': "February 6, 2002 Mr. Todd Vande Berg Director of Development City of Zephyrhills 5335 8th Street Zephyrhills, FL 33540 RE: Ben's Carwash Completion Status TRE Project No. 98277 Dear Mr. Vande Berg: The above-referenced site is substantially complete including landscaping and irrigation and is in substantial conformance with the site plan as approved by the City of Zephyrhills. There are a few minor deviations from the original site plan, but the site still conforms to the Land Development Code. Specific deviations that we have noted include the following: 1. Onsite driveway aisle widths were reduced from 12 feet to 10 feet for a total onsite driveway width of 20 feet. There h only one row of parallel parking spaces and the aisle is one-way. The City of Zephyrhills Land Development Code calls for a minimum aisle width of 14 feet for one-way parallel lots with spaces on each side of the driveway. With a 2D-foot aisle and parking on only one side, the site meets and exceeds the Code. The benefit of this rhange is that there is less pavement and thus more green space. 2. The dumpster pad was moved from its originally designed position. However, this change was coordinated between City staff and the Owner. 3. The driveway approaching the tunnel entrance was widened moderately to facilitate vehicle entry. As you mentioned during our telephone conversation today, FDOT is waiting for a few minor restoration issues to be resolved. The items of concern have been addressed by the Owner, but must be reviewed by this office, noted on Record Drawings and submitted to FDOT with a final Record Drawing submittal form. This letter is issued as a follow-up to that letter, with some of those concerns reiterated and some new ones added. Items noticed during a recent site visit during which Bill Burgess of the City of Zephyrhills expressed some concems prompted this letter. This letter expresses some of the concerns expressed by Mr. Burgess in the field, along with some other items that carne to our attention. 1. Reinforcing bars were added to the north retaining wall of the pond (common wall with building) by drilling and epoxy grouting. Why were the bars retrofitted in this manner, instead of having been installed contiguous with the footer reinforcing? Were these bars retrofitted correctly (i.e., do the drilling penetrations go deep enough, etc.)? 2. Reinforcing bars were cut off in the concrete channel within the car wash tunnel. New bars were added by drilling and epoxy grouting. Again, there are questions as to whether this installation was done properly. The channel is an important structural component and the continuity of contiguous bars could be critical to its structural integrity. We recommend evaluation by a structural engineer. 3. Have you ordered any concrete cylinder tests? This is the only way you will know whether the concrete used meets the specified strength. This recommendation was made in our previous correspondence. 4. Have you ordered any compaction tests? This is the only way you will know if the ground supporting important structural components of your structure was compacted to the specified dry density. 5514 7th Street . There is concern on the part of some in our office who happened to drop by during one of the concrete pours that the concrete used in the poured channel and walls may have had a high slump, possibly because of too much water in the mix. Again, cylinder tests are important to ensure the quality of concrete used. 6. The steel sleeves through the pond walls are at angles to the pipes installed within them. 7. The plans call for Link Seals between the storm pipes and the sleeves through which they run. None are installed. 8. As was previously mentioned, the blocks within curved sections of the pond walls are stacked, not laced. The structural ramifications of this are not known. We recommend evaluation by a structural engineer. 9. There is no way of knowing if adequate reinforcing was placed on the top course of the retention pond retaining walls. As indicated in our last letter, during previous observations it was noted that only one bar was placed on top of the wall as opposed to two as called for by the plans. 10. During recent observations while driving by the site, it appeared that the curved section of the retaining wall facing 7th Street leans toward the inside of the pond at the top. 11. The small radii of the pond are constructed with half-blocks. Extra reinforcing probably should have been used in these areas. 12. There appears to be no vibrating equipment on the site for use in ensuring adequate filling of the block cells. . 13. Reinforcing bars in the south wall of the building are not spaced in accordance with the foundation plan drawings between window openings. 14. Reinforcing bars appear to be missing in the offset block wall for vending machines. 15. During a recent stop at the site we noted a large hole in the middle of the pad area for the vending machines. We were told this was the tooter excavation. The footer should be centered with respect to the proposed wall and reinforced in accordance with the plans. 16. There are still washouts on the neighboring car wash site. All disturbed earth on the neighboring side should be restored, compacted, shaped and sodded as soon as possible, both for safety and aesthetic considerations. 