A. NOTICE OF DEED RESTRICTIONS
The undersigned understands that this permit luay be subject to ~deed restrictions" which
may be more restrictive than City regulations. The undersigned assumes responsibility for
cofupliance with any applicable deed restricti'ons.

B; UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES
If the owner has hired a contractor or contraGtors to undertake work, they may be required
to be licensed in accordance with state and local regulations. If the contractor is not
licensed as required by law, both the owner and contractor may be cited for a misdemeanor
violation under state law. If the owner or intended contractor are uncertain as to what
licensing requirements may apply for the intended work~ they are advised to contact the
City of Zephyrhills Building Department, 813-788-6611.
Furthermore, if the owner has hired a contractor or contractors, he is advised to have the
contractor(s) sign portions of the ~Contractor Sections" of this application for which they
will be responsible. If you, as the owner signs as the contractor, you are indicating that
you, rather than the contractor, are responsible for the work. If the contractor wishes
you to sign as contractor that may be an indication that he is not properly licensed and is
not entitled to permitting privileges in the City of Zephyrhills.

I certify that I, the applicant, have been provided with a copy of ~Florida's Construction
lien Law - Homeowner's Protection Guide" prepared by the Florida Department of Agriculture
and Consumer Affairs. If the applicant is someone other that the ~owner", I cerify that I
have obtained a copy of the above described document and promise in good faith to deliver
it to the ~owner" prior to commencement.
E. CONTRACTOR'S/OWNER'S AFFIDAVIT
I certify that all the information in this application is accurate and that all work will
be done in compliance with all applicable laws regulating construction, zoning, and land
development.
Application is hereby made to obtain a permit to do work and installation as indicated. I
certify that 00 work or installation has commenced prior to issuance of a permit and that
all work will be performed to meet standards of all laws regulating construction, City
codes, zoning regulations, and land development regulations in the jurisdiction. I also
certify that I understand that the regulations of other governmental agencies may apply to
the intended work, and that it is my responsibility to identify what actions I must take to
be in compliance. Such agencies include but are not limited to: *Department of
Environmental Regulation-Cypress Bayheads, Wetland Areas and Environmentally Sensitive
Lands, Water/Wastewater Treatment
*Southwest Florida Water Management District-Wells, Cypress Bayheads, Wetland Areas,
Altering Watercourses
*Army Corps of Engineers-Seawalls, Docks, Navigable Waterways
*Department of Health & Rehabilitative Services, Environmental Health Unit-Wells,
Wastewater Treatment, Septic Tanks
*U.S. Environmental Protection Agency-Asbestos abatement
I also certify that, if fill material is to be used in Flood Zone ~A" or ~A,etc.", it is
understood that a drainage plan addressing a ~compensating volume" will be submitted which
is prepared by a professional engineer registered in the State of Florida prior to permit
issuance.
A permit issued shall be construed to be a license to proceed with the work and not as
authority to violate, cancel, alter, or set aside any provisions of the technical codes,
nor shall issuance of a permit prevent the Building Official from thereafter requiring a
correction of errors in plans, construction, or violations of any code. Every permit
issued shall become invalid unless the work authorized by such permit is commenced within
six months of issuance, or if work authorized by the permit is suspended or abandoned for a
period of six months after the time the work is commenced. One 90 day extension of time
may be allowed for the permit with fee charge of $15.00. The extension shall be requested
in writing to the Building Official. An approved inspection must be logged during each six
month period, or the project will be considered abandoned.

FIRST AMENDMENT TO THE
ARLENE V. LYKINS
REVOCABLE TRUST AGREEMENT
OF 1993
This FIRST AMENDMENT to the ARLENE V. LYKINS REVOCABLE TRUST

AGREEMENT OF 1993 (the "Trust") is made this /9 day of

1998, by and among ARLENE V. LYKINS, as Grantor of
the Trust, and ARLENE V. LYKINS and GARY R. LYKINS, as Co-Trustees
(the "Trustee").
WIT N E SSE T H:
WHEREAS, the Grantor entered into the ARLENE V. LYKINS
REVOCABLE TRUST AGREEMENT OF 1993, with herself and WHEELER
LYKINS, as Co-Trustees, on the 26th day of August, 1993; and
WHEREAS, pursuant to Paragraph A of Article IV of the Trust,
Grantor reserved the right to alter, amend, or revoke the Trust,
either in whole or in part, without the consent of the Trustee or
any beneficiary of the Trust, by delivering written notice to the
Trustee to that effect; and
WHEREAS, the Grantor, pursuant to said reserved power, wishes
to amend the Trust and appoint GARY R. LYKINS as Co-Trustee;
NOW, THEREFORE, the Grantor hereby amends the Trust as
follows:
1. Paragr'iph C of Article XVI of this Trust is hereby
revised such th It as revised it shall read in its entirety as
follows:
C. Successor Trustee. The current Co-Trustees of
the Trust shall be the Grantor and GARY R. LYKINS.
Upon Grantor's death, or if during the continuance of
this Trust, the Grantor becomes incapaci ta ted or shall
otherwise fail or cease to act as Trustee for any
reason, then the Grantor shall automatically be
succeeded by SUN TRUST BANK, NATURE COAST, Pasco
County, as Co-Trustee or sole Trustee, as the case may
be. After Grantor's death, no fewer than two trustees
shall serve hereunder at all times during the period
that a trust is in existence under paragraph E of
Article VI. Any successor Trustee automatically shall
have all the title, powers, and discretion herein given
the original Co-Trustees and automatically shall be
vested with all the powers and duties of this Agreement
without the necessity of any act, conveyance, or
transfer; provided, however, any successor Trustee
shall not be liable in any way for any acts or defaults
of any predecessor Trustee, nor for any loss or expense
from or occasioned by anything done or neglected to be
done by any predecessor Trustee, and the successor
Trustee shall be liable only for its own acts and
defaults in respect to the property received by it, and
it is relieved of all duties it otherwise may have to
examine into and satisfy itself as to the accounts of
any predecessor Trustee.

2. A new paragraph I of Article XXIII is added and shall
read in its entirety as follows:
I, The Grantor reserves the right during the
Grantor's lifetime to reside upon any real property
transferred to this trust that is the Grantor's
principal residence. It being the intent of this
provision to retain for the Grantor the requisite
beneficial interest and possessory right in and to such
real property to comply with section 196.041, Florida
Statutes, such that such beneficial interest and
possessory right consti tute in all respects "equitable
title to real estate" as that phrase is used in Section
6, Article VII, of the Consti tution of the State of
Florida, Notwithstanding any contrary provision of
this Agreement, the Grantor's interest in any such real
property in which the Grantor resides pursuant to the
provisions of this trust shall be deemed to be an
interest in real p~operty and not personalty and shall
be deemed the homestead of the Grantor.
3. In all other respects, except as hereinabove amended,
Grantor ratifies and confirms all of the provisions of Grantor's
said Revocable Trust of 1993, as executed on the 26th day of
August, 1993.
