A. NOTICE OF DEED RESTRICTIONS
The undersigned understands that this permit may be subject to ~deed restrictions" which
may be more restrictive than City regulations. The undersigned assumes responsibility fo~
compliance with any applicable deed restrictions.
B. UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES
If the owner has hired a contractor or contractors to undertake work, they may be required
to be licensed in accordance with state and local regulations. If the contractor is not
licensed as required by law, both the owner and contractor may be cited for a misdemeanor
violation under state law. If the owner or intended contractor are uncertain as to what
licensing requirements may apply for the intended work, they are advised to contact the
City of Zephyrhills Building Department, 813-788-6611.
Furthermore, if the owner has hired a contractor or contractors, he is advised to have the
contractor(s) sign portions of the ~Contractor Sections" of this application for which they
will be responsible. If you, as the owner signs as the contractor, you are indicating that
you, rather than the contractor, are responsible for the work. If the contractor wishes
you to sign as contractor that may be an indication that he is not properly licensed and is
not entitled to permitting privileges in the City of Zephyrhills.

Application is hereby made to obtain a permit to do work and installation as indicated. I
certify that no work or installation has commenced prior to issuance of a permit and that
all work will be performed to meet standards of all laws regulating construction, City
codes, zoning regulations, and land development regulations in the jurisdiction. I also
certify that I understand that the regulations of other governmental agencies may apply to
the intended work, and that it is my responsibility to identify what actions I must take to
be in compliance. Such agencies include but are not limited to: *Department of

I also certify that, if fill material is to be used in Flood Zone ~A" or ~A,etc.", it is
understood that a drainage plan addressing a ~compensating volume" will be submitted which
is prepared by a professional engineer registered in the State of Florida prior to permit
issuance.
A permit issued shall be construed to be a license to proceed with the work and not as
authority to violate, cancel, alter, or set aside any provisions of the technical codes,
nor shall issuance of a permit prevent the Building Official from thereafter requiring a
correction of errors in plans, construction, or violations of any code. Every permit
issued shall become invalid unless the work authorized by such permit is commenced within
six months of issuance, or if work authorized by the permit is suspended or abandoned for a
period of six months after the time the work is commenced. One 90 day extension of time
may be allowed for the permit with fee charge of $15.00. The extension shall be requested
in writing to the Building Official. An approved inspection must be logged during each six
month period, or the project will be considered abandoned.

Commercial Fire Alarm System, UL, with panel, battery, heat and smoke detector
strobes and horn strobe, pull stations, fire wire, boxes, conduit, mise
connectors, etc. System will meet NFPA 72, Lifesafety 101 business occupancy,
and all other state and local fire codes, and be installed by a state licensed
fire alarm dealer. All wiring to be installed above ceilings and behind walls
per code, any wiring that cannot be installed behind walls for lack of space,
will be surface mounted in conduit. We will provide all submittals, obtain
necessary permits and local licensing, tag and certify installation, and get
final fire marshal approval of system. We will provide install & owner man-
uals, fire system log book, plackards, and approved drawings. We are covered
by liability insurance and workers comp. Code does not require off-site
monitoring of system, should customer choose this option, there will be an
additional charge. System quoted per plans as given by owner, any changes of
that plan that will require additional alarm components, will be accomplished
by signed change orders.
Customer will provide: (1) a 110v dedicated power line from electric panel to
the alarm panel, for primary power, (2) all info needed to obtain proper build
ing permits, (3) temp on-site elect power for install.
System will be prewired as soon as permits are obtained, component trim
when major part of renovation is complete, so as not to damage system.

(8) nationally and internationally recognized alerting sounds: Horn, Bell, March Time Horn,
Code-3 Tone, Code-3 Horn, Slow Whoop, Siren or Hi/lo Tone. Our Code-3 horn and tone
patterns are engineered to comply with NFPA-72 (1993) requirement of Temporal Pattern
Code-3 with an effective date of July '96 without requiring additional equipment. Synchronized
strobe versions are available.
Features
o Approvals include: Underwriters Laboratories UL 1971 and UL 464 Listings, FCC Part 15,
Factory Mutual (FM), California State Fire Marshal (CSFM), New York City (MEA), Chicago (BFP),
European Community (CE) approvals on all models.
o Designed to meet or exceed NFPA/ANSI Standards and ADA Accessibility Guidelines. Meets
OSHA 29 Part 1910.165.
o One alarm appliance with (8) eight selective signals to provide superior sound penetration for
various ambient and wall conditions with two field selectable sound output levels.
o Code-3 Horn and Tone meet ANSI/NFPA temporal pattern for standard emergency evacuation
signaling.
o Audible and strobe can operate from a single NAC circuit with any of the (8) audible sounds.

o Series MT Appliances have IN and OUT wiring terminations that accept two #12 to #18 American Wire
Gauge (AWG) wires at each terminal. Inputs are polarized for compatibility with standard reverse
polarity type supervision.
o Selectable input voltage on non-strobe DC versions. Strobe versions are factory set for either 12 VDC,
24 VDC or 115 V AC.
o Mounts to either 4" square or double gang boxes (important for retrofit installations). Attractive flush