17. As mentioned in the last correspondence, the retention ponds must be cleaned of all excess concrete, fine silty material and debris as a final step. 19. Buried rubble near the block walls should be removed. Again, as I mentioned in our last letrer, we recognize that you have not retained us to provide frequent observation services at your site, and we have not invoiced you, as yet, for our time. We have simply noted these things while driving by or during informal visits. We mention them to you strictly out of concern for your investment. However, we feel you need some independent oversight at the job-site to better ensure the quality of work. We would recommend laboratory testing on all future concrete work and would recommend excavating and backfilling the future pavement areas a second time so that you can ensure that the material , will be brought to grade in sequential, compacted l~. We would recommend that each lift be tested to ensure that it attains at least 98 % of the maximum dry density of the material in accordance with the modified proctor. Finally, we recommend that the structural engineer of record for this project be called in to inspect work completed to date to evaluate its structural integrity and make recommendations for additional retrofit work, if necessary, to ensure the structure's stability. We bring these things to your attention for your protection and for the ,protection of your end product. Undoubtedly, you have invested significant time and resources in this project, and we want the final product to be successful. Victory Terrace Apartments - Final Site Plan Staff reviewed the revised site plan submitted by Ed Rogers for the proposed apartments that will be located on the west side of Airport Road and south of South A venue. Staff visited all issues that were raised from the previous meeting. The Owner is proceeding with the changes because of the late discovery of this incompatibility and the impending proposed opening date. The change results in additional retaining wall construction, which will be of the same design cross-section as the original. ENGINEERING, PLANNING, ENVIRONMENTAL PERMITTING October 31,2001 Mr. Bill Burgess City of Zephyrhills 5335 8th Street Zephyrhilffi,FL 33540 RE: Ben's Car Wash Slab Construction/ Dowels TRE Project No. 98277 Dear Bill: It has come to my attention that the Contractor for the above-referenced project is ready to pour the slab and is waiting on the City to inspect the reinforcing prior to concrete placement, which is scheduled for 7:00 tomorrow morning. The Owner and Contractor have indicated that the City has questioned the use of epoxy-grouted dowels on the job, probably as a result of my recent letter. My letter questioned the use of the dowels, particularly with regard to their location and depth. I have since observed placement the dowels at the jobsite and have verified that new dowels have been placed at the appropriate depth. Typically, good retrofit dowel placement calls for placement of the dowels at a depth of approximately one inch per eighth-inch of dowel diameter. No.5 bars, for example, should be placed at a depth of 5 inches. The holes for the dowels whose installation we observed were drilled with an 8-inch bit and have thus been placed at a depth of 8 inches. Placement of dowels at an appropriate depth and subsequent grouting with an appropriate epoxy material is an acceptable practice. The dowels were grouted with an acceptable grout manufactured for the purpose for which it was used. We noted that some dowels (in the 2-foot spaces between the 8-foot window openings) were missing, but the contractor was in the process of installing them while we were on site. At the rate the work was progressing, we would expect all of the dowels to be in place before the close of business today and should be in place in time for you to inspect them. Based upon our observations, it appears that the dowels, as placed, are in the proper location. There are actually a few more than called for on the plans. (5) The agency shall promptly give written notice to all parties of the action taken on the petition, shall state with particularity its reasons if the petition is not granted, and shall state the deadline for filing an amended petition if applicable. All activities shall be implemented as set forth in the plans. specifications and performance criteria as approved by this permit. Any deviation from the permitted activity and the conditions for undertaking that activity shall constitute a violation of this permit. ' 2. This permit or a copy thereof , complete with all conditions. attachments. exhibits. and modifications, shall be kept at the work site of the permitted activity. The complete permit shall be available for review at the work site upon request by District staff. The Permittee shall require the contractor to review the complete permit prior to commencement of the activity authorized by this permit. 