CONDITIONS OF PERMIT AFFIDAVIT
A. NOTICE OF pEED RESTRICTIONS
The undersigned understands that this permit may be subject to ~deed restrictions" which
may be more restrictive than City regulations. The undersigned assumes responsibility for
compliance with any applicable deed restrictions.
B. UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES
If the owner has hired a contractor or contractors to undertake work, they may be required
to be licensed in accordance with state and local regulations. If the contractor is not
licensed as required by law, both the owner and contractor may be cited for a misdemeanor
violation under state law. If the owner or intended contractor are uncertain as to what
licensing requirements may apply for the intended work, they are advised to contact the
city of Zephyrhills Building Department, 813-788-6611.
Furthermore, if the owner has hired a contractor or contractors, he is advised to have the
eonttactor(s) sign portions of the ~Contractor Sections" of this application for which they
will be responsible. If you, as the owner signs as the contractor, you are indicating that
you, rather than the contractor, are responsible for the work. If the contractor wishes
you to sign as contractor that may be an indication that he is not properly licensed and is
not entitled to permitting privileges in the City of Zephyrhills.

I also certify that, if fill material is to be used in Flood Zone ~A" or ~A,etc.", it is
understood that a drainage plan addressing a ~compensating volume" will be submitted which
is prepared by a professional engineer registered in the State of Florida prior to permit
issuance.
A permit issued shall be construed to be a license to proceed with the work and not as
authority to violate, cancel, alter, or set aside any provisions of the technical codes,
nor shall issuance of a permit prevent the Building Official from thereafter requiring a
correction of errors in plans, construction, or violations of any code. Every permit
issued shall become invalid unless the work authorized by such permit is commenced within
six months of issuance, or if work authorized by the permit is suspended or abandoned for a
period of six months after the time the work is commenced. One 90 day extension of time
may be allowed for the perMit with fee charge of $15.00. The extension shall be requested
in writing to the Building Official. An approved inspection must be logged during each six
month period, or the project will be considered abandoned.

54.0 Power-Bond
54.1 Introduction
The Power-Bond Epoxy Pump System consists of a self
contained pump style cartridge in which the adhesive
components are mixed. A standard manually operated
caulking tool can be used in the installation of thread-
ed rod in solid concrete and masonry materials.
Power-Bond epoxy bonds the anchor rod to the base
material so no expansion forces are exerted against
the walls of the hole in the base material. This makes
the mortar ideal for use in anchoring to a variety of
base materials ranging from soft common brick to
hard marble or granite.
54.2 Product Description
The Power-Bond Epoxy Pump System is supplied in 10
fluid ounce cartridges to meet the needs of most
smaller applications. For easy pumping, a lower viscos-
ity epoxy material is used as opposed to the paste style
epoxies used in side by side cartridge systems. Once
the material in the cartridge is mixed, it must be used
within the specified gel time. The cartridge is
designed for use in a standard heavy duty caulking
gun.

Each pump cartridge contains pre-measured amounts
of epoxy resin and hardener which are separated by a
set of special engineered plastic barrier rings. As the
pump is actuated. the barrier rings are pulled to the
top of the cartridge allowing the components to flow
together. As the cartridge is pumped, the compo-
nents are mixed together neatly and safely. Once the
epoxy has been thoroughly mixed, the plunger handle
is pulled up to extend the built-in mixing nozzle. The
handle portion of the nozzle is then removed and the
cartridge is ready for use. Since the epoxy must be
used within the specified gel time once the cartridge is
actuated, the best installation method is to prepare
the anchor holes in advance and have the anchor rods
ready prior to mixing the components.
Power-Bond epoxy can be used anchor threaded rod,
bolts, and reinforcing bars into concrete and other
solid base materials. It is not recommended for use
with screen tubes due to its low viscosity.
54.3 Mixing Guidelines
Pull plunger handle in the direction
shown until the white barrier rings
reach the top of the cartridge. Pull
hard to insure divider has been
opened and pulled to the top of the
cartridge.
While holding the bottom of the car-
tridge, mix the epoxy by pumping the
plunger handle. Be sure to use full
strokes to insure the material is fully
mixed. Do not use material that is not
mixed properly.
Upon completion of mixing, pull the
plunger handle in the direction shown
to extend the built-in nozzle. Be sure
the plunger is completely extended.
Unscrew the handle portion of the
plunger. The nozzle has an 0.0. of
0.420 inches.
Insert into a caulking gun and dis-
pense material. For use in anchoring,
follow proper hole preparation proce-
dures. Material must be allowed to
cure for the specified time prior to
loading.

