditions of Permit Affidavit to the Owner for his signature. He will
forward it to the Contractor and the general contractor will deliver it to the Building
Department.
Your help in reviewing the drawings will be appreciated. The contractor will pay all required
permits and will fill any other paperwork that may be required.

COMMERCIAL BUILDING PERMIT REQUIREMENTS

1. All Site Plan Review requirements must be satisfied before peomit issuance.
2. Three (3) sets of Building Plans - professionally signed & sealed showing
Electrical, Plumbing and Mechanical layouts.
3. Applicable Florida Energy Efficiency Code Form.
4. Licensing Requirements: (al Proof of either State Certified License or County
Competency License with State Registration (b) County Occupational License (c)
Certificate of Insurance on Worker's Comp or State Exemption (d) $20.00 processing
fee for each contractor. Notarized authorization letter is required if anyone
other than license holder signs pennit or application. Property that will be
rented, leased or sold, will have to be contracted by properly licensed people as
per state law.
5. Fully executed building permit application must be submitted with construction
plans with payment of application fee of $.03 per square foot ($15 minimum); said
fee will be ~creditedH at time of permitting.
6. Sewer & Water Connection Fees, Transportation Impact Fees and Water l"Ieter Fee must
be paid prior to or at the time of permit issuance.
7. For Construction Codes see attachment Ordinances #430 & #669.
8. Annual Solid Waste Assessment Fee must be paid prior to issuance of Certificate of
Occupancy.
9. Allowance of ten (10) working days after submittal date required for plans
approval.
10.Inspection requests are scheduled for the following workday.
11. City Right-of-Way Use Permit.

. Service upgrades or sewer line replacements.
No application or contract required. Same day permitting.
· IF JOB IS UNDER $2,500 THE FOLLOWING IS REQUIRED:
(a) Copy of contract with owner & contractor's signature.
(b) If no contract - then application required-signed by both & notarized.
(c) By owner - permit only - unless subs are involved - then use application for signup.
. IF JOB IS OVER $2,499.00 THE FOLLOWING IS REQUIRED:
(al Application complete - notarized signatures of both owner & contractor and contract
wi th both signatures. Before pennitting, all subcontrators are to have signed on
application with registrations and worker's comp insurance must be current. Must have
certified copy of notice of commencement.
(b) By owner - same route as (a) exclUding notice of commencement.

same.
· Time element involving permitting - some permits can be i:ssued over counter same day as

· COMMERCIAL NEW CONSTRUCTION: MINIMUM TEN (10) WORKING DAYS AFTER SUBMITTAL DATE. REQUIRED
ON SITE-CONSTRUCTION PLANSI SANITARY FACILITIES AND 1 DUMPSTER. All commercial requirements
must meet compliance.
· Prints to be left =or review must pay application fee of $.03 per square foot ($15 min);