3. Activities approved by this permit shall be conducted in a manner which does not cause violations of state water quality standards. The Permittee shall implement best management practices for erosion and a pollution control to prevent violation of state water quality standards. Temporary erosion control shall be implemented prior to ~ and during construction, and permanent control measures shall be completed within 7 days of any construction activity. Turbidity barriers shall be installed and maintained at all locations where the possibility of transferring suspended solids into the receiving waterbody exists due to the permitted work. Turbidity barriers shall remain in place at all locations until construction is completed and soils are stabilized and vegetation has been established. Thereafter the Permittee shall be responsible for the removal of the barriers. The Permittee shall correct any erosion or shoaling that causes adverse impacts ,to the water resources. 4. Water quality data for the water diSCharged from the Permittee's property or into the surface waters of the state shall be submitted to the District as required by the permit. Analyses shall be performed according to procedures outlined in the current edition of Standard Methods for the Examination of Water and Wastewater by the American Public Health Association or Methods for Chemical Analyses of Water and Wastes by the U.S. Environmental Protection Agency. If water quality data are required. the Permittee shall provide data as required on volumes of water discharged. including total volume discharged during the days of sampling and total monthly volume discharged from the property or into surface waters of the state. 5. District staff must be-notified in advance of any proposed construction dewatering. If the dewatering activity is likely to result in offsite discharge or sedimenttransport into wetlands or surface waters, a written dewatering plan must either have been submitted and approved with the permit application or submitted to the District as a permit prior to the dewatering event as a permit modification.' A water use permit may be required prior to any use exceeding the thresholds in Chapter 40D-2, FAC. 6. Stabilization measures shall be initiated for erosion and sediment control on disturbed areas as soon as practicable in portions of the site where construction activities have temporarily or permanently ceased. but in no case more than 7 days after the cQnstruction activity in that portion of the site has temporarily or permanently ceased. 7. Off-site discharges during construction and development shall be made only through the facilities authorized by this permit. Water diSCharged from the project shall be through structures having a mechanism suitable for regulating upstream stages. Stages may be subject to operating schedules satisfactory to the District. 8. The Permittee shall complete construction of all aspects of the surface water management system, including wetland compensation (grading, mulching, planting), water quality treatment features. and discharge control facilities prior to beneficial occupancy or use of the development being served by this system. This permit does not convey to the Permittee or create in the Permittee any property right, or any interest in real property, nor does it.authorize any entrance upon or activities on property which is not owned or controlled by the Permittee, or convey any rights or privileges other than those specified in the permit and Chapter 400-4 or Chapter 400-40, FAC. 19. The Permittee is hereby advised that Section 253.n, F.S., states that a person may not commence any excavation, construction, or other activity involving the use of sovereign or other lands of the state, the title to which is vested in the Board of Trustees of the Intemallmprovement Trust Fund without obtaining the required lease, license, easement, or other form of consent authorizing the proposed use. Therefore, the Permittee is responsible for obtaining any necessary authorizations from the Board of Trustees prior to commencing activity on sovereignty lands or other state-owned lands. 20. The Permittee shall hold and save the,District harmless from any and all damages, claims, or liabilit!~s which may arise by reason of the activities authorized by the permit or any use of the permitted system. 21 . _ Any delineation of the extent of a wetland or other surface water submitted as part of the permit application, including plans or other supporting documentation, shall not be considered binding unless a specific condition of this permil or a formal determination under section 373.421 (2), F.S., provides otherwise. 22. The Permittee shall notify the District in writing within 30 days of any sale, conveyance, or other transfer of ownership or control of the petmitted system or the real property at which the permitted system is located. All transfers of ownership or transfers of a permit are subject to the requirements of Rule 400-4.351, FAC. The Permittee transferring the permit shall remain liable for any corrective actions that may be required as a result of any permit violations prior to such sale, conveyance or other transfer. 23. Upon r :.lscnable notice to the Permittee, District authorized staff with proper identification shall have permission to enter, inspect, sample and test the system to insure conformity with District rules, regulations and conditions of tho pFlrmit::i. 24. If histor:.;al or archaeological artifacts are discovered at any time on the project site, the Permittee shall immed:ntely notify the District and the Florida Department of State, Division of Historical Resources. 25. The Permitte'J shall immediately notify the District in writing of any previously submitted information that is later discovered to be inaccurate.