54, 3 Power~Bond Material Properties
The Power-Bond adhesive is a low viscosity amine
based epoxy for use in a pump style cartridge. The
performance criteria for use as an anchoring system
for threaded rods and reinforcing bars are described
in the sections that follow.

54.3,1 Effect Of Elevated Temperature
As with all adhesive anchors, the bond strength of the
Power-Bond adhesive mortar is effected by elevated
temperatures in the base material. As the tempera-
ture of the base material increases, the bond strength
of the anchor will decrease. Typical performance of
the Power-Bond adhesive mortar at elevated base
material temperatures is shown below. The values are
based on maintaining the concrete test samples used
at a given temperature for a minimum of 24 hours
before applying a test load.

The resistance of the cured Power-Bond mortar to var-
ious chemicals was determined according to ASTM
Specification (-581 by laying molded samples of the
resin in the respective chemical agents. The samples
were subjected to a bending strength test before and
after a 12 month exposure to the chemicals. The
adhesive was rated as resistant if there was no visible
deterioration and less than 25 % reduction in bending
strength. This exposure is extreme. Under normal
installation conditions, the adhesive is exposed to the
chemical agents only at the surface of the concrete
around the top of the anchor hole.

54.8 Suggested Specification
The adhesive used shall be a two component, low vis-
cosity epoxy which is dispensed from a pump type car-
tridge. The cartridge shall have engineered plastic bar-
rier rings which keep the components sealed until they
are mixed in a built-in dispensing nozzle. The barrier
rings shall consist of a dual co-axial mechanism which
automatically opens as the cartridge is actuated. A four
section mixing blade assembly shall be used to com-
bine the components of the epoxy as the cartridge is
pumped. The epoxy shall be thoroughly mixed prior to
dispensing. Upon completion of pumping, the nozzle
shall be extended and the plunger handle shall be
removed from it. The adhesive system used shall be
Power-Bond as supplied by Powers Fastening, Inc.
Anchor holes shall be drilled with a bit meeting the
requirements of ANSI Standard B212.15 and shall be
approved by the manufacturer. The adhesive and the
hardware components shall be installed according to
the manufactures written instructions. The threaded
anchor rod (or reinforcing bar) used shall meet the
requirements of . The hardware used

5433 Power-Bond™ Epoxy Material Setting Time
The table below lists the setting times for the Power-
Bond epoxy. The times listed vary according to the
volume of epoxy and base material temperature. The
gel time is the maximum working time. At the initial
cure time, the epoxy can sustain a design load; how-
ever. this should be verified by job site testing. The full
curing time is the minimum time required for the
epoxy to meet its physical properties.

Blow the hole clean with com-
pressed air, brush the hole, and

dry or damp, but should be free
of standing water or frost. If

54.5 Power-Bond™ Estimating Guide
The following tables list the estimated number of
anchors which can be installed per Power-Bond pump
cartridge, The actual number of anchors dispensed
may vary depending upon job site installation proce-
dures and waste. To estimate the number of cartridges
which may be required, divide the number of anchors
to be installed by the figures listed below.
For example, to install 2,000 3/4" threaded anchor rods
in a hole 6" deep, the estimated number of anchors is
9.2 per cartridge. The estimated number of cartridges
is 2000 divided by 9.2 or 217 cartridges.

strength anchor rod. The capacity of the anchor rod itself should be taken into account in the design of any anchorage.

The bond strengths in the following table are based on applying a safety factor of 4 to the ultimate load capacities of
the Power-Bond Adhesive injection gel for three concrete compressive strengths. The allowable steel strength values are
based on the AISC Manual of Steel Construction (Ninth Edition). Values for B-7 steel rod are based on A325 capacities.
The allowable load used should be the lesser of the bond or steel strength.

54.7.4 A!lowilble Load Capacities for Installations Using Reinforcing Bars Rod in Concrete
The values for the bond strength in tension are based on applying a safety factor of four to the ultimate capacities listed
in the previous section. The values for the bond strength in shear are based on applying a safety factor of four to the
ultimate shear load capacities for the tests conducted with high strength threaded rod. The tension loads for reinforcing
bars are based on the allowable stresses as listed in Section 1926.3.2 of the Uniform Building Code. The shear loads for
reinforcing bars are based on the allowable stresses listed in Section 1926.7.6 and 1911.7.4. The shear-friction coefficient
based on section 1911.7.4.3 for normal weight concrete was 0.6. The allowable shear load is based on applying the
allowable shear stress to the cross sectional area of the bar and then factoring by the shear-friction coefficient. The
allowable load should be the lesser of the bond strength or the steel strength.