Strobes are designed to flash at 1 flash per second minimum from 20-31 VDC (for 24 VDC models) or 12-15.6 VDC (for 12 VDC models). Note that ADA guidelines presently specify a
flash rate of 1 to 3 flashes per second and NFPA-72 (1996) specifies a flash rate of 1 to 2 flashes per second. Check the minimum and maximum output of the power supply and standby
battery and subtract the voltage drop from the circuit wiring resistance to determine the applied voltage to the strobes.
2. 115 VAC models have been UL tested to operate from 96 VAC to 132 VAC. Strobe candela at -350 Centigrade is reduced to 7.5 candela.
3. Anechoic dBA is measured on axis in a non-reflective (free field) test room using fast meter response. For peak dBA (measured with peak meter response), add 5 dBA to typical anechoic
values shown in Table 1 and 2.
4. Reverberant dBA is a minimum UL rating based on sound power measurements in a reverberant test room.

Enhancements
Wheelock's Series RS Strobes and Series RSS Non-Sync/Sync Strobes have been enhanced with
Lower Current Draw and the Series RSS has ZERO Inrush while maintaining their outstanding
performance, reliability and cost-effectiveness in meeting or exceeding the latest requirements of NFPA
72 (National Fire Alarm Code) 1996, ANSI 117.1 (American National Standard for Accessible and Usable
Building and Facilities), and UL Standard 1971 (Signaling Devices for the Hearing Impaired). RS/RSS
Strobe Appliances, when properly specified and installed in accordance with NFPA/ANSI Standards, can
provide the Equivalent Facilitation allowed under ADA Accessibility Guidelines (ADAAG General
Section 2.2) by meeting or exceeding the ,illumination which results from ADA's strobe intensity guidelines
of 75 candela at 50 feet. This is an illumination of 0.030 lumens per square foot.
Wheelock's Series RS/RSS Strobes employ an integral Patented Strobe Mounting Plate that makes it
easy to mount to a variety of backboxes. The strobes can be mounted to single-gang, double-gang,
4" square, 100 mm European backboxes, or the SHSS backbox (Order Code 7254) for surface
mounting. An attractive cover plate is provided for a clean, finished appearance on all models.
Wheelock's Series RS Strobes are available with 15 and 15/75 candela intensities for Wall mount only
and non-sync applications.
The Series RSS Strobes NOW include Non-Sync and Sync in ONE appliance. The SM or DSM Sync
Module must be used to achieve Sychronization of the strobe. Synchronized strobes can eliminate
possible restrictions on the number of strobes in the field of view. Wheelock's synchronized strobes offer
an easy way to comply with ADA recommendations concerning photosensitive epilepsy. The
strobe options for the Series RSS are 15, 15/75, 30, 75 and 110 candela intensities for Wall mount and
15, 30, 75 and 100 candela intensities for Ceiling mount applications.

Since the inception of UL 1971, strobes cannot be Listed for outdoor use, Wheelock
offers WM3T strobes for outdoor installation requiring weatherproof devices and
private mode operation where UL 1971 strobes are not required. They are UL 1638
Listed at 117 cd and are designed for surface mounting indoors or outdoors.

General Deswlptlon
Model 2100D is a 2-wlre pholoelectronic smoke deteclor
that uses a state-of-the-art oplical sensing chamber. Tnis
detector is designed to provide open area prolection and to
be used With compatible UL-listed panels only. Model
2100TD features a restorable. built-in. fixed-temperalure
(1350 F) thermal detector.

Installalion of these detectors is simplified by the use of a
mounting bracket and a plug-in screw terminal block th~t
can be prewired to the syslem, allowing the detector to be
easily instaUed or removed for cleaning. The delector's sen-
sitivity can be tested in place using th~ M0D400R Test
Module. An LED on the detector provides a local visual in-
dication of lhe detector's slalus. If power is applied to lhe
detector. and it is funclioning normally in standby, the sta-
tus LED blinks every len seConds. The LED also lalches on
in alarm.
Models 2100D and 2100TD feature a visual indication thai
maintenance is required - if the seming chamber drifts out
oC its sensitivity limits, the LED cease~ to blink.
The delectors also include an OUlPUl that allows an
optional Model RA400Z Remore Annunciator 10 be
connected.

GENERAL
The Fire-Lite BG-I0 Series manual alarm stations are
non-coded and dual action.
FEATURES

the station has been operated.

APPLICATIONS
Use In provide a convenient means to manually initiate a
fire ahmn.
CONSTRUCTION
. Shell, door, andh;mdle are molded of durable Lexan~with
a lexturcd finish.
. Back plate made of 16 ga. steel.
. Switch contacts are normally open.
INSTALLATION
For semi-flush mounting, attach directly to a standard single-
gang electrical box. Terminal block with screws allows
quick installation,
OPERATION
Pulling the handle down causes it to latch in the down
position and to close the normally-open switch. The handle
is restored manually by unscrewing the alIen head screw so
tl1e:!op of tl1e case can pivot forward, allowing the spring-
loaded handle to return to its normal position. The case can
tl1en be pi voted back to its normal position and secured
with the allen-head screw.
The BG-l OL utilizes a mechanical key instead of the allen-
head screw.
The BG-l 0 Pff has a normally-open auxiliary switch which
is closed by rotating a key clockwise. (The key is non-
removable in this position.)
