A. NOTICE OF DEED RESTRICTIONS
The undersigned understands that this permit may be subject to ~deed restrictionsn which
may be more restrictive than City regulations. The undersigned assumes responsibility for
compliance with any applicable deed restrictions.
B. UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES
If the owner has hired a contractor or contractors to undertake work, they may be required'
to be licensed in accordance with state. and local regulations. If the contractor is not
licensed as required by law, both the owner and contractor may be cited for a misdemeanor
violation under state law. If the owner or intended contractor are uncertain as to what
licensing requirements may apply for the intended work, they are advised to contact the
City of Zephyrhills Building Department, 813-788-6611.
Furthermore, if the owner has hired a contractor or contractors, he is advised to have the
contractor(s) sign portions of the ~Contractor Sectionsn of this application for which they
will be responsible. If you, as the owner signs as the contractor, you are indicating that
you, rather than the contractor, are responsible for the work. If the contractor wishes
you to sign as contractor that may be an indication that he is not properly licensed and is
not entitled to permitting privileges in the City of Zephyrhills.

I certify that I, the applicant, have been provided with a copy of ~Florida's Construction
lien Law - Homeowner's Protection Guiden prepared by the Florida Department of Agriculture
and Consumer Affairs. If the applicant is someone other that the ~ownern, I cerify that I
have obtained a copy of the above described document and promise in good faith to deliver
it to the ~ownern prior to commencement.
E. CONTRACTOR' S/OWNER' S AFFIDAVIT
I certify that all the information in this application is accurate and that all work will
be done in compliance with all applicable laws regulating construction, zoning, and land
development.
Application is hereby made to obtain a permit to do work and installation as indicated. I
certify that no work or installation has commenced prior to issuance of a permit and that
all work will be performed to meet standards of all laws regulating construction, City
codes, zoning regulations, and land development regulations in the jurisdiction. I also
certify that I understand that the regulations of other governmental agencies may apply to
the intended work, and that it is my responsibility to identify what actions I must take to
be in compliance. Such agencies include but are not limited to: *Department of
Environmental Regulation-Cypress Bayheads, Wetland Areas and Environmentally Sensitive
Lands, Water/Wastewater Treatment
*Southwest Florida Water Management District-Wells, Cypress Bayheads, Wetland Areas,
Altering Watercourses
*Army Corps of Engineers-Seawalls, Docks{ Navigable Waterways
*Department of Health & Rehabilitative Services, Environmental Health Unit-Wells,
Wastewater Treatment, Septic Tanks
*U.S. Environmental Protection Agency-Asbestos abatement
I also certify that, if fill material is to be used in Flood Zone ~A" or ~A,etc.", it is
understood that a drainage plan addressing a ~compensating volume" will be submitted which
is prepared by a professional engineer registered in the State of Florida prior to permit
issuance.
A permit issued shall be construed to be a license to proceed with the work and not as
authority to violate, cancel, alter, or set aside any provisions of the technical codes,
nor shall issuance of a permit prevent the Building Official from thereafter requiring a
correction of errors in plans, construction, or violations of any code. Every permit
issued shall become invalid unless the work authorized by such permit is commenced within
six months of issuance, or if work authorized by the permit is suspended or abandoned for a
period of six months after the time the work is commenced. One 90 day extension of time
may be allowed for the permit with fee charge of $15.00. The extension shall be requested
in writing to the Building Official. An approved inspection must be logged during each six
month period, or the project will be considered abandoned.

CLOTHES DRYER EXHAUST
SMS04.1 Installation. Clothes dryers shall be exhausted in accordance with
the manufacturer's instructions. Dryer exhaust systems shall be
independent of all other systems and shall convey the moisture and any
products of combustion to the outside of the building.
SMS04.2 Exhaust penetrations. Ducts that exhaust clothes dryers shall
not penetrate or be located within any fireblocking, draftstopping or any
wall, floor/ceiling or other assembly required by the building code to be
fire-resistance rated, unless such duct is constructed of galvanized steel
or aluminum of the thickness specified in ~M603.3 and the fire-resistance
rating is maintained in accordance with the Florida Building Code,
Building.
SMS04.3 Cleanout. Each vertical riser shall be provided with a means for
cleanout.
SMS04.4 Exhaust installation. Dryer exhaust ducts for clothes dryers
shall terminate on the outside of the building and shall be equipped with a
backdraft damper. Screens shall not be installed at the duct termination.
Ducts shall not be connected or installed with sheet metal screws or other
fasteners that will obstruct the flow. Clothes dryer exhaust ducts shall
not be connected to a gas vent connector, gas vent or chimney. Clothes
dryer exhaust ducts shall not extend into or through ducts or plenums.
SMS04.5 Makeup air. Installations exhausting more than 200 cfm (0.09
m^3/s) shall be provided with makeup air. Where a closet is designed for
the installation of a clothes dryer, an opening having an area of not less
than 100 square inches (0.0645 m^2) shall be provided in the closet
enclosure.
SMS04.6 Domestic clothes dryer ducts. Exhaust ducts for domestic clothes
dryers shall have a smooth interior finish and the maximum developed length
shall not exceed 25 feet (7620 rom) from the dryer location to the outlet
terminal. The maximum length of the duct shall be reduced 2 1/2 feet (762
rom) for each 45-degree (0.79 rad) bend and 5 feet (1524 rom) for each 90-
degree (1.6 rad) bend. The exhaust duct shall be a minimum nominal size of
4 inches (102 rom) in diameter. The entire exhaust system shall be
supported and secured in place. The male end of the duct at overlapped
duct joints shall extend in the direction of airflow. Clothes dryer
transition ducts used to connect the appliance to the exhaust duct system
shall be limited to single lengths not to exceed 8 feet (2438 rom) in length
and shall be listed and labeled for the application. Transition ducts
shall not be concealed within construction. Developed duct lengths longer
than 25 feet (7620 rom) shall be allowed for specific dryer installations
where the dryer manufacturer's installation instructions specify the
allowable developed length of an engineered system.
5104.2,.6 Certificate of Protective Treatment for prevention of
termites. A weather resistant jobsite posting board shall be provid~d to ,
receive duplicate Treatment Certificates as each required protective
treatment is completed, providing a copy for the person the permit is
issued to and another copy for the building permit files. The Treatment
Certificate shall provide the product used, identity of the applicator,
time and date of the treatment, site location, area treated, chemical
used, percent concentration and number of gallons used, to establish a
verifiable record of protective treatment. If the soil chemical barrier
method for termite prevention is used, final exterior treatment shall be
completed prior to final building approval.
S104.2.7 Notice of termite protection. A permanent sign which identifies
the termite treatment provider and need for re-inspection and treatment
contract renewal shall be provided. The sign shall be posted near the
water heater or electric panel.
